1.Rules of lymphatic metastasis of rectal carcinoma

Objective To investigate the regulations of lymphatic metastasis of rectal carcinoma which can provide help of clinical diagnosis and treatments.Methods A retrospective analysis was performed of 222 patients with rectal carcinoma who underwent surgical resection from November 2006 to January 2012.Summarize the relevance of each factor and lymph node metastasis by logistic regression analysis.Results (1) Gender and lymph node metastasis has no significant correlation (x2 =0.000,P > 0.05).(2) Lymph node metastasis rate reduced with increasing age and the difference was statistically significant (x2 =5.001,P < 0.05).(3) There is no significant difference between tumor location and lymph node metastasis rate (x2 =0.052,P > 0.05),when the distance between anal rectal cancer and the anus was closer,the tendency of lymph node metastasis was higher,and there has the obvious correlation with lymph node metastasis rate of its size,2.5 to 5.5 cm group and more than 5.5 cm lymph node metastasis rate was significantly higher than less than 2.5 cm group with a significant difference (x2 =2.267,P < 0.05).(4) Tumor invasion of the intestinal circumference was closely related with lymph node metastasis rate (x2 =2.323,P <0.01).(5) Lymph node metastasis rate was closely related to the depth of invasion,metastasis rate increased with the depth of invasion increasing(x2 =2.277,P <0.05).(6) Lymph node metastasis was significantly higher with the decrease of the degree of tumor differentiation (x2 =4.322,P < 0.01).Logistic regression analysis showed that the degree of tumor differentiation,infiltration and invasion of intestinal circumference diameter lymph node metastasis influential,OR (95 % CI) was 5.562 (0.105-0.444),2.178 (0.144-0.971),1.563 (0.038-8.613) respectively (P <0.01).Conclusion The lymphatic metastasis of rectal carcinoma is related to age,tumor size,intestinal circumference invasion,depth of tumor invasion and the differentiating degree of the tumor,and the differentiating degree is the major factor.
2.Influence of the growth traits and virulence genes of Enterococcus faecalis isolate N41 by cocultivation with Salmonella

To explore the impact of Salmonella on Enterococcusfaecalis isolate N41 under the coexisting,the growth traits and the transcription of 26 virulence genes of N41 at various growth phases were detected.Salmonella and/or N41 were inoculated and done plate counting,then growth curves were drawn and bacterial total RNA were extracted at given time points,quantitive realtime PCR was used to analyze the RNA transcription levels of 26 virulence genes of N41.The result showed that comparison with single incubation,the bacteria concentration of N41 dropped about 3.9 times,and Salmonella dropped about 110-times.Among 26 virulence genes of N41,the RNA transcription levels of 12 virulence genes such as ebpA,ebpC,rnjB,ace,ebpR,psr and so on were promoted at the four growth phases,but the RNA transcription levels of SlyA and sprE were dropped.Except that the RNA transcription levels of CylL-S,CylL-L,efaA and AS had no significant changes at the four growth phases,the mostly rest genes increased dramatically at post-log phase.This indicated that when incubated together,N41 inhibited the growth of Salmonella significantly,and Salmonella promoted the transcription levels of virulence genes of N41 as a whole.The results lay a foundation for further research on the interaction between bacteria species and the pathogenicity mechanism of Enterococci.
3.Clinical analysis of 3D printed total hip arthroplasty in elderly patients with acetabular fracture

Objective:To study the clinical efficacy of 3D total hip arthroplasty in elderly patients with acetabular fracture.Methods:A total of 43 patients with non-obsolete acetabular fractures admitted to Xining First People's Hospital from May 2016 to December 2018 were randomly divided into the control group (22 cases) and the observation group (21 cases) for a perspective study. The control group received routine surgery and the observation group was treated with total hip arthroplasty combined with 3D printing. The patient's operation time, intraoperative blood loss, and hospitalization time were recorded. The hip function was assessed by Harris Hip Scale (HHS) after 6 months of follow-up. The patient's quality of life was assessed using the short form 36 item health survey questionnaire (SF-36) before and 6 months after surgery. The complications of the patients during operation and follow-up were recorded.Results:The operation time, hospital stay and intraoperative blood loss were significantly lower in the observation group than those in the control group ( P<0.05). At 6 months after operation, the hip joint function of the observation group was significantly better than that of the control group ( P<0.05); the quality of life scores were significantly higher than the control group ( P<0.05). There was no significant difference in the incidence of adverse reactions between the two groups ( P<0.05). Conclusions:3D printed total hip arthroplasty can effectively improve the operation and hip function of elderly patients with non-obsolete acetabular fracture and the quality of life of patients without increasing adverse reactions.
