2.Comparison between the continuous Infusion and the Intermittent Administration of Vecuronium.

The neuromuacular blocking effects of the repeated bolus injection and continuous injection of vecuronium have been compared in 20 patients by means of evoked twitch tension. Group I received a loading dose (0.08 mg/kg) of vecuronium and followed by maintenance doae (0.02 mg/kg) when twitch tension had recovered to 25% of control. Group II received a loading doses (0. 08mg/kg) of vecuronium plus a continuous infusion delivering 0.06 mg/kg/hr. Anesthesia was maintained with nitrous oxide, oxygen and 1.5% enflurane. The neuromuscular blocking effect were reversed by the neostigmine 1.0 mg and robinul 0.2 mg when twitch tension had recovered 25% control in both groups near the end of operation. With repeated injection of vecuronium (group I), the duration of blockade of 25% recovery was 21.6+/-4.2 minutes and the recovery times from 25% to 75% was 6.8+/-1.9 minutes The continuous infusion of vecuronium (group II) produced consistent neuromuscular blockade at the average level of 90% twitch depression. The times from the end of infusion to 25% and fr,om 25% to 75% are averaged 16.0+/-2.5 and 4.2+/-0.7 minutes respectively. The reeovery index was more rapid in infusion group than maintenance group (P<0.05).

6.Present status of Enterobius vermicularis infestation among children in kindergartens.

BACKGROUND: An Epidemiological study was undertaken to evaluate the prevalence of E. vermicularis infestation among children in kindergartens. METHODS: A total of 247 cellotape anal swab specimens from kindergarten children in Inchon were examined for E. vermicularis eggs. Each specimen was collected in the morning. RESULTS: Out of 247 specimens from two kindergartens in Inchon, E. vermicularis eggs were detected in 168 specimens, the egg detection rate being 68.0%. The egg detection rates in boys and girls were 63.9% out of 133 children and 72.8% out of 114 children, respectively. The egg detection rate in girls was higher than that of boys. The egg detection rates by age group was 65.5%(age 3), S7.4%(age 4), 72.0%(age 5) and 75.4%(age 6), respectively. CONCLUSIONS: Enterobiasis is a cosmopolitan infection. Surveys for children of kindergartens indicate that E.vermicularis is probably the most widely distributed human helminth, its incidence varing from 59.2% to 73.8% in groups studied since pinworm infection is frequently a familial problem that infestation of the household frequently results in reinfection. Careful washing of hands and cleansing of fingernails before meals and after use of the toilet are the most practical measures, all member of family should be treated simultaneously when practicable.

8.Clinical Experience of Metastatic Prostatic Carcinoma.

26 cases of stage D prostatic carcinoma admitted to Dept. of Urology during the period from August 1, 1970 through July 31, 1984 were observed clinically. 1. 26 cases (43.3%) among 60 cases of prostatic carcinoma belonged to stage D at the time of diagnosis. 2. The presenting symptoms showed prostatic obstruction in 21 cases, gross hematuria in 8 cases and lumbago in 2 cases. 3. Serum acid phosphatase activity was elevated in 12 cases (46.2%). Serum alkaline phosphatase activity was elevated in 8 cases (30.8%), 6 cases of whom had osseous metastases. 4. 25 cases revealed adenocarcinoma of the prostate and 1 case revealed squamous cell carcinoma. On the pathological examination, 13 cases were poorly differentiated, 9 cases were intermediate range and 2 cases were well differentiated adenocarcinoma. 5. Orchiectomy was done in 23 cases immediately following biopsy and D. E. S. therapy was done in 7 cases with radiation therapy in 1 case. 6. 15 cases were followed up for more than 1 year, revealing survival rates 73.3% in 1 year, 66.7 % in 2 years and 60% in 3 years.

9.A Case of Congenital Midline Cervical Cleft Associated with Congenital Cardiac Anomaly.

Congenital midline cervical cleft is a rare congenital anomaly of the ventral neck. Less than 100 cases have been reported in the literature. This anomaly is part of a spectrum of midline branchiogenic syndromes resulting from abnormal migration of cells derived from the branchial arches. It presents as a small skin tag superiorly with a linear, ventrically oriented atrophic patch. We present a case of congenital midline cervical cleft associated with congenital cardiac anomaly.

10.A Case of Impetigo Herpetiformis Terminating in Fetal Death.

Impetigo herpetiformis is a form of pustular psoriasis that occurs during pregnancy and may be life-threatening. The pathogenesis and etiology of impetigo herpetiformis is not clear, but several reports have associated onset of this disorder with hypocalcemia, hypoparathyroidism, and the use of oral contraceptives. The typical lesions are erythematous patches that are studded with tiny superficial pustules particularly coalescing at their margins causing pain, and a burning sensation. Laboratory findings include an elevated erythrocyte sedimentation rate, increased peripheral blood leukocyte counts, hypoalbuminemia, and in severe cases, hypocalcemia. The histopathology is the same as that of pustula psoriasis. Our patient, a 31 year-old female (Intra Uterine Pregnancy 28 weeks), was admitted to our hospital due to generalized skin eruption and severe constitutional symptoms. Her clinical features, laboratory and histopathologic findings were compatible with that of impetigo herpetiformis. In spite of our all efforts, intrauterine fetal death was noted at her 33rd week of pregnancy. We report a case of impetigo herpetiformis terminating in fetal death with the review of literature.
