1.A Case of Aplasia Cutis Congenita, Type V.

Aplasia cutis congenita is a congenital localized or widespread absence of the skin. We report a case of aplasia cutis congenita, type V, in a 6-day-old male infant born with the stellate and linear skin lesions covered by granulation tissue and soft capsule with slightly elevated erythematous edges on the trunk and lower extremities without any associated family history. The patient had amniotic bands and were diagnosed as aplasia cutis, type V. The patient received conservative treatment such as antiseptic dressing and prophylactic systemic antibiotics with healing of the ulcer.

2.Clinicopathological and Dermoscopic Features of Clear Cell Acanthoma

Background:
Clear cell acanthoma is a rare benign epidermal tumor that is considered a nonspecific reactive dermatosis. To date, no reports have described systematic analyses of clear cell acanthoma in Koreans.
Objective:
We investigated the clinical, histopathological, and dermoscopic features of clear cell acanthoma.
Methods:
In this study, we evaluated 14 lesions of clear cell acanthoma.
Results:
We observed female predominance (male:female ratio 1:1.3), and the mean patient age was 55.8±18.0 years (range 15∼76 years), with the peak incidence observed in patients in their 60s. Notably, 57.1% of lesions developed on the lower extremities. The lesion was most commonly misdiagnosed as seborrheic keratosis. Histopathological examination revealed a well-demarcated area of keratinocytes with pale-staining cytoplasm and psoriasiform epidermal hyperplasia and mild spongiosis, exocytosis of neutrophils, and thinned suprapapillary plates. Dermoscopic examination showed a clear cell acanthoma with characteristic vascular findings, including pinpoint red dots, globular red dots, and glomeruloid structures with linear or reticular patterns.
Conclusion
Clear cell acanthoma commonly presents as a small solitary lesion measuring <1 cm in diameter that occurs on the lower extremities in women in their 60s. Histopathological characteristics of this lesion include Periodic acid Schiff-positive with pale cytoplasm, and dotted, globular, and glomeruloid vessels with a linear or reticular pattern on dermoscopy. This was a single-center, small-sized retrospective study; however, in our view, the results of this study would contribute to the literature by providing a better understanding of clear cell acanthoma in Koreans.
3.A Case of Beauveria Bassiana Keratitis Confirmed by Gene Sequencing.

PURPOSE: To report a case of Beauveria bassiana keratitis that was confirmed by gene sequencing. CASE SUMMARY: A 70-year-old man presented to our hospital with complaints of ocular pain and deterioration of the visual acuity in his left eye after injury caused by a wood branch one week previously. Visual acuity in the left eye was 20/400 at the time of the first visit. Slit lamp examination showed a central 0.7 x 2.5-mm-sized epithelial defect surrounded by cellular infiltration in the stroma. Scraping of the corneal lesion for microbiological examinations was performed. Initial Gram stain, potassium hydroxide (KOH) mount, and culture were negative. However, fungal hyphae were observed on a KOH mount of the repeated corneal scraping specimen, and a Beauveria species was suspected based on the culture. Beauveria bassiana was confirmed using a MicroSEQ(R) D2 large-subunit ribosomal DNA fungal sequencing kit. Natamycin eye solution was initially instilled bihourly (every two hours), but the persistent epithelial defect and progressive stromal melting finally resulted in a descemetocele. Temporary and permanent amniotic membrane transplantations were performed, and amphotericin B eye solution was administered bihourly (every two hours). The ulcerous lesion gradually improved with no evidence of recurrence. CONCLUSIONS: Recently, cases of fungal keratitis have been increasing. Therefore, molecular diagnosis methods such as gene sequencing can be helpful in diagnosis and in improving the prognosis of fungal keratitis caused by rare fungi, as we found in this case.

4.The Effect of Job Stress and Lifestyle on Blood Lipid Levels in Male Aircrew Personnel.

INTRODUCION: Cardiovascular disease has the potential to lead to sudden in-flight incapacitation and permanent grounding. The aims of this study are to examine the relationships between lifestyle, job stress and blood lipid levels of male aircrew personnel of a Korean airline and to identify which factors influence their hyperlipidemia. METHODS: Two hundred sixteen male aircrew personnel completed a questionnaire by self-report and consented to participate in the study. The questionnaire collected data related to job stress, life style, serum cholesterol levels and general characteristics of the aircrew. The cholesterol levels of the subjects were collected from their most recent health check-up records. Subjects were divided into two groups (the desirable group and the risk group) based on their serum cholesterol level, 200 mg/dl. RESULTS: Mean age and marital status were significantly different between the two groups. More subjects in the risk group had habits of eating high lipid foods, while more subjects in the desirable group exercised more frequently than the risk group. In logistic regression analysis, after controlling age and marital status, types of working situation (domestic duty or international duty, odds ratio=.390, p=.018), diet (odds ratio=.429, p=.037), and exercise (odds ratio=.320, p=.055) were influencing factors on aircrew's serum cholesterol levels. CONCLUSIONS: The cholesterol level of aircrew personnel is closely related to their lifestyle, such as lipid diet and exercise. The type of work situations, e.g. staying in an airplane for long periods of time or staying abroad, may influence these diet patterns and exercise habits.

5.A Preliminary Study on the Expression of C-X-C Chemokine Genes in Colonic Mucosa of Patients with Ulcerative Colitis.

PURPOSE: Chemokines are potent regulators of the host inflammatory or immune responses. Mucosal synthesis of chemokines may be important in the pathogenesis of mucosal inflammation in ulcerative colitis (UC). We performed this study to investigate the expression of C-X-C chemokine genes in UC. METHODS: Mucosal tissues were obtained from six normal controls and six UC patients by endoscopic biopsies. In patients with UC, mucosal tissues were separately obtained from both involved and uninvolved regions. RNA was extracted and mRNA levels of five C-X-C chemokines were determined by quantitative reverse transcription-PCR using internal RNA standards. RESULTS: Mucosal mRNA levels of all chemokines tested increased in the involved region of UC compared with the uninvolved region of UC or normal controls. CONCLUSION: Our data suggest that mucosal expression of C-X-C chemokines contributes to the pathogenesis of UC

7.Spontaneous Healing of Multiple Penetration Wounds Induced by Cactus Spines

Cactus spines can easily puncture the skin with only minor pressure. Removal of the offending cactus spine is the treatment of choice; however, it is very difficult, even with tweezers. A 17-year-old Korean girl presented for evaluation of multiple purple-colored macules and papules on the left cheek and left thumb. Two months earlier, she had pricked her finger and face on cactus spines while volunteering in Haiti. An ultrasound identified wedge-shaped echogenic materials suspected as cactus spines. We recommended surgical removal of the spines, but she vehemently refused. We then advised her to take amoxicillin/clavulanic acid and undergo regular follow-up. During the follow-up period, the spines dislodged spontaneously for easy removal and the patient removed them with tweezers. The lesions actually improved over the next several months. Here, we report this rare and interesting case for educational purposes because such a case has never been previously reported in Korea.

9.Evaluation of the effect of a 0.0584% hydrocortisone aceponate spray on clinical signs and skin barrier function in dogs with atopic dermatitis.

The purpose of this study was to evaluate the effects of a topical spray containing 0.0584% hydrocortisone aceponate (HCA) on canine atopic dermatitis (CAD) and to evaluate the skin barrier function during the treatment of CAD. Twenty-one dogs that fulfilled the diagnostic criteria for CAD were included in this study. The HCA spray was applied once a day to the lesions of all dogs for 7 or 14 days. Clinical assessment was performed before (day 0) and after treatment (day 14), and clinical responses were correlated with changes in skin barrier function. CAD severity significantly decreased after 14 days of HCA treatment based on the lesion scores (p < 0.0001), which were determined using the CAD extent and severity index (CADESI-03) and pruritus scores (p < 0.0001) calculated using a pruritus visual analog scale. Transepidermal water loss, a biomarker of skin barrier function, was significantly reduced compared to baseline (day 0) measurements (p = 0.0011). HCA spray was shown to be effective for significantly improving the condition of dogs suffering from CAD. This treatment also significantly improved cutaneous hydration and skin barrier function in the animals.

10.Clinical use of a ceramide-based moisturizer for treating dogs with atopic dermatitis.

In humans, skin barrier dysfunction is thought to be responsible for enhanced penetration of allergens. Similar to conditions seen in humans, canine atopic dermatitis (CAD) is characterized by derangement of corneocytes and disorganization of intercellular lipids in the stratum corenum (SC) with decreased ceramide levels. This study was designed to evaluate the effects of a moisturizer containing ceramide on dogs with CAD. Dogs (n = 20, 3~8 years old) with mild to moderate clinical signs were recruited and applied a moisturizer containing ceramide for 4 weeks. Transepidermal water loss (TEWL), skin hydration, pruritus index for canine atopic dermatitis (PICAD) scores, and canine atopic dermatitis extent and severity index (CADESI) scores of all dogs were evaluated. Skin samples from five dogs were also examined with transmission electron microscopy (TEM) using ruthenium tetroxide. TEWL, PICAD, and CADESI values decreased (p < 0.05) and skin hydration increased dramatically over time (p < 0.05). Electron micrographs showed that the skin barrier of all five dogs was partially restored (p < 0.05). In conclusion, these results demonstrated that moisturizer containing ceramide was effective for treating skin barrier dysfunction and CAD symptoms.
