4.Phycomycosis Involving Ankle Joint: Report of a case.

Phycomycosis is an uncommon opportunistic fungal infection, involving several organs, such as brain, lungs, gastrointestinal tract and skin. Invasion of joint cavity by phycomycetes, however, has never been reported. We report a case of 33-year-old male who had had prehallux with congenital equinovarus and underwent correction operation on the right ankle joint. There after, joint pain and edema had developed on that site. He received reoperation and underwent excisional biopsy from there. Operation showed hypertrophied yellowish synovium. On microscopic examination, the tissue showed several foci of fibrinoid necrosis and suppurative and granulomatuous inflammation. Periodic acid Schiff stain demonstrated broad. Aseptate hyphae that were wrinkled and folded.

5.A Case fo Keratosis Palmaris et Plantaris Thost-Unna Type.

A case of 17 years old male with KPP of Tbost-Unna Type is reported. The akin lesions were characterized by the presence of confluent, sharply demarcated hyperk eratotic plaques on the palmar and plantar surfaces. Hypercontraction of palms and soles and hyperhidro is of scalp, face, palms and soles were also noted. Roentogenologic study revealed secondary changes of osteoporosis, flexion deformity and osteoarthritis due to soft tissue contracture and phalangeal and metatarsal bones of hands and feet showed bony resorption with tapering appearance. Histalogic findings of palmar lesion showed excessive hyperkeratosis, parakeratosis and acantosis in the epidermis and no pathology was noted in the dermis and skin appendages, He was treated with hot water compress, 5% Salicylic acid ointment and vit. A and D for 2 months and favorable effect was noted. Authors reviewed relevant literature and made discussions also.

6.Comparison of Two Arthroscopic Coracoplasty Approaches in Subscapularis Tears.

BACKGROUND: Few studies have reported the results of arthroscopic coracoplasty concomitantly conducted with subscapularis tear. Therefore, this study was conducted to examine and compare the outcomes of arthroscopic subscapularis repair after arthroscopic coracoplasty using either the subacromial approach or rotator interval approach. METHODS: We retrospectively reviewed 51 patients who underwent coracoplasty with subscapularis repair. The patients were grouped according to whether the subacromial approach group (24 patients) or rotator interval approach group (27 patients) was used during coracoplasty. Preoperative and postoperative visual analogue scale scores, American shoulder and elbow surgeons scores, Korean shoulder scores, and range of motion (ROM) were assessed. Assessment of repaired rotator cuff tendon integrity was performed at 1 year after surgery using either magnetic resonance imaging or ultrasonography. RESULTS: At final follow-up, overall functional scores and ROM improved significantly in both groups when compared with preoperative values (p>0.05). The re-tear rates were not significantly different between groups; however, the rotator interval approach group showed a significant increase in ROM compared with that in the subacromial approach group (p<0.05). CONCLUSIONS: Arthroscopic coracoplasty conducted concomitantly with subscapularis repair can provide a satisfactory outcome. There were no significant differences between the two approach groups regarding final functional scores and re-tear rates. However, the rotator interval approach group showed a greater increase in ROM at final follow-up, especially in external rotation.

9.Validation of Simple Screening test for Dementia in the Elderly: The Time and Change Test.

BACKGROUND: Dementia has emerged as a leading public health problem with elderly persons and its early detection is important for treatment in curable cases. Although dementia screening tests are available, they are still complex and time consuming in practice and therefore difficult to use. Our goal was to validate the time and change(T&C) test, a simple, standardized method for detecting dementia in the elderly populations. METHODS: Participants were 59 patients aged 65 years or older at an urban hospital from November 1 to December 31, 2001. The time test evaluated the understanding of clock hands indicating 11:10, and the change test the ability making 1,000 Won from a group of coins consisting one 500 Won, seven 100 Won, seven 50 Won. T&C ratings were validated against a reference standard based on physician`s diagnosis. Test-retest reliability and inter-observer reliability were assessed. RESULTS: The T&C test had a sensitivity of 73.0%, specificity of 90.9%, positive predictive value of 93.1%, and negative predictive value of 66.7%. Test-retest and inter-observer agreement rates were 95% and 95%, respectively. The T&C test was not influenced by education. The time and change tests took a mean of 7.5 seconds and 19.6 seconds to complete respectively, and was acceptable to participants. When timed cut points were added, the T&C test had a sensitivity of 86.5%, specificity of 40.9% and test-retest and inter-observer agreement rates were 77.3% and 81.8%, respectively. CONCLUSION: The T&C test is a simple, accurate reliable, performance-based tool for detection of dementia in the elderly.

10.Extracranial Meningioma in Parotid Area: A case report.

Meningioma is one of the most common intracranial tumor in adult. The tumor, however, occurs very rarely outside the central neuraxis, such as orbit, nasal cavity of parotid area. We report a case of extracranial meningioma in the parotid area. A 38-year-old man was presented with a round mass in the left parotid area. Magnetic resonance imaging revealed a round mass attached to anterior pole of the left parotid. The mass was easily separated from facial nerve. Microscopically, the mass was composed of ovoid or spindle cells. The tumor cells tended to show bundle formation, lobular arrangement and occasionally whorling appearance. This case is important at the means that meningioma should be included in differential diagnosis of mass in the parotid area.
