As Kelsey stated “Four out of five individuals will have a significant complaint of back painat one time or another in their lives and impairment of the spine are the most frequent cause of time lost from work”., low back pain affects the quality of life for just about everyone, medical and economic impacts of low back pain are enormous. Nevertheless, neither the government not industry has been seriously involved in the study of the cause, prevention, and management of back pain. The purpose of this study was to review and analysis the clinical materials of low back painand draw differences, if any, between discogenic and stenotic low back pain. Three hundreds and fourty nine cases suffering from low back pain and/or sciatic pain, excluded fresh traumatic and inflammatory conditions of the spine, have been admitted and treated at Dept. of orthopaedic surgery, Eul-Ji General Hospital, Dae-Jeon from May, 1981 to April, 1984. They were consisted of H.N.P. (172 cases), Spinal stenosis (61 cases), Spondylolisthesis (42) cases), Degenerative joint Disease of the spine (48 cases), and Others (26 cases). Among them, two hundred and twenty six cases who were followed more than four months were mainly subjectecl to this study.

3.Clinical Experience of Metastatic Prostatic Carcinoma.

26 cases of stage D prostatic carcinoma admitted to Dept. of Urology during the period from August 1, 1970 through July 31, 1984 were observed clinically. 1. 26 cases (43.3%) among 60 cases of prostatic carcinoma belonged to stage D at the time of diagnosis. 2. The presenting symptoms showed prostatic obstruction in 21 cases, gross hematuria in 8 cases and lumbago in 2 cases. 3. Serum acid phosphatase activity was elevated in 12 cases (46.2%). Serum alkaline phosphatase activity was elevated in 8 cases (30.8%), 6 cases of whom had osseous metastases. 4. 25 cases revealed adenocarcinoma of the prostate and 1 case revealed squamous cell carcinoma. On the pathological examination, 13 cases were poorly differentiated, 9 cases were intermediate range and 2 cases were well differentiated adenocarcinoma. 5. Orchiectomy was done in 23 cases immediately following biopsy and D. E. S. therapy was done in 7 cases with radiation therapy in 1 case. 6. 15 cases were followed up for more than 1 year, revealing survival rates 73.3% in 1 year, 66.7 % in 2 years and 60% in 3 years.

4.Primary Neoplasm in a Vesical Diverticulum: A Case Report.

5.A Case of Cerebral Infarction Caused by Tumor Emboli from the Site of.

Multiple cerebral infarction developed in a 36-year-old woman with adenocarcinoma of the right lung with direct cardiac invasion. Neurological examination and brain MRI showed findings of ordinary infarction. Transesophageal echocardiography showed left atrial pedunculated mass which was the potential source of embolism. We report a rare case who had such multiple spontaneous tumor emboli large enough to result in clinically detectable cerebral infarction.

6.Immunohistochemical Staining of Amyloid Deposit with Monoclonal Anti - keratin Antibodies in Primary Localized Cutaneous Amyloidosis.

Nine cases of primary localized cutaneous amyloidosis were studied by immunoperoxidase technique (ABC method) employing anti-keratin antibodies. All specimens were examined using consecutive paraffin sections to confirm the correspondence between amyloid existing area and reactive sites. Anti-keratin antibody 34pE which recognize 68, 58, 56.5, 56kd keratin peptides reacted with amyloid deposits in both lichen amyloidosus and macular amyloidosis. However, anti-keratin antibodies 34pB4 and 35pH did not react with amyloids. In general, Dylon staining positive material, keratin reacted with 34pE and amyloid P showed similar distribution in serial sections, but did not show the same one. Several keratin bodies reacted with 34pE, which were not stained with Dylon staining or antiamyloid P were found in the dermis of one specimen. These results suggest that immunohistochemical staining with antikeratin antibody 34pE using formalin-fixed, paraffin-embedded sections appeared to be a useful method in studying the histogenesis of primary localized cutaneous arnyloidosis.

7.Complications of Ureteroileal Conduit: Review of 88 Cases.

Review of complications was made on 88 cases of the ileal conduit admitted to Dept. of Urology during the period of 13 years from August 1, 1970 through March 31, 1982. 1. There were 70 males and 18 females (ratio of 4:1) ranged in age from 7 to 72 years with peak incidence in the 6th decade. 2. The most common indication of this procedure was bladder tumor and other indication were renal tuberculosis with contracted bladder, neurogenic bladder, urethral rupture and urethral tumor. 3. 64% of patients with abnormal renal function preoperatively had some improvement postoperatively. 4. Operative mortality was 6.8%. The late mortality was 28%. The operative death rate in benign disease was 6.9% and the late death rate was 3.6%. The operative death rate in malignant disease was 6.9% and the late death rate was 42%. 5. Early complication rate in the benign disease was 17.2% and the late complication was 28.8%. Early complications included wound infection and dehiscence. diarrhea, prolonged ileus. and parastromal dermatitis. The late complications included intestinal obstruction, parastromal dermatitis, stromal stenosis and pyocystis.

8.Amyloid Deposit in Malignant Cutaneous Epithelial Tumors.

We examined the amyloid deposit in various malignant cutaneous epithelial tumors by using Dylon stain. Histochemically identifiable amyloid deposits associated with basal cell epitheliomas(BCEs), squamous cell carcinomas(SCCs) and Bowens diseases were studied with monoclonal cytokeratin antibodies and anti-amyloid P antibodies. The results were as follaws, l. Amyloid deposits were detected in 5 of 12 BCEs, 4 of 11 SCCs, 3 of 9 Bowens diseases and 1 of 8 actinic keratoses. Amyloid deposits were not detected in o keratoacanthomas, 2 verrucous carcinomas, 1. Pagets disease and 3 extramammary Pagets diseases. 2. Anti-keratin antibody 34BE12 and anti-amyloid P antibody reacted with amyloid deposits in 3 of 5 BCEs which showed abundant amyloid deposits by Dylon stain. 3. Of 4 SCCs which showed amyloid deposits by Dylon stain, anti-keratin ant.ibody 34pE12 reacted with amyloid deposits in 2 cases and anti-amyloid P antibody reacted with amyloid deposits in 3 cases. 4. Anti-amyloid P antibody reacted with arnyloid deposits in 3 of 3 Bowens diseases which showed arnyloid deposits by Dylon stain. Anti-keratin antibody 34pE12 reacted with amyloid deposits only 1 of them. These findings suggest that epidermal keratins are the percursor substance of amyloid in malignant cutaneous epithelial tumors. The difference of antikeratin staining pattern between BCE-associated amyloid and SCC or Bowens disease-associated amyloid may be the results of difference in development stage of cutaneous amyloids.

9.Prophylactic Intravesical Chemotherapy in Superficial Bladder Tumors.

Superficial bladder tumors are usually treated by TUR. However, recurrence of the tumor after complete resection occurs in about 50-70%, of the patients with a significant percentage of these recurrences showing a higher degree of malignancy. In 10% of the cases, the tumor progresses to invasive carcinoma. Most of these recurrent tumors will occur within 6-12 months. The multifocal nature of these tumors and the frequent recurrences after TUR indicate a need for adjuvant chemotherapy to reduce or delay the incidence of recurrence tumors. We studied to evaluate prophylactic effects of topical chemotherapy in 116 cases with superficial bladder tumors admitted to the Department of Urology, Keimyung University School of Medicine during the period from Jenuary, 1971 through July, 1985. All patients were followed up for more than 6 months. Study group consists of 83 cases (Thio-tepa: 39, Adriamycin; 44) treated with topical chemotherapy following TUR during the period from January, 1978 through July, 1985. Control group consists of 33 cases treated by TUR only during the period from January 1971 through December, 1977. Following results were obtained: 1. 10.Renal Cell Carcinoma: Review of 28 Cases.

Review of renal cell carcinoma was made on 28 cases admitted to Dept. of Urology during the period of 11 years from September 1, 1972 through September 30, 1983. 1. There were 17 males and females (ratio of 1.5:1) ranged in age from 19 to 69 years with peak incidence in the 6th decade. 2. The most common symptom and sign was hematuria in 75% but the classic triad of this tumor was present in 29%. 3. The abnormal laboratory findings were elevated ESR in 75%, anemia in 64%, and reversed A/G ratio without hepatic metastasis in 14%. 4. On the urogram renal mass was found in 50%, calyceal destruction in 48%, and nonvisualization of kidney in 39%. 5. The renal angiogram of 10 cases demonstrate hypervascularity in 7 cases, hypovascularity in 1 case, and tumor staining in 7 cases. 6. Staging of the tumor revealed stage in 13 cases, stage III in 5 cases, and stage IV in 7 cases. 7. The histological type revealed clear cell in 20 cases, granular cell in 1 case, and mixed cell in 7 cases. 8. Treatments were simple nephrectomy in 16 cases, simple nephrectomy with adjuvant therapy in 3 cases, and radical nephrectomy with adjuvant therapy in 5 cases.
