1.A Study on Postgonococcal Urethritis.

The subject were 653 male patients with uncomplicated gonorrhea at the VD Clinic of Choong Ku Public Health Center in Seoul from January to November 1985. 653 male patients with uncomplicated gonococcal infection were treated one of the following regimens. Five treatment regimens used were 2, 0 gm spectinomycin single 1M, 4 0 mega unit fortified procain penicillin G+2. 0 gm kanamycin sulfate IM preceded by 1 pgm probenecid PO, 6.0 mega unit fortified procain penicillin G+2 pgm kanamycin sulfate IM preceded by 1. 0 gm probenecid PO, 5, 0 rnega unit aqueous crystaline penicillin+2. 0 gm kanamycin sulfate irn preceded by 1, 0 gm probenecid PO and 9 tablets of cotrimoxazole+ 2. 0 gm kanamycin sulfate IM. There were no significant differences in the incidence of PGU among the five treatment groups. The overall PGU rate was 67.3% 59.2% and 53.6% at 3~5, 7~10 and 14 days after treatment respectively. The incidence of PGU at 7-10 days was not significantly higher than that of at 14 days after treatment. It is suggested that it is best to test PGU at 7 days after treatment. Because the longer one waits, the harder to follow the patients, and in earlier period, post-inflammatory irritation might be too frequent. It is also suggested that at 3 5 days after treatment examination of urethritis might serve to compare the effect of treatment regimens on the incidence of PGU.

2.Clinical Study and Therapeutic Experience in Tinea Versicolor.

Fifty-six patients with tinea versicolor were studied clinically and therapeutically, from July 1984 to August 1985, at the Department of Dermatology, Hanyang University. L atients were treated with 20g sodium thiosulfate solution, 1g isoconazole nitrate cream, lg bifonazole cream and. oral ketoconazole. The result obtained were as follows: 1. The average age of all patients at visit was RO. 1 years, the oldest patient was 72 year-old and the youngest patient was 13 year-old. The male to female ratio was 3. 7: 1. 2. Distribution of lesions were anterior chest(26.8%), back(23.9%), abdomen (19%), axilla (17%), neck(6. 3%), pubic area(4. 2%), upper extremities(1. 4%), hip(0.7%) and lower extremities(0.7%). The incidence of hyperpigmented lesions was 76. 8%, and that of hypopigmented lesions was 23. 2g. 3. Average of treatment duration in each trial group indicated 3. 71-I--0 83 weeks (M+SD) in 20g sodium thiosulfate regimen group, 3. 07+ 0. 73 weeks in 1g isoconazole nitrate, 3.29+1.14 weeks in bifonazole, R. 00+0. 68 weeks in oral ketoconazole. No treatment results show statistically significant differences among the trial groups.

3.A Case of Scleredema.

Scleredema is a rare scleradematosis of unknown cause involving the face, neck, upper portion of the trunk, and proximal upper extremities. The clinical features of the disease are nonpitting indurated edema or stiffness of the neck, which may be sudden or insidious in onset. Diabetes mellitus and its complications have been frequently described to be associated with this disease. We present a case of scleredema developed on the nape, upper portion of the back and shoulder in 60-year-old man who had been suffered from diabetes mellitus for the past ten years.

5.Frontal lobe dysfunctions in Parkinson's disease.

A wide range of cognitive impairments and depression can be observed in most patients with Parkinson's disease (PD). But the underlying mechanism of these changes has not to be clarified yet. This study evaluated whether the frontal lobe dysfunctions contribute to the cognitive deficits in PD by comparison with age and education-matched controls. The neuropsychological tests known to be sensitive to frontal lobe dysfunction were performed to 38 patients with PD and matched 24 controls. These were composed of 'Category Fluency Test (CFT)' including objects, animals, & nations and 'Wisconsin Card Sorting Test (WCST)'. General cognition and depression were measured 'Korean Mini-Mental State Examination (K-MMSE)' and 'Beck Depression Inventory (BDI)' respectively. We examined the degree of motor disabilities in PD using the Hoehn-Yahr Stage. Patients with PD were assessed before noon and during the ""On"" time of levodopa to reduce the diurnal variation of mood and drug effects. Although PD patients, compared with controls, showed decreased ability in attention & calculation, memory, and visuo-spatial construction, the most prominent deficit was frontal lobe dysfunction evaluated with WCST and CFTs. Cognitive impairments in PD did not attribute to the discrepancy of education level or associated depression. So we suggest that the frontal lobe dysfunction may play an important role for cognitive impairments in PD.

6.Urine culture and serologic test for diagnosis of trichomoniasis in male patients with nongonococcal urethritis.

Trichomoniasis, a common and sometimes distressing condition in women, is caused by infection of the genitourinary tract by the pathogenic protozoan Trichomonris aginalis(T. vaginalis). Men may also be infected, although frequently without any symptoms, and most cases of trichomoniasis are sexually transmitted. The best evidence for an etiologic role for T. vaginalis is in nongonococcal urethritis(NGU). In epidemiologic studies, T,ginalis has been isolated from 1 to 68 percent in men with NGU. The nurnber of NGU is increasing rapidly than gonorrhea in deeloped nations, and in some developing countries, especially in Korea, In view of the difficulties in findng trichomonads by direct microsco by in men, urine culture and serodiagnosis plays an important role in the diagnostic work. Urine culture, indirect fluorescent antibody test(IFAT) and enzyme-linked immunosorbent assay (ELISA) were used to elecect T.vaginalis in 221 patients with male NGU. These patients visited the VD Clinic of ChoongKu Public Health Center in Seoul from May to August, 1989 and from March, 1990 to December, 1991. The results obtained were as follows.: 1. T. vaginalis was grown from only one of the 221 samples on TPS-1 meida. 2. In 208 NGU patients, there were 10(4.8%) positive reactions on IgG antibody in IFAT. 3. In 208 NGU patient, there were 14(6.7%) positive reactions to IgG antibody in ELISA. 4. No statistical significant correlation was found between then sults of IFAT and ELISA in individual patients. In conclusion, 1. Male trichomoniasis in patients with NGU was detected at a, significantly low rate. Due to the small number of patients in the test, further epidemiologic stidies of trichomoniasis in male NGU patient are necessary. 2. Serologic studies (IFAT and ELISA) have detected sorre trichomoniasis in male patients with NGU. These tests seem to be helpful in the diagnosis of male t-ichomoniasis.

7.Dynamic CT Findings of Bilateral Castleman Disease in the Neck: A Case Report.

We present a case of hyaline vascular type Castleman disease involving the bilateral cervical lymph nodes. To our knowledge, no previous case of this localized form of the disease has been reported. Dynamic CT demonstrated a hypervascular pattern of enhancement, with central less enhanced areas that corresponded histologically to fibrosis. For the diagnosis of this uncommon lymph node disease, these findings might be helpful.

8.Relationship of Recurrent Abdominal Pain Syndrome and Gastric Emptying Time.

PURPOSE: Recurrent abdominal pain syndrome (RAPS) is not uncommon in school-aged children, but the etiology and pathogenesis are not understood well. But recent studies suggest that motility disorder makes up a majority of the pathogenesis. The aim of this study was to investigate gastric emptying time (GET) which is a method to evaluate gastrointestinal motility in patients with recurrent abdominal pain syndrome. METHODS: Radionuclide GET was investigated in 34 patients who visited the Department of Pediatrics, Pusan National University Hospital due to recurrent abdominal pain from January 1996 to June 1997. After 8 hours of fasting, the patient received two pieces of toast and a fried egg tagged with 2 mci of 99mTc albumin colloid and sequential counting was accomplished. We calculated the geometric mean of two counts measured from AP-PA scans and investigated 60 minutes of intragastric radionuclide retention rate. RESULTS: A total of 34 patients (12 boys, 22 girls, M : F=1 : 1.8) participated in this study. The age ranged from 5 to 12 years and the mean age was 8.8 years. Associated symptoms were headache (7 patients), nausea (7 patients), constipation (2 patients), vomiting (1 patient), and diarrhea (1 patient). Anti-Helicobacter pylori IgG Ab was positive in 4 cases. Endoscopic gastritis was found in 6 cases. The mean of 60 minutes of intragastric radionuclide retention rate was 72 +/- 17% and 27 cases (79%) were delayed significantly. CONCLUSION: The results of our study suggest that delayed gastric emptying time is a cause of abdominal pain in patients with recurrent abdominal pain syndrome.

9.Sclerosing Hemangioma of the Lung: A report of 2 cases.

Sclerosing hemangioma of the lung is uncommon benign neoplasm of uncertain histogenesis, although their radiological appearance is relatively distinct and well-defined. Recently, we experienced 2 cases of sclerosing hemangiomas of the lungs in 61 and 39 years old women. The light microscopic findings of the tissues are similar to the features reported by Liebow and Hubbell(1956). The basic cellular response is thought to be type II pneumonocytes because of findings of multilamellar-like bodies within stromal cells with electron microscopy in case I in addition to other characteristics generally found in epithelial cells.

10.Malignant Melanoma of the Lacrimal Sac.

Malignant melanoma of the lacrimal sac is extremely rare. There are only 22 cases that have been reported in the world literature. One report in Korea concerned a 56 year old man presenting a 1.5-year history of bloody and purulent ocular discharge from the right eye. Chocolate colored bloody discharge appeared when pressing on the right medial canthus. Cytological examination of bloody discharge suggested malignant melanoma. A dacryocystogram showed obstruction of blood flow in the right nasolacrimal duct. CT scan showed a soft tissue density in the right lacrimal sac without any associated bony destruction. Medial maxillectomy inc1uding dacryocystectomy and postoperative irradiation were done. The patient is currently 5 months after surgery with no evidence of recurrence. Prognosis of malignant melanoma of lacrimal sac is very poor. Thus, early diagnosis and treatment is important.
