3.Communicable Disease Surveillance System and Sentinel Surveillance.

Surveillance is the ongoing and systematic collection, analysis of disease related data and dissemination of the information to the people who need it, and application of it to disease prevention and control. So, it is composed of reportable disease; system of data collection, analysis, distribution; coorporation of medical institution with public health organization and effective control. The most dynamic and important part of surveillance is the feedback mechanism. As for positive ways of feedback, there are dissemination of collected and analysed information and reimbursement to the reporters. Legal punishment is the negative way of feedback. The common systems that the developed countries use for communicable disease surveillance are notifiable disease surveillance, sentinel surveillance, and laboratory surveillance systems. Notifiable disease surveillance is for the diseases that their impact is serious to the community when epidemic occurs even though the number of cases is small. Most countries have this type of surveillance, but they suffer from low reporting rate. Sentinel surveillance is for the disease which is mild or whose trend is to be traced for the evaluation of the public health programs. Sampled reporters notifies the disease information voluntarily not by law. Laboratory surveillance is to support the notifiable and sentinel surveillance. It provides laboratory information on the microorganism and antibody level of the disease. Surveillance of communicable diseases in Korea is totally dependent on the notifiable disease reporting system, and there are many problems involved. To improve the surveillance of communicable diseases, it is strongly recommended that the sentinel and laboratory surveillance systems be adopted.

4.A Case of Treatment of Acute Renal Failure due to Bilateral Ureter Injuries after Bilateral Inguinal Herniorrhaphy.

Inguinal herniorrhaphy is the most frequent general surgical operation performed by pediatric surgeons, but some complications can usually be attributed to inappropriate approach and traumatic dissection. We experienced a case of acute renal failure due to bilateral ureter injuries after bilateral inguinal hernia repairs. The patient was a 3 month old male infant who presented as anuria and generalized edema after bilateral herniorrhaphy and was treated with peritoneal dialysis for 2 weeks. After 4 days of peritoneal dialysis, general conditions and symptoms were improved, but anuria persisted for 2 weeks. We performed IVP and abdominal CT. Both studies showed abnormal dye accumulation in peritoneal cavity. Ureteroneocystostomy on right and segmental resection of ureter and end to end anastomosis of left ureter were done without any complications. He has been doing well and has had normal kidney function. Therefore we report a case of acute renal failure due to bilateral ureter injuries after both inguinal herniorrhaphy with a brief review of literatures.

5.Incidence of Parasites in Seoul Area Based on An Examination of the Severance Hospital Out-Patients.

Of 14,682 samples of feces examined, 81.7% were positive for intestinal helminths. Trichocephalus trichiurus was observed most frequently, in 73.2%. Eight other species of helminths found were Ascaris lumbricoides, Hookworm, Trichostrongylus orientalis, Clonorchis sinensis, Tapeworm, Hymenolepis nana, Paragoniumus westermani and Enterobius vermicularis, listed in decreasing order. Double infections were found more frequently than single infections, and mixed infection of five or more species of helminths occurred in 0.2% Of 10,320 samples of feces examined, 22.3% were positive for intestinal protozoa, Entamoeba coli was observed most frequently, in 11.1%. Six other species, Endolimax nana, Giardia lamblia, Entamoeba histolytica, Iodamoeba butschlii, Chilomastix mesnill and lsospora hominis were found, in the order. It was noticed that E. histolytica was found more frequently in trichurial infection than with other helminths. Of 1,050 samples of sputum from the T. B. Clinic, 7.0% were positive for paragonimus eggs. Trichomonas vaginalis were found in 35.8%of 1,146 vaginal swabs examined. Seven cases of Plasmodium vivax infection were found among 140 suspected blood smears.

7.A Statistical Standard for Detecting Epidemic of Notifiable Acute Communicable Diseases in Korea.

Many problems have been stated in the surveillance system of notifiable acute communicable diseases in Korea. Lack of objective tools for detecting epidemic is one of the most fundamental. We propose a statistical standard for detecting epidemic of those diseases that could be easily and promptly applicable to the existing data. Suggested standard measure is computed from the median and the spread of upper and lower hinge(spr(H)) which is robust to the assumption of normal distribution, so frequently used in exploratory data analysis as a measure of variation, and the results are compared with those of existing method using recent 3 years from January 1994 to December 1996 of monthly data of 8 notifiable acute communicable diseases in Korea. Monthly pattern of statistical epidemic between the proposed (median) and existing(mean) methods is similar. Therefore, we propose that the statistical epidemic should be defined when the current occurrence exeeds the standards of both methods. When the data collection is made weekly than monthly, the proposed method of determining the time of epidemic will be much helpful for the management of notifiable diseases.

8.Cervical Ganglioneuroma Associated with Neurofibromatosis Type 1.

PURPOSE: Ganglioneuromas are well-differentiated tumors derived from neuroectodermal neural crest cells. Although these tumors can occur anywhere along the sympathetic chain from the base of the skull to the pelvic cavity, they usually develop in the posterior mediastinum and retroperitoneum these tumors are rarely found in the cervical region. METHODS: We report the case of a 16-year-old male patient with neurofibromatosis type 1 who was admitted because of a palpable mass centrally located on the left side of the neck. A preoperative contrast-enhanced neck computed tomography image showed a low-density homogeneous mass on the parapharyngeal space along with marked displacement of the trachea and carotid vessels. Round and soft masses were also detected on both axillae. RESULTS: The patient subsequently underwent complete excision of the neck mass via the transcervical approach. The mass was smooth and well encapsulated between the sternocleidomastoid muscle and the trachea. Further, the mass appeared to arise from the cervical sympathetic chain, which was preserved during surgery. Both the axillary masses were also excised. The histopathological findings were ganglioneuroma for the neck mass and neurofibroma for both the axillary masses. CONCLUSION: Cervical ganglioneuromas are rare tumors that present as enlarging parapharyngeal cervical masses in the oropharynx or neck. To our knowledge, a case of cervical ganglioneuroma associated with neurofibromatosis type 1 has never been reported. In patients with neurofibromatosis, multiple tumors may develop, and therefore periodic clinical and radiological follow-up is recommended. Further, repeated imaging analysis should be performed if the presence of another tumor is suspected.

9.Molecular cloning and characterization of peroxiredoxin from Toxoplasma gondii.

A cDNA of 1.1 kb comprising the gene encoding the peroxiredoxin of Toxoplasma gondii (TgPrx) has been cloned. The open reading frame of 591 bp was translated into a protein of 196 amino acids with a molecular mass of 25 kDa. Conserved 2 cysteine domains of Phe-Val-Cys-Pro and Glu-Val-Cys-Pro indicated TgPrx belonged to 2-Cys Prx families. TgPrx showed the highest homology with that of Arabidopsis thaliana by 53.9% followed by Entamoeba histolytica with 39.5% by the amino acid sequence alignment. Polyclonal antibody against recombinant TgPrx detected 25 kDa band in T. gondii without binding to host cell proteins. TgPrx was located in the cytoplasm of T. gondii extracellularly or intracellularly by immunofluorescence assay. The expression of TgPrx was increased as early as 30 min after the treatment with artemisinin in the intracellular stage, while no changes in those of host Prx I and TgSOD. This result implies that TgPrx may function as an antioxidant protecting the cell from the attack of reactive oxygen intermediates. It is also suggested that TgPrx is a possible target of chemotherapy.
