Squamous cell carcinoma of the ovary is rare and usually arise from preexisting cystic teratomas, endometriosis or Brenner tumors which undergo carcinomatous degeneration in their epithelial elements. Some of squamous cell carcinoma of the ovary is known to be associated with cervical neoplasm, but the pathogenesis is still unknown. Recently we experienced one case of ovarian squamous cell carcinoma in patients with microinvasive squamous cell carcinoma of uterine cervix. We report this case with possible pathogenesis and brief review of literatures.

Intravascular stents have been developed as an adjunct to balloon angioplasty to scaffold the inner vessel surface to improve patency of stenoses and occlusions of arteries and veins. Several types of stents have been described in the literature and have been used in animal experiments as well as in clinical studies, but the clinical application of venous stents is still rather limited and mostly restricted to arteries. We experienced one case of successful palliative treatment in a extrinsically compressed lilac vein by metastatic lymph node using a expandable metallic stent.

4.Expandable metallic stents: An experimental study in the nasolacrimal system of dogs.

Balloon dacryocystoplasty has been reported to be a safe, easy and effective nonoperative treatment for nasolacrimal stenosis. The results were not encouraging, however, because of its high failure and recurrence rates. To evaluate the feasibility of using modified Gianturco expandable metallic stents for maintenance of the dilated nasolacrimal system(NLS), 20 stents of 3 mm in diameter and 10mm long were placed in 20 nasolacrimal ducts of 10 dogs for 1 month to 10 months. It was more difficult to introduce the stents into the proximal portion(A) of the NLS than into the distal portion(B) due to the narrow and bony canal of the A portion. Twenty stents showed no migration in follow-up studies of up to 10 months. One complete occlusion occurred in a stent placed in A portion. Autopsy studies showed stents were covered with epithelium within 2 months after placement. Our experience suggest that the placement of Gianturco self-expandable stents may be a useful method of dilating and maintaining the luminal diameter of the NLS, although care must be taken to select the proper stent size.

7.Effects of Red or Black Ginseng Extract in a Rat Model of Inflammatory Temporomandibular Joint Pain.

Temporomandibular joint (TMJ) pain is characterized by persistent jaw pain associated with dysfunction and tenderness of the temporomandibular muscles and joints. The aim of this study was to investigate whether treatment with red or black ginseng extract helps in the modulation of inflammatory TMJ pain. Male Sprague-Dawley rats weighing 220~260 g were used. The experimental group was subdivided into 4 groups based on the treatment method (n=6, each group): formalin (5%, 30 µl), formalin after distilled water (vehicle), formalin after red or black ginseng extract (per oral, single or repeated, respectively). To induce TMJ pain, 30 µl of formalin was injected into the articular cavity under ether inhalation anesthesia. The number of noxious behavioral responses of scratching the facial region proximal to the injection site was recorded for 9 successive 5-min intervals following formalin injection. Repeated treatment with red or black ginseng extract reduced the nociceptive responses in the second phase (11~45 min). Nuclear factor erythroid 2-related factor 2 (Nrf2) is an oxidative stress-mediated transcription factor. Both ginsengs significantly down-regulated the increased Nrf2 level compared to the vehicle group. In the test for liver and kidney functions, repeated treatment with red or black ginseng was not different compared to the vehicle group. These results indicate that red and black ginseng extract might be promising analgesic agents in the treatment of inflammatory TMJ pain.

8.Perforation of Intrathoracic Stomach after Ivor Lewis Operation for Esophageal Cancer: 2 cases report.

We report our experience with 2 cases of perforation of intrathoracic stomach after Ivor Lewis operation for esophageal cancer. There was no problem in the anastomotic site, but the drainage from pleural cavity increased after oral intake. The stomach perforation was proved by rethoracotomy. The perforation site was repaired by sutures with pedicled intercostal muscle.

BACKGROUND: Adequate exposure of the mitral valve is a prerequisite for mitral procedures including the repair. An extended transseptal approach in mitral valvular operations is known to have certain technical advantages in recent years because of the anatomic posterior location of the mitral valve, especially in reoperations and in the presence of the small atrium in spite of the possibility of arrhythmia due to injury of sinus nodal artery. MATERIAL AND METHOD: We compared the preoperative status, operative, and postoperative factors among patients in two study groups, transseptal only (Group I, n=10) and extended transseptal approach (Group II, n=25). RESULT: There were no differences in age, sex, NYHA functional class, left atrial size, and left ventricular function. The incidence of the redo-operation was high and early postoperative arrhythmia, which was improved later, appeared in 3 patients in Group II, but not in Group I. CONCLUSION: We believe that atrial septal incision could be extended up to the atrial roof whenever exposure of the mitral valve during a transseptal approach is inadequate because the late results were similar.

10.Clinicopathological Experience on Ovarian Mass in Childhood and Adolescence.

OBJECTIVE: The purpose of this study was to review the incidence, clinicopathological feature, treatment, outcome in the series of childhood and adolescence with ovarian mass PATIENTS AND METHODS: Retrospective reviews of the medical recordings for 116 patients with ovarian mass under the age of twenty years old who were admitted, operated and confirmed with histopatholgical study at the department of obstetric and gynecology, Gachon medical school, Inchon, Korea for 8 years from Jan. 1993 till Dec. 2000. RESULTS: Of 116 the patients who underwent surgical treatment, the incidence of malignant ovarian tumors was 15 cases(12.9%). If ovarian neoplasm alone are considered, the rate of malignancy increases to 17.6%. The frequency of ovarian malignancies correlated inversely with patient age. In the 0-10 age group ,40% had malignancies, as compared with 20.3% in the 11-15 age group and 15.3% in the 16-20 age group. On histopathological classification, the tumors originated from germ cell tumors were 55.2%, epithelial cell tumors were 40%, and sex-cord stromal tumors were 4.7%. According to FIGO classification of malignant ovarian tumor, stage I(80%) was most common, followed stage IV(13.3%), stage II(6.6%) and III(0%). 13 of the 15 malignant ovarian tumors, unilateral salpingoophorectomy was done in an attempt for reproductive organ conservation; 2 cases of stage IV disease were treated with hystrectomy, bilateral salpingoophorectomy and omentectomy. On follow up, only two ovarian malignancy stage IV died within 3 months and another is well. CONCLUSION: The frequency with which ovarian meoplasms malignancy in the under 20 age group is 17.6%. Because of their malignant potential in young girl, prompt evaluation and treatment is imperative.
