3.A Case of Benign Symmetric Lipomatosis.

Benign symmetric lipomatosis is characterized by massive symmetric fat deposition, predominantly in the neck and shoulder girdle ares. We present a case of benign symmetric lipomatosis with alcoholic liver cirrhosis in a 54-year-old man. About two years ago, solitary skin-colored, non-tender swelling on the right upper arm was developed. The lesion had been aggravsted abruptly and rapidly in size and distribution since about one month ago. Histopathologic findings showed completely replaced reticular dermis and invaded subcutaneous tissue by mature fat cells.

4.A Case of Trichorrhexis Nodosa.

We report a case of trichorrhexis nodosa in a 27-year-old male. Scanning electron microscopic findings reveals those of typical trichorrhexis nodosa nodal swelling of affected hair showed longitudinal fissures and fractures in the cortex. A paintbrush effect was noted at the site of fracture. Simple avoidance of trauma gave some regrowth of hairs.

5.Intraocular Lens Power Calculation in High Myopic Eyes with Previous Radial Keratotomy.

PURPOSE: To elucidate a proper method of intraocular lens (IOL) power calculation in highly myopic eyes with previous radial keratotomy (RK). METHODS: Five post-RK-surgery eyes with pre-RK-surgery myopia over -10.0 diopters (D) were studied retrospectively. The keratometric values obtained via the clinical history method, the contact lens over-refraction method, conventional keratometry, and Orbscan II were compared to the true keratometric value calculated retrospectively using the SRK/T formula. RESULTS: The mean pre-RK-surgery refractive power was -19.35+/-5.86D (-14.50 ~ -29.50D) and cataract surgery was performed on average 16.6 years after RK. The true keratometric value was closest to that estimated by the contact lens over-refraction method in 1 eye and to the flatter keratometric value between Sim K and the 3mm zone mean Pwr obtained with axial keratometric power map of Orbscan II in 4. CONCLUSIONS: The flatter keratometric value between Sim K and the 3mm zone mean Pwr from Orbscan II was closest to the true post-RK-surgery keratometric value of the central cornea.

6.Long-standing Adult Horizontal Strabismus with Early Childhood Onset.

Management of infantile and early childhood onset strabismus should be completed during early childhood to gain better binocular function and vision. The author experienced 38 adult patients who had horizontal strabismus without any adequate treatment lasting over 14 years with onset before 6 years of age. They showed large angle of deviation and deep amblyopia. After surgery, stereopsis was never developed in all of the 14 congenital strabismic patients, whereas 30% of the 24 patients with acquired onset showed gross stereopsis. There was abnormal muscular contracture requiring adjustable stutre. All of the patients showed abnormal character psychologically. This study revealed that the lunger the duration of misalignment, the greater the chance of deep amblyopia large angle of deviation, abnormal sensory and motor adaptation, psychological influence, and ahnormal muscular contracture. Therefore early initiation of therapy during early childhood should be emphasized.

10.A Case of Intracranial Granulocytic Sarcoma Associated with Myelodysplastic Syndrome.

Granulocytic sarcoma is an uncommon extramedullary localized tumor composed of granulocytic precursor cells. The majority of cases have been reported in association with acute myeloid leukemia(AML) especially in children, but infrequently may occur in patients with myeloproliferative disorders or myelodysplastic syndromes. Most common sites of involvement were bone, soft tissue, lymph nodes and skin, but gastrointestinal tract, testis, central nervous system are also involved rarely. We present an unusual case of granulocytic sarcoma of the intracranium and the retro-orbital soft tissue occurring in a patient with concurrent myelodysplastic syndrome, diagnosed by brain magnetic resonance images and ultrasound guided needle biopsy.
