1.An anatomical study of the mandibular ramus in Korean patients with dentofacial deformity.

Orthognathic surgery of the mandibular prognathism and the retrognathism is tend to be performed on the mandibular ramus to prevent inferor alveolar nerve injuries. The purpose of this study is to find a safe and accurate reference point on mandibular ramus for orthognathic surgery by comparative anatomical study of dentofacial deformity patients. We use 38 Korean Cadavers with normal occlusion(Group 1), 3-dimensional simulation of computerized tomogram of 23 patients with retrognathism (Group 2), 27 patients with mandibular prognathism (Group 3). Following results are obtained : 1. The maximum thickness of the mandibular ramus is 8.78+/-1.15mm for Group 2, 7.61+/-1.26mm for Group 1, 6.95+/-0.82mm for Group3 respectively (P=0001). The minimum thickness is 5.51+/-1.08mm for Group 1 , 5.06+/-0.40mm for Group 2, 4.56+/-0.78mm for Group3, respectively (p=0.0001). But, the thickness at the level of 5mm above the lingular is 0.78+/-0.65mm for Group 2, 5.63 +/-1.28mm for Group 1, 5.32+/-0.91mm for Group 3, respectively. There is no significant difference between these groups(P=0.0510). 2. The horizontal location from the midwaist point to lingular is 0.18+/-1.57mm for Group 1, 0.69+/-1.33mm for Group 2, 0.66+/-1.66mm for Group 3, and there is no significant difference between these groups(p=0.0835). But the vertical location from the midwaist point to lingular is 1.45+/-2.64mm for Group 1, 0.63+/-1.44mm for Group 2, 0.34+/-1.81mm for Group 3, and there is significant difference between these groups(p=0.0030). 3. The horizontal location from the midwaist point to mandibular foramen is 0.29+/-1.75mm for Group 1, 0.63+/-1.44mm for Group 2, 0.34+/-1.81mm for Group 3, and there is no significant difference between these groups(p=0.5403). But the vertical location from the midwaist point to mandibular foramen is -3.33+/-4.43mm for Group1, -4.79+/-2.26mm for Group 2, -6.06+/-2.99mm for Group 3, and there is significant difference between these groups(P=0.0001). 4. The horizontal length from the disto-buccal cusp tip of mandibular second molar to lingula is 30.97+/-4.17mm for Group 3, 28.29+/-2.65mm for Group 1, 25.48+/-0.77mm for Group 2 (p=0.0000), and also vertical length is 7.72+/-3.22mm for Group 3, 6.38+/-1.83mm for Group 1, 5.89+/-2.30mm for Group 2 (P=0.0014). 5. The location of lingular is 0.50 from anterior border of mandibular ramus in all groups, if it assumed the length from anterior border to posterior border is 1. And it is almost 0.33 from the sigmoid notch, if it assumed the length from sigmoid notch to antegonial notch is 1. 6. In Group 1, Antilingular prominence is located on (1.12+/-1.43mm, 4.01+/-2.36mm) from the midwaist point, and there is no correlation between antilingular prominence and lingular, mandibular foramen.

2.Analysis of Parricide and Filicide in Korea.

Parricide, the crime of murdering a parent, accounts for about 5% of all homicides. Filicide is the crime of murdering one's own child. This study aimed to review demographic features and criminal characteristics of individuals who committed parricide and filicide in Republic of Korea (ROK). This study is based on data from the Korea Police Crime Analysis System, from 2006~2013. We assessed the diverse characteristics of both victims and perpetrators. Over the selected period, 381 parents were killed by their children and 230 children were killed by parents in the ROK. Parricides caused by schizophrenic murders accounted for 39.6% of all cases. Moreover, approximately 44.4% of the perpetrators attempted suicide following the maternal filicide. In our findings, psychiatric illness was a very important predictor in parricide, and these further suggest that young mothers with severe mental illness require careful monitoring by mental health support service.

4.Implementation of a Prehospital ALS (Advanced Life Support) Program in the Seoul Metropolitan Area.

PURPOSE: The purpose of this study was to determine the feasibility of the implementation of prehospital advanced life support programs and share in-hospital medical direction center operation experience. METHODS: From Oct. to Dec. 2008, twenty fire safety centers in Seoul metropolitan city took part in a pilot implementation of advanced life support programs with medical services as follow: prehospital 12 lead ECG transmission for patients with chest pain, a prehospital CPAP (continuous positive airway pressure) program for patients with dyspnea, a prehospital stroke scale application for patients with neurologic problems, and real-time audio-visual information transmission for patients with traumatic injuries. RESULTS: A total of 6,741 patients were transported to hospitals by emergency response ambulances to twenty five different safety centers. Of the total number of patients, 304 received advanced life support management. The prehospital use of 12 lead ECG transmission was 57.89%, real-time ECG was 27.45%, continuous positive airway pressure was 20.60%, stroke scale was 25%, and real-time audio-visual information was 5.98%. A Delphi survey using a panel which was expert in prehospital implementation of advanced life support programs concluded that prehospital ECG transmission and stroke scale programs should be implemented. However, they decided against implementation of the continuous positive airway pressure and real-time audio-visual information programs. CONCLUSION: Overall, the feasibility of implementation of a prehospital conventional 12 lead ECG program was good and the Delphi survey concurred that the ECG transmission and prehospital stroke scale programs should be implemented.

5.Effect of Computerized Neuropsychologic Test in Subacute Post-Stroke Patient With Cognitive Impairment.

OBJECTIVES: To investigate the effects of Computerized Neuropsychologic Test (CNT) on cognitive function and daily life performance in subacute post-stroke patients with cognitive impairment. METHODS: Korean Mini-Mentals State Examination (K-MMSE), Korean version of Modified Barthel Index (K-MBI) were investigated in 125 subacute post-stroke patients with cognitive impairment. We analyzed K-MMSE and K-MBI which were conducted 63 patients who had received CNT and 62 patient who had not received CNT from baseline to 8 weeks follow-up. In the experimental group, initial K-MMSE and K-MBI were conducted 13.3 ± 6.8 weeks after the onset of stroke and their age was 63.4 ± 13.3. In the control group, initial K-MMSE and K-MBI were conducted 13.2 ± 7.7 weeks after the onset of stroke and their age was 65.1 ± 11.6. RESULTS: The 8 weeks follow-up total K-MMSE score and total K-MBI score of experimental group were significantly higher than control group (P < 0.05). In K-MMSE subsection, orientation, judgement, recall, language & visual reconstruction were significantly higher in experimental group than control group (P < 0.05). In K-MBI subsection, personal hygiene, bathing self, toilet, dressing, ambulation, chair/bed transfer were significantly higher in experimental group than control group (P < 0.05). The change of total K-MMSE score of experimental group was significantly correlated with change of total K-MBI score (P < 0.05), but control group was not (P > 0.05). In K-MMSE subsection, change of orientation, registration, language and visual reconstruction were correlated with total K-MBI s core after CNT. Especially, the experimental group, total K-MBI score of the left hemisphere damage group was significantly higher than the right hemisphere damage group (P < 0.05). CONCLUSIONS: This study shows that CNT is effective on subacute post-stroke patients with cognitive impairment. Improvement of cognitive function can expect a positive outcome on daily life performance, in particular, it can be expected to improve the prognosis of patients with stroke, the left hemisphere lesions.

6.Feasibility of a New Desktop Motion Analysis System with a Video Game Console for Assessing Various Three-Dimensional Wrist Motions.

BACKGROUND: The restriction of wrist motion results in limited hand function, and the evaluation of the range of wrist motion is related to the evaluation of wrist function. To analyze and compare the wrist motion during four selected tasks, we developed a new desktop motion analysis system using the motion controller for a home video game console. METHODS: Eighteen healthy, right-handed subjects performed 15 trials of selective tasks (dart throwing, hammering, circumduction, and winding thread on a reel) with both wrists. The signals of light-emitting diode markers attached to the hand and forearm were detected by the optic receptor in the motion controller. We compared the results between both wrists and between motions with similar motion paths. RESULTS: The parameters (range of motion, offset, coupling, and orientations of the oblique plane) for wrist motion were not significantly different between both wrists, except for radioulnar deviation for hammering and the orientation for thread winding. In each wrist, the ranges for hammering were larger than those for dart throwing. The offsets and the orientations of the oblique plane were not significantly different between circumduction and thread winding. CONCLUSIONS: The results for the parameters of dart throwing, hammering, and circumduction of our motion analysis system using the motion controller were considerably similar to those of the previous studies with three-dimensional reconstruction with computed tomography, electrogoniometer, and motion capture system. Therefore, our system may be a cost-effective and simple method for wrist motion analysis.

7.Association between defoliant exposure and survival to discharge after out-of-hospital cardiac arrest

Purpose:
We investigated the association between defoliant exposure and survival to discharge after out-of-hospital cardiac arrest.
Methods:
This is a retrospective case-control study based on the cardiopulmonary resuscitation (CPR) registry. The electronic medical records of out-of-hospital cardiac arrest victims from 6/9/2008 to 12/31/2016 were analyzed statistically. The case patients group had a history of defoliant exposure while the control group did not. Among the 401 victims studied, a total of 110 patients were male out-of-hospital cardiac arrest patients. Baseline characteristics and the parameters involved in cardiac arrest were analyzed and compared between the two groups after propensity score matching. The primary outcome was survival to discharge, and secondary outcomes were sustained return of spontaneous circulation (ROSC) and survival to admission.
Results:
After propensity score matching a total of 50 patients (case=25, control=25) were analyzed. Primary outcome (survival to discharge) was not significantly different between case and control groups [(OR, 1.759; 95% C.I., 0.491-6.309) and (OR, 1.842;95% C.I., 0.515-6.593), respectively]. In the subgroup analysis, there were also no significant differences between the control group and subgroups in primary and secondary outcomes according to defoliant exposure severity.
Conclusion
There is no statistically significant association between defoliant exposure and survival of out-of-hospital cardiac arrest.
8.Long-term results of vertical height augmentation genioplasty using autogenous iliac bone graft.

PURPOSE: In order to clarify the clinical utility of the vertical height augmentation (VHA) genioplasty using autogenous iliac bone graft (IBG), this study examined the postsurgical changes in hard and soft tissues of the chin and the stability of the grafted bone. PATIENTS AND METHODS: Twenty-three patients who had undergone VHA genioplasty using autogenous IBG were evaluated radiographically and clinically. A comparison study of the changes in hard to soft tissues after surgery in all 23 patients was performed with preoperative, 1-month, 3-months, 6-months, and/or 1-year postoperative lateral cephalograms by tracing. Stability, bone healing, and complication of the grafted bone was evaluated by follow-up roentgenograms and clinical observation. RESULTS: Between the preoperative and 6-month postoperative tracings, an average vertical augmentation of the osseous segment was 4.2 mm at menton and that of the soft tissue menton was 4.0 mm. There was a high predictability of 1: 0.94 between the amounts of hard versus soft tissue changes with surgery in the vertical plane. The position of the genial bone segment was stable immediately after surgery and soft tissue was not changed significantly from 1 month to 1 year after operation. Clinical and radiological follow-up results of the iliac bone graft showed normal bony union and were generally stable. CONCLUSIONS: VHA genioplasty using IBG is a reliable method for predicting hard and soft tissue changes and for maintaining postoperative soft tissue of the chin after surgery.

9.Functional Severity of Coronary Stenosis in Relation to Luminologic Severity in AMI: Comparison with Angina.

BACKGROUND AND OBJECTIVES: Positive correlations between quantitative coronary angiography and functional indexes of coronary stenosis are well known in angina pectoris. However, there is little data concerning correlations with acute myocardial infarction (AMI). The objective of this study was to evaluate the differences in correlation of functional severity and luminologic severity between patients with angina and acute myocardial infarction. SUBJECTS AND METHODS: The study population consisted of 23 patients with AMI and 25 patients with angina pectoris. We performed intravascular ultrasound (IVUS) and intracoronary pressure measurement following diagnostic coronary angiography, and measured angiographic diameter stenosis (DST), minimal luminal diameter (MLD), minimal luminal area (MLA), and reference area stenosis (r-AST). Additionally, the fractional flow reserve (FFR) was defined by the ratio of the distal mean coronary pressure (Pd) to the aortic mean pressure (Pa). RESULTS: The IVUS parameters and DST in patients with AMI showed more severe stenosis than seen in patients with angina; MLD (1.37+/-0.30 mm vs 1.73+/-0.63 mm, p<0.05), MLA (1.99+/-0.80 mm 2 vs 3.20+/-2.25 mm 2, p<0.05), r-AST (79+/-9% vs 64+/-24%, p<0.05), DST (78+/-14% vs 68+/-21%, p<0.05) respectively. FFR showed no significant difference between the patients with AMI and angina (0.733+/-0.14 vs 0.729+/-0.14, p>0.05). FFR was correlated less with r-AST in patients with myocardial infarction than angina ( - 0.55 vs - 0.84). The r-AST in patients with AMI, in order to be the best cut-off values that fit with a FFR<0.75, was higher than seen in patients with angina (83% vs 67%). CONCLUSION: FFR in AMI was not significantly different from that seen in angina despite the presence of a significant difference of IVUS parameters between the two patient groups. The functional severity of stenosis in relation to its luminologic severity may be lessened following acute myocardial infarction.

10.Bone & Soft Tissue Injuries Diagnosed by Magnetic Resonance Imaging in Thoracolumbar Fractures.

PURPOSE: To assess diagnostic efficacy of the MRI in thoracolumbar fractures, especially in changes of bone and soft tissue which cannot be documented by other diagnostic tools. MATERIALS AND METHODS: Among 85 patients managed for thoracolumbar fractures between January 1997 and June 2003, MRI was performed in 30 patients to get more informations. Plain X-ray, CT and MRI of these cases were reviewed retrospectively by two orthopaedic spine surgeons and one radiologist to investigate the informations which only MRI could afford. RESULTS: 14 (46.7%) among 30 patients had occult fractures of vertebrae other than main fracture which had not been diagnosed as fractured. Besides 6 patients who showed distraction of posterior structure on plain X-ray, injury of posterior ligament complex was confirmed by MRI in 12(40%) patients. Additionally, MRI visualized other soft tissue injuries such as intramuscular and subcutaneous hematoma, changes of the spinal cord and intervertebral disc. In 16 among 30 patients, informations achieved from MRI were the most important factors in deciding treatment modality. CONCLUSION: MRI seems to be efficient in visualizing not only soft tissue injury such as ligament but also occult fractures of additional vertebra in thoracolumbar fractures, therefore MRI seems to be an important diagnostic tool in decision of treatment modalities, especially in cases of uncertain stability.
