Panniculitis is an inflammation of subcutaneous fat tissue, and found in variety of clinical setting, like erythema nodosum, lupus or other connective tissue diseases. Panniculitis, as a clinical feature of dermatomyositis, is rare and should be included in one of the sign to be differentiated. We present a 77-year-old female dermatomyositis patient who showed a clinically and histologically evident lobular panniculitis, and successfully treated with oral prednisolone.

Chronic erythema nodosum, a variant of erythema nodosum, is characterized by the presence of non-ulcerated lesions located mainly on the legs and by a lack of a migratory tendency. These lesions can persist for years. Hypertrichosis is defined as the excessive growth of hairs and is classified into generalized or localized forms. An acquired and localized hypertrichosis has been associated with Becker's nevus, pretibial myxedema, local inflammation or irritation, orthopedic casts and splints, and topical application of steroids. A 48-year-old Korean female visited our clinic for evaluation of a 2-year history of scattered, erythematous to brownish, tender patches or subcutaneous nodules on both shins. Her skin lesions had waxed and waned under treatment, and after 6 months localized hypertrichosis had appeared on her skin lesions. We report a case of acquired and localized hypertrichosis associated with chronic erythema nodosum.

We evaluated the therapeutic effect of potassium iodide in fourteen patients with erythema nodosum. The results were as follows: 1) All patients were given a saturated solution of potassium iodide, 600 to 900 mg orall y a day for 3 days to 5 weeks. 2) Three patients were observed initial days, two patients within 9 days. 3) There were no correlations between therapeutic effect of potassium iodide. 4) Specific side effects were not found during the treatment with potassium iodide.

We report three cases of sarcoidosis with pulmonary involvement. The skin lesions showed three types of cutaneous manifestations which were erythema nodosum, pruritic maculopapular eruptions, and subcutaneous nodules respectively. Erythema nodosum in our cases accompanied by arthralgia and bilateral hilar adenopathy, so we call it Lofgrens syndrome. Histopathologic findings of the case with eythema nodosum revealed septal panniculitis and the others showed characteristic histopathologic findings of sarcoidosis so-called naked tubercle. Regressions of the skin lesions were obtained with systemic and/or topical corticosteroid therapy except for subcutaneous nodules. Subcutaneous nodules initially improved but new lesions have been found.

We experienced a case of erythema nodosum of unknown etiology successfully treated with therapy based on Kampo diagnosis. The case was 44 year-old woman who had erythema nodosum over her bilateral leg extensors several times a month, became cold easily and had stomatitis. At first, we administered seinetsuhokito, which resulted in the disappearance of her stomatitis. However, she still had a recurrence of her erythema nodosum. Because of a condition of interior heat and qi counterflow, we considered her situation heibyo. We administered seinetsuhokito and byakkokakeishito every other day, which resulted in the disappearance of both her stomatitis and erythema nodosum. Therefore, we suggest that it is worthy to think about heibyo situations, when treating patients with complications.
