OBJECTIVE: Sperm must be properly prepared in in vitro fertilization (IVF)-embryo transfer (ET) programs in order to control the fertilization rate and ensure that embryos are of high quality and have appropriate developmental abilities. The objective of this study was to determine the most optimal sperm preparation method for IVF. METHODS: Patients less than 40 years of age who participated in a fresh IVF-ET cycle from November 2012 to March 2013 were included in this study. Poor responders with less than three mature oocytes were excluded. Ham's F-10 medium or sperm-washing medium (SWM) was used in combination with the density-gradient centrifugation/swim-up (DGC-SUP) or SUP methods for sperm preparation. A total of 429 fresh IVF-ET cycles were grouped according to the media and methods used for sperm preparation and retrospectively analyzed (DGC-SUP/Ham's F-10, n=82; DGC-SUP/SWM, n=43; SUP/Ham's F-10, n=181; SUP/SWM, n=123). RESULTS: There were no significant differences among these four groups with respect to the mean age of the female partners, duration of infertility, number of previous IVF cycles, and retrieved oocytes. We determined that both the DGC-SUP and SUP methods for sperm preparation from whole semen, using either Ham's F-10 or SWM media, result in comparable clinical outcomes, including fertilization and pregnancy rates. CONCLUSION: We suggest that both media and both methods for sperm preparation can be used for selecting high-quality sperm for assistive reproductive technology programs.

Body surface potential map, an electric potential distribution on the body torso surface, enables us to infer the electrical activities of the heart. Therefore, observing electric potential projected to the torso surface can be highly useful for diagnosing heart diseases such as coronary occlusion. The BSPM for the heart of a patient show a higher level of sensitivity than 12-lead ECG. Relevant research has been mostly based on clinical statistics obtained from patients, and, therefore, a simulation for a variety of pathological phenomena of the heart is required. In this study, by using computer simulation, a body surface potential map was implemented according to various occlusion locations (distal, mid, proximal occlusion) in the left anterior descending coronary artery. Electrophysiological characteristics of the body surface during the ST segment period were observed and analyzed based on an ST isointegral map. We developed an integrated system that takes into account the cellular to organ levels, and performed simulation regarding the electrophysiological phenomena of the heart that occur during the first 5 minutes (stage 1) and 10 minutes (stage 2) after commencement of coronary occlusion. Subsequently, we calculated the bipolar angle and amplitude of the ST isointegral map, and observed the correlation between the relevant characteristics and the location of coronary occlusion. In the result, in the ventricle model during the stage 1, a wider area of ischemia led to counterclockwise rotation of the bipolar angle; and, during the stage 2, the amplitude increased when the ischemia area exceeded a certain size.

PURPOSE: This study was intended to investigate the types and seriousness of the couvade syndrome, pregnancy-related physical and psychological symptoms among expectant fathers whose spouses were pregnant. METHOD: The subject was consists of 100 expectant fathers at one hospital in Seoul, Korea. The pregnant women had not been diagnosed any medical complication. Data were analyzed by SPSS/PC program. RESULT: 1) The total mean score was 1.85: the mean score of perceived physical symptoms (1.87) revealed higher than the mean score of psychological symptoms (1.81). 2) With the respect to the general characteristics of subjects, there were statistically significant correlations between subject's level of education and couvade symptoms (r=-.209, p=.037), gestational age and couvade symptoms (r=-.227, p=.023), family total income and couvade symptoms (r=-.198, p=.048), perceived self health status and couvade symptoms (r=-.254, p=.011). 3) With the respect to the general characteristics of subjects, there were statistically significant differences in pregnant woman's age (t=1.363, p=.044),occupation of subject (F=3.594, p= .009), educational level of subject (t=3.506, p=.002), family total income (F=16.822, p= .000), perceived self health status (F=3.151, p=.047). CONCLUSION: Couvade syndrome is an issue for nurses who perform an important role in the care of pregnant women and their spouses.

PURPOSE: To investigate the relationships among mother's perception of her own newborn, her caring-confidence level, and her child-rearing attitude; and to provide data for developing effective nursing education programs on maternal newborn care. METHOD: Explorative survey questionnaires were completed by 159 married mothers of newborns from November 2003 to March 2004. Data were analyzed using SPSS/Win 10.0 by t-test and Pearson's Correlation coefficient. RESULTS: The mean scores of the mother's perception of her own newborn, her caring-confidence level, and her child-rearing attitude were .26, 3.13, and 3.43 respectively. There was a positive correlation between mother's perception of her own newborn and her child-rearing attitude. There was a significant correlation between mothers caring-confidence level and her child-rearing attitude. Score of caring-confidence was significantly different depending on the availability of helper and history of delivery. CONCLUSION: Availability of helper and history of delivery significantly affected mothers caring-confidence. This in turn, along with mothers perception of her own newborn, enhanced mothers child-rearing attitude. Thus, an effective education for mothers should include these factors.

PURPOSE:We report the results of image-guided percutaneous biopsies with a biopsy gun and evaluate the clinical usefullness. MATERIALS AND METHODS: One hundred and five biopsies under ultrasonographic or fiuoroscopic guidance were performed. Various anatomic sites were targeted (liver;50, chest;22, kidney;12, pancreas;8, intraperitoneum;7, retroperitoneum; ). RESULTS:Obtained tissue was diagnostic in 98 of the 105 biopsies(93%). In each instance, representative core tissue specimens were obtained. Evaluation of the core tissue by pathologist revealed consistent, uniform specimens that contained significant crush artifact in no case. Five biopsies yielded inadeguate tissues which were too small for histopathologic interpretation or were composed of necrotic debris. Two biopsies yielded adequate tissues, but tissues were not of the target. The diagnoses were malignancy in 77 biopsies and benign disease in 21 biopsies. No complications other than mild, localized discomfort were encountered except a transient hemoptysis and pneumothorax which was observed in two patients. CONCLUSION:Cutting biopsy with a biopsy gun provided sufficient amount of target tissue for an accurate diagnosis of malignant and benign disease. It was a safe and useful procedure for percutaneous biopsy.

BACKGROUND: The purpose of this study was 1) to review the length of an adequate period of conservative treatment and the appropriate surgical indications for herniated lumbar disc(HLD), 2) to collate the scientific evidences on surgeries for HLD, and 3) to collect expert opinions on HLD. METHODS: We searched for articles in PubMed, the Cochrane Library and KoreaMed up to 1 October 2008, and these articles were concerned with the natural history of HDL, systemic reviews of HDL and expert opinions on HLD. We also searched for meta-analyses and randomized or quasi-randomized controlled trials(RCTs or QRCTs) of surgery for HLD. We performed a meta-analysis using the Cochrane method. The survey consisting of 21 questions was delivered to all the members of the Korean Spinal Neurosurgery Society(KSNS) via E-mail. RESULTS: A total of 59 articles were included in this study. There were 16 articles concerning the period of conservative management and the surgical indications. Among the 33 articles on surgery for HLD, there were 4 meta-analyses, 27 RCTs, and 2 QRCTs. Among the 938 members of the KSNS, 72 responded to the survey. A minimum of 1 to 3 months of conservative management was the most preferred answer(58%), followed by a conservative management period of less than 1 month(33%). Percutaneous endoscopic discectomy was more preferred by the hospitals that specialized in spinetreatment than by the university hospitals(p<0.05). CONCLUSION: Conservative management for a minimum of 2 weeks to 3 months is recommended for patients with tolerable pain only. The patients with neurological compromise or intolerable pain should be considered for surgery. There is strong evidence on the relative effectiveness of surgical discectomy versus chemonucleolysis versus placebo. There is no scientific evidence on the effectiveness of any other form of minimally invasive procedure.

PURPOSE: The purpose of this study was to measure (the degree of) injury risk perception in preschool children. METHODS: The data were collected from child day care centers and kindergartens located in Seoul, and Gyeonggi and Kwangwon Province. A questionnaire consisted of 28 pictures was administered to 186 preschool children. RESULTS: The mean score for the injury risk perception was 21.83 (+/-3.89), and 77.98 converted into a 100-point scale. According to sub-categories, 'burn prevention' (.96+/-.13) was the highest, 'interpersonal safety' (.44+/-.31) was the lowest. There were significant differences in injury risk perception according to gender (t=-2.358, p=.019), age (t=-2.101, p=.037), experience of safety education (t=-3.719, p<.001), area of residence (t=-3.445, p=.001), injury experience (t=3.212, p=.002), and mother's occupation (t=-4.858, p<.001). The highest item in the percentage of correct answer item was 'making jump on the desk', the lowest item in the percentage of correct answer item was 'not wearing safety equipment when rollerblading'. CONCLUSION: Based on this study, studies should be continued to standardize the instrument. In addition, it is recommended that an injury prevention education program should be developed based on the results of this study to stimulate demand and interest.

Herpes simplex esophagitis can occur in those with normal immune function, but is more often seen in those who are immunocompromised. In one series, 5 percent of post-kidney transplant recipients had herpes esophagitis. We experienced a case of herpes simplex esophagitis, following renal transplantation in a 9 year old male. He complained of epigastric pain, nausea and blood-tinged vomiting. Endoscopic examination showed volcano ulcer, mucosal friability and multiple confluent ulcers covered by whitish exudates on elevated margin in the middle and lower esophagus. Microscopic findings revealed multinucleated giant cells, margination of chromatin, intense nonspecific inflammation and strong positive for herpes simplex virus immunohistochemical staining. Esophageal lesions and symptoms improved after acyclovir therapy.
