1.Heterotopic Pregnancy after In Vitro Fertilization and Embryo Transfer:The Role of Routine Transvaginal Ultrasonography.

PURPOSE: Spontaneous heterotopic pregnancy (HP) is a rare .condition, however, the rising incidence after in-vitro fertilization and embryo transfer (IVF-ET) may present serious problems as the diagnosis of this potentially fatal condition is often missed. The purposes of this study were to investigate the frequency of HP after IVF-ET, to analyze the clinical presentation and the outcome of coincidental intrauterine pregnancy, and to evaluate the value of routine transvaginal ultrasonography in early diagnosis and treatment of HP. MATERIALS AND METHODS: There were 11 cases of HP among 484 pregnancies after IVF-ET (The incidence was 2. 3%) in recent 2 years. We evaluated the clinical findings and the findings of transvaginal sonography in those 11 patients. RESULTS: The combined extrauterine pregnancies were tubal (n=9), cornual (n=l), and both the tubal and cornual (n=l). Vaginal bleeding was absent in 4 cases and one of them was asymptomatic. HP may be suspected if there was an elevated serum hCG in the absence of intrauterine multiple gestations. In all cases, the condition was diagnosed by transvaginal sonography performed prior to surgical intervention. The coincidental intr'auterine pregnancies were ended in successful full-term delivery in 7 cases, while one pregnancy was ongoing at that time. CONCLUSION: The incidence of HP was evidently higher in IVF-ET than in spontaneous pregnancies. Transvaginal sonography was useful in accurate diagnosis in clinically suspected cases and even in asymptomatic cases. Routine transvaginal sonography played an important role in early diagnosis and prompt management of HP.

2.A case of multiple trichoepithelioma.

A 21-year-old female ahd asymptomatic, flesh-colored, firm papules and nodules on the face, chest and back. 3.A case of nodular amyloidosis.

Localized primary cuteneous nodular amyloidosis is the rarest form of the cutaneous amyloidosis, which appears as single or multiple nodules on the extremities, trunk, genitalia or face. A 44-Year-old woman had asymptomatic, translucent brown to pink papules and nodules on the toes for 5 years. Histopathologic exarnination showed deposition to pale, eosinophilic, amorphous material throughout the dermis, Under the electron microscope, the material consisted of straight, nonbranching, nonanastoimosing filaments. We thus made the diagnosis of nodular amyoidosis.

4.Allergic contact dermatitis to hydroquinone in bleaching cream.

Bleaching creams containing hydroquinone are modestly effective in treating some cutane ous pigmentary disorders. Hydroquinone creams frequently cause mild irritant reactions and postinflammatory hyperpigmentation, but rarely allergic reactions. A case of allergic contact dermatitis due to hydroquinone in a 21-year-old woman was confirmed by the patch test.

5.Relationship of Shift Work to Gastrointestinal Disorders in Hospital Nurses.

BACKGROUND: This study researched the relationship between shift work and one of its relevant health problems, functional gastrointestinal disorders. The study targeted nurses working at a university hospital, and compared the differences in gastrointestinal disorders of shift workers and non-shift workers and identified causes that significantly influence these symptoms. METHODS: In August 2013, a self-report survey was conducted on nurses working at a university hospital. Among those sent out, 1,201 responded and 851 were used for the final analysis. RESULTS: It was shown that the frequency of gastrointestinal disorders in shift workers was 1.881 times higher than that of non-shift workers. Factors that influenced gastrointestinal disorders included age, marital status, work duration, type of employment, job satisfaction, and perceived health status. Gastrointestinal disorders were seen more often with older age, being married, longer duration of employment, non-regular worker, low job satisfaction, bad perceived health status, and shift works. CONCLUSIONS: To prevent gastrointestinal disorders in nurses who work shifts, a health checkup system should be in place for clinical nurses who are of older age and have longer employment records. To decrease the causes of gastrointestinal disorders, campaigns suggesting regular diet and quitting alcohol use should be held consistently. Also, a category related to gastrointestinal disorders should be added to the employee health checkup for nurses with long work experience.

6.The Factors that affect the Health Promoting Behaviors of Middle-aged Workers between Type D and Non-type D Personality.

PURPOSE: This study is to identify the factors that affect middle-aged workers' health promoting behaviors. METHODS: 214 middle-aged workers participated in surveys, and the data were collected from August to September 2012 and analyzed by using descriptive statistics, Pearson's correlation coefficient, and multiple regression analysis with PASW 18.0 program. RESULTS: It was found that among the factors, the degree of job stress (t=7.69, p<.001) and depression (t=6.23, p<.001) were significantly high for type D individuals, while non type D showed meaningful degree of self-related health status (t=-3.66, p<.001) and health promoting behaviors (t=-4.71, p<.001). The notable variable that affected the health promoting behaviors of the middle-aged workers was depression for both type D (beta=-.357, p= .029) and non type D (beta=-.325, p<.001) individuals, and this variable accounted for 24.1% and 18.2% respectively. CONCLUSION: The study revealed that the middle-aged workers showed high degree of job stress and depression that influenced on their health promoting behaviors. Thus, they are recommended to receive health management programs that offer treatment through consulting considering their individual personalities.

8.Cytogenetic Analysis in 543 cases of amniocentesis.

OBJECTIVE: To analyze 543 cases of amniocentesis with indications, cytogenetic results. METHOD: This study includes 543 cases cytogenetic study results which amniocentesis to detect fetal chromosomal abnormality in the Cytogenetic Laboratory at Eun Hospital in Kwang-Ju from August 1996 to December 2000, as gestational ages, indications of amniocentesis, maternal age distributions, chromosome aberrations with cytogenetic results. The cytogenetic results of chromosome aberration was identified by parents inheritance and de novo karyotypes as parents periperal blood cytogenetic study. RESULTS: Amniocentesis performed mostly from 15 weeks to 20 weeks of gestaional ages. Requested indication of amniocentesis presents abnormal maternal serum screening (37%), infertility (23%) and maternal old age (> or =35) (17%). Chromosome aberration according to amniocentesis indication was suspected fetal anomaly by ultrasonogram (8.6%), previous family history (5.7%). Chormosome aberration following maternal ages, more 40 years old women were found highly 7.9%. The frequency of chromosome aberration was 5.5% but de novo chromosome aberration was 2.2%. Numerical aberration was overall new karyotypes (1.3%). Structural aberration was inheritance karyotypes (3.3%) and de novo karyotypes (0.9%). CONCLUSION: Amniocentesis is a effective diagnostic tools in fetal chromosome aberration. Indication of fetal chromosomal anormality by ultrasonography and advanced maternal ages is an important diagnostic method with chromosome aberrations. Prenatal fetal chromosome aberrations included inheritance and de novo karyotypes. Especially, identification of de novo chromosome aberrations may predict fetal anomaly and counsel the fetus for pregnant parents.

9.Difference in Sexual Knowledge and Attitudes between High School and College Girls.

PURPOSE: This study was performed to identify the difference in attitudes and knowledge between high school and college girls regarding sexual education. METHOD: Data was collected from questionnaires given to 99 high school girls and 110 college girls in Seoul and Kyunggi-do. Data was analyzed using the SPSS 12.0 program. RESULT: In middle school, the time given to sexual education influenced the students present knowledge of sexual education. The sources of sex education were friends, seniors and mass media. The score for sexual knowledge in high school girls was 12.78 and for college girls 24.19. The field that obtained the highest score in territorial sexual knowledge was 'reproductive physiology' in the high school girls. There was little awareness of venereal disease, AIDS, contraception. Sexual attitudes among college girls was more open than that of high school girls. There was a positive correlation between sexual knowledge and attitudes(r=.535, p<.001). The significant difference of items were residence and sexual experience(p<.001). CONCLUSION: This study was shown that realistic knowledge teaching of venereal diseases, contraception and AIDS should be included in planning of sexual education programs. Specially, the groups had sexual experience and college girls in the dormitory will need more specific teaching.
