1.An Unusual Case of Acinic Cell Tumor.

Acinic cell tumor, a rare tumor of the salivary gland origin, consists of cells similar to the serous cells of the salivary gland. The tumor is almost exclusively paroti4 gland origin, accounting for 2. 7 to 4. 0% of all parotid tumors. The tumor is not,benign but has at least a low-grade malignant potential. We described herein a case of acinic cell tumor which had occurred on an unusual site, right inner epicanthal area of a 9-year-old girl Microscopically, a hematoxylin-eosin stained biopsy specimen showed rnany lobules com of round or polyhedral tumor cells. Each tumor cell had distinct cell membrane, clear cytoplasm, and relative1y eccentric nucleus. The cytoplasm of tumor cell cantaine4 diastase resistant PAS-positive material.

2.5 cases of the Cryptorchidism Treated with Hormonal Therapy (H.C.G.).

5 cases of cryptorchidisms were treated with H.C.G. during the period from Feb. 1980 to Sept. 1980. Among them, in 3 cases of bilateral cryptorchidism, H.C.G. effect was surprisingly excellent in doses of 5,500 I.U. of puberogen for 7 weeks, 6,000 I.U. for 4 weeks and 4,000 I.U. for 4 weeks respectively. In 2 cases of unilateral cryptorchidisms the H.C.G. effect was rather unsuccessful, but seemed to be meaningful. H.C.G. treatment was thought to be valuable prior to surgery especially in bilateral cryptorchidism.

5.Recent Results of Quantitative Culture and Sensitivity Test of Urine.

This paper is presented to report the results of urinary quantitative culture and drug sensitivity tests for isolated micro-organisms in this study. Studies were done on 172 cases from the department of Urology, Eul Ji General Hospital during the period from Jan. 1979 up to Dec. 1980. 1. Quantitative urine culture in 172 cases were performed and of these, 89 cases grew micro-organism wile the remaining 83 cases showed no growth. Among the 89 cases, 67 cases (75. 2%) revealed significant bacteriuria with colony count over 10 5/ml in urine while 22 cases (24. 8%) were below 10 5/ml. Radio of Male: Female is 29 : 38 2. 55 cases (82. 1%) among the 67 cases with significant bacteriuria could the bacteria be identified on a smear and Gram stain. 54 cases (80.6%) of the 67 significant bacteriuria casts revealed pyuria of more than 5/HPF while 13 cases (19.4%) were below 5/HPF 3. 60 cases (89. 6%) of the 67 cases significant bacteriuria has Gram (-) bacilli while 7 cases(10.4 %) were Gram (+) cocci by direct smear. The frequency of isolated organism were as following E. coli ....33 cases (49. 3%). Pseudomonas ....11 cases (16. 4%) Enterobactor ....6 cases(9.0%) Proteus and stapbylo. Aureus .....4 cases (5.8%) respectively. 4. On the whole, all isolated micro-organisms showed considerable susceptibility to panimycin (82. 7%) and then frequency of susceptible drug were and then the frequency of susceptible drug were Cefamezine, Kedacillin, Gentamicin.

6.Clinical Observation on Chronic Prostatitis.

A Clinical observation was made on 220 cases of chronic prostatitis during the period from January 1980 to March 1981. The following results were obtained: 1. Age distribution was between 20 and 70 years old, showing the highest incidence in 20 to 29 years old (44. 6%) and the next in 30 to 39 years old (36.8%). 2. Subjective symptoms were in order of urethral pain or tingling, morning drops or urethral discharge, urinary frequency and perineal discomfort. 33 cases (14.3%) were free of symptoms. 3. Previous diseases were acute or chronic urethritis on 110 case (50.0%), previous prostatitis on 25 cases (11.4%), urinary stone on 7 cases (3.2%), S.P.H. on 7 cases (3.2%). 15 cases (6.7%) were free of previous diseases. 4. In urine examination, 94 cases (43.2%) showed W.B.C. more than 5/H.P.F 126 cases (56.8%) were within normal limit. 5. In 137 cases in which prostatic gram stain was done, microorganisms were gram (+) cocci on 28 cases (20.5%). gram (-) bacilli on 13 cases (9.4%), gram (+) bacilli on 8 cases (5.9%). No microorganism were on 88 cases (64.2%).

7.Carcinoma of the Prostate Gland Accompanied by Severe Chronic Anemia.

We report a case of Undifferentiated carcinoma of the prostate in a 66 years old male resulted in severe chronic anemia (Hb value: 2.9gm/100dl, Hct:9%). The patient was treated with multiple blood transfusions, diethylstilbestrol and cystostomy with good improvement of general condition.

8.Bilateral Synchronous Seminoma in Abdominal Cryptorchid testes.

We present a case with bilateral synchronous seminoma in abdominal cryptorchid testes in a 35 year-old male patient and a review of the literature. Diagnosis was made by abdominal C.T. scan and exploration. The tumor stage was IIa. Radiation therapy and re-exploration of abdomen were done. In six months of follow-up, no evidence of recurrence was recognized.

9.Tester to Measure the Hardness of the Nasal Tip: A New Device.

The nasal tip occupies important position in Asian rhinoplasty because Asian has a nasal tip featuring of round or bulbous shape, lower height and wider base compared with that of Caucasian. The hardness of the nasal tip may be increased during the course of a routine rhinoplasty by maneuvers such as incision or excision of the lower lateral cartilage, suturing, and grafting of the cartilages. Changes of the hardness may be a component of postoperative dissatisfaction. However there is no device to measure the hardness of the nasal tip. We developed a "nasal tip durometer" using Teclock Durometer GS series 701 G. The spring rate of Teclock Durometer was converted from 0.223 kgf/mm to 0.036 kgf/mm to apply to the human body. After the remote air switch was fixed to apply with the same speed in a patient's nose, we manufactured the frame attachable to the surgical operation table. The reliability, validity, and responsiveness to clinical change of our durometer were evaluated in a clinical study of 20 patients. Our durometer was highly reliable (Spearman correlation coefficient, r=0.95), valid (Spearman correlation coefficient, r= 0.85), and responsible (effect size=11.37). We concluded that nasal tip durometer was useful to measure the hardness of the nasal tip objectively.

10.Extensive silicone lymphadenopathy after breast implant insertion mimicking malignant lymphadenopathy.

Silicone implants are widely used in aesthetic and reconstructive breast surgery. Silicone lymphadenopathy is a well-known rare complication of implant insertion. Silicone leakage from a rupture or silicone bleeding can accumulate in lymph nodes. Foreign body reactions in the affected lymph nodes may be misdiagnosed as metastasis or malignant lymphadeno pathy upon initial presentation if silicone lymphadenopathy is not considered in the initial diagnosis. We report a case of siliconoma with extensive involvement of multiple lymph nodes mimicking malignant features to emphasize that clinicians should carefully evaluate each patient's medical history and disease status during differential diagnosis.
