3.A comparative clinical study on decompression and enucleation to treat cystic lesions of the jaws.

PURPOSE: Among the various surgical methods used for the effective treatment of cystic lesion in the jaws historically, decompression procedure has some of superior prognosis compare to direct enucleation. In order to propose the efficacy of decompression we performed this retrospective study to compare decompression procedure with one-stage enucleation in clinical results and prognosis. PATIENTS AND METHODS: We reviewed 175 patients who had been histopathologically diagnosed cystic lesions from 1996 to 2000 in our department. Patients who had been received decompression alone or secondary enucleation after decompression were 31 cases, and enucleation alone were 144 cases. The age and sex of the patients, the area, size, and histological type of the lesions, and detailed operation and complications including recurrence were investigated. The minimal follow-up period was 2 years. RESULTS: In 31cases of decompression, male patients were 22cases(71%) similar to male predilection(62.3%) in total 175 cases. Cystic lesions were developed evenly in all age groups totally. Decompression was mainly performed in teenagers but enucleation was used in elder decades. In decompression cases the lesions were located in mandibular posterior, maxillary posterior, mandibular anterior, and maxillary anterior in order, which had some differences in total and enucleation cases. In enucleation cases, less than 3cm in size was 77.1% but larger than 3cm was 93.5% in decompression cases. Histopathologically, dentigerous cysts(54.8%), unicystic ameloblastomas(16.1%), and odontogenic keratocysts(12.9%) were seen in decompression cases and no recurrence or metaplasia and infection was observed. On the other hand, permanent tooth loss, numbness, recurrence, and so on were accompanied after enucleation. CONCLUSION: Although decompression procedure has disadvantages such as many of visiting times and slow recovery of the surgical defect, decompression is the best choice of treatment for large cystic lesions of the jaws, because it prevents functional and cosmetic defect, allows bone regeneration, and makes easy secondary enucleation.

5.4-Hydroxybenzaldehyde Restricts the Intracellular Growth of Toxoplasma gondii by Inducing SIRT1-Mediated Autophagy in Macrophages

Toxoplasma gondii is an intracellular protozoan parasite that infects approximately one third of the human popu- lation worldwide. Considering the toxicity and side effects of anti-toxoplasma medications, it is important to develop effec- tive drug alternatives with fewer and less severe off-target effects. In this study, we found that 4-hydroxybenzaldehyde (4- HBA) induced autophagy and the expression of NAD-dependent protein deacetylase sirtuin-1 (SIRT1) in primary murine bone marrow-derived macrophages (BMDMs). Interestingly, treatment of BMDMs with 4-HBA significantly reduced the number of macrophages infected with T. gondii and the proliferation of T. gondii in infected cells. This effect was impaired by pretreating the macrophages with 3-methyladenine or wortmannin (selective autophagy inhibitors) or with sirtinol or EX527 (SIRT1 inhibitors). Moreover, we found that pharmacological inhibition of SIRT1 prevented 4-HBA-mediated expres- sion of LC3-phosphatidylethanolamine conjugate (LC3-II) and the colocalization of T. gondii parasitophorous vacuoles with autophagosomes in BMDMs. These data suggest that 4-HBA promotes antiparasitic host responses by activating SIRT1- mediated autophagy, and 4-HBA might be a promising therapeutic alternative for the treatment of toxoplasmosis.
6.Relationship Between Uncommon Computed Tomography Findings and Clinical Aspects in Patients With Acute Pyelonephritis.

PURPOSE: Computed tomography (CT) has become popular in the diagnosis of acute pyelonephritis (APN) and its related complications in adults. The aim of this study was to investigate the relationship between uncommon CT findings and clinical and laboratory data in patients with APN. MATERIALS AND METHODS: From July 2009 to July 2012, CT findings and clinical data were collected from 125 female patients with APN. The six uncommon CT findings (excluding a wedge-shaped area of hypoperfusion in the renal parenchyma) studied were perirenal fat infiltration, ureteral wall edema, renal abscess formation, pelvic ascites, periportal edema, and renal scarring. The clinical parameters analyzed were the age and body mass index of the patients as well as the degree and duration of fever. Laboratory parameters related to inflammation and infection included white blood cell count, C-reactive protein (CRP) level, erythrocyte sedimentation rate, pyuria, and bacteriuria. RESULTS: The most common CT finding was perirenal fat infiltration (69 cases, 55%). A longer duration of fever, higher CRP level, and grade of pyuria were related with perirenal fat infiltration (p=0.010, p=0.003, and p=0.049, respectively). The CRP level was significantly higher in patients with renal abscess and ureteral wall edema (p=0.005 and p=0.015, respectively). CONCLUSIONS: The uncommon CT findings that were related to aggravated clinical and laboratory parameters of APN patients were perirenal fat infiltration, ureteral wall edema, and renal abscess formation. The inflammatory reaction and tissue destruction may be more aggressive in patients with these CT findings.

7.Relationship between Food-frequency and Glycated Hemoglobin in Korean Diabetics: Using Data from the 4th Korea National Health and Nutrition Examination Survey.

BACKGROUND: This study was performed to evaluate the association between food intake frequencies and long-term blood glucose control using glycated hemoglobin (HbA1c) in Korean diabetes. METHODS: This study used data collected from the 4th Korea National Health and Nutrition Examination Survey. Patients with diabetes were defined as people who responded "yes" to the question "Are you currently suffering from diabetes?" or in whom serum fasting glucose was 126 mg/dL or more. Using the food frequency questionnaire and HbA1c in surveys, we examined the association between the annual food-frequencies of each food group and long-term blood glucose control. RESULTS: After adjustment for other covariates, HbA1c decreased significantly as the frequency of the intake of fish increased in each population (P = 0.001 for all, P = 0.043 for men, P = 0.001 for women). The intake of mackerel played an especially important role in the control of diabetes. Among women, HbA1c decreased as the frequency of intake of legumes increased (P = 0.029) and increased as the frequency of intake of staple carbohydrates increased (P = 0.015). On the other hand, any intake of other food groups showed no significant relationship with HbA1c. CONCLUSION: Frequent intake of fish in all populations and legumes in woman were associated with good glycemic control in diabetics. Frequent intake of carbohydrates was associated with elevated HbA1c in women.

8.A new manufacturing method of all ceramic restoration using alumina tape and presentation of clinical cases.

In an effort to facilitate fabrication procedure of all ceramic crowns, a novel preparation method for all ceramic crown using alumina tape was developed. The alumina tape having a uniform thickness was cast by using Doctor blade method. The physical properties of newly introduced alumina tape has biaxial flexure strength of 500.600 MPa. The value of toughness is 3.18~3,28 MPa.m(1/2) which corelates with facture and the linear shrinkage rate of the alumina tape is 0.44% during core production. The marginal fitness of the alumina tape all-ceramic restoration with 90degrees shoulder margin had average marginal discrepancy at 78.3micrometer and average marginal gap at 44.4micrometer. At the marginal preparation of 135degrees deep chamfer, the average marginal discrepancy at 82.1micrometer and the average marginal gap at 40.2micrometer had been reported. This fabrication procedure of all ceramic crowns with alumina tapes is easier and less technical sensitive for dental technicians. After restoration with new all ceramic crowns we followed the patients 2 years later, there were no complications as porcelain fractures or periodontal disease. We had good esthetic clinical results with new all ceramic crowns.

9.Schwannoma in the Postauricular Sulcus being Misdiagnosed as Lipoma: A Case Report.

Schwannomas or neurilemmomas are the most often found as solitary, slow-growing, asymptomatic, well-encapsulated, firm yet transversely mobile benign mass in head and neck region. Most of Schwannomas appear in the cranial nerve, especially vestibular nerve, but must always be considered to be able to appear in any parts where the nerve tissues are distributed. Sometimes, inaccurate preoperative clinical diagnosis leads to accidentally resecting the affected nerve and produces permanent neurological deficits. We experienced a extremely rare schwannoma being confused with lipoma in upper left postauricular sulcus. After simple mass excision without considering possibility of schwannoma, the patient complained of dull sensation and pinprick pain in the superior part of the ipsilateral postauricular area. When clinical symptom and physical examinations are insufficient to distinguish schwannoma from other benign soft masses, and the mass along the critical nerve pathway can possibly be originated from the designated nerves, we would like to recommend radiologic examinations such as ultrasonography and/or MRI for correct diagnosis prior to operation. Moreover, when it is adjacent the neurovascular structures, meticulous microscopic surgery must always be performed so as not to impair the important structures.

10.Intraradicular Lumbosacral Disc Herniation: A Case Report And Review of the Literature.

Intraradicular disc herniation is a very rare form of disc herniation because the disc should migrate distally and laterally to enter the nerve root sheath. No case has been diagnosed before surgery. We descibe a 72-year-old woman who has an intraradicular disc herniation into the S1 nerve root and summarize the common features of the past reported 12 cases and this one. MRI showed enlarged nerve sleeve simulating root cyst at the S1 body level. At surgery, fusiform swelling of S1 nerve root adhered to anterior structure was seen and root tumor was suspected. Through the incised opening of the root sheath, two pieces of disc fragment were removed and final diagnosis of intraradicular disc herniation was made. Biopsy of extracted frag-ments revealed degenerative collagenous tissue. Patient's symptom was relieved almost immediately after surgery. An unusual form of intraradicular disc herniation is presented with review of the English language literatures.
