2.Gene therapy for erectile dysfunction: an update.

Erectile dysfunction (ED) affects over 50% of men between 50 and 70 years of age, and 40% of men aged about 40 years suffer from some form of erectile dysfunction. ED, with more than 100 million sufferers in the world, has become a serious disease besetting the male patients. Great strides have been achieved in understanding the pathogenesis and potential therapeutics of ED over the past two decades. The development of phosphodiesterase type 5 (PDE5) inhibitors has revolutionized the treatment of ED, but they are not as effective on ED related with diabetes, post-prostatectomy condition and severe veno-occlusive disease. As a potential therapeutic option for ED, gene therapy might provide an effective means. This article presents an overview on the progress of gene therapy in the management of ED associated with diabetes, aging, nerve injury and vascular lesion.

4.Psychological factors involved in erectile dysfunction.

Erectile dysfunction (ED) is a major sexual dysfunction involving both physiological and psychological factors. While the primary physiological cause of ED is associated with endocellular degeneration and disorders of neural reflex, its psychological causes are mainly hyposexuality, anxiety and depression. Increasing importance is being attached to the psychological aspects in the diagnosis and management of ED in recent years, and the evaluation questionnaires aimed at the psychosocial significance in ED patients' conditions and psychological interventions solo or combined with drugs have inspired many researches for an optimal solution of the disease. Improvement of the patient's erectile performance is no longer the only outcome we pursue. Instead, the satisfaction of both the patient and his partner has a rising weight in the final evaluation of a therapy. The wide application of PED5 inhibitors and their psychological effect on ED patients makes it necessary for us gain a deeper insight into the combination of psychological intervention and drug therapy in the management of ED.

7.Efficacy of tadalafil for erectile dysfunction: an updated review.

Tadalafil is an effective drug in treating erectile dysfunction (ED), and its clinical efficacy has been confirmed by a great many researches. Tadalafil is distinguished from sildenafil and vardenafil by its prolonged action lasting 36 hours for a sigle dose, compared with about 4 hours for sildenafil. Furthermore, this drug is effective in improving the erectile function of ED patients including those with various comorbid conditions. Tadalafil can help ED patients to regain morning erection and recover the confidence as a man. More and more ED patients choose tadalafil as the first line therapy because of its long efficacy and its conformability to the therapeutic requirement by restoring ED patients to normal, natural and pleasurable sexual life.

Mild-symptom erectile dysfunction (MSED) is commonly seen in clinical practice, but receives inadequate attention from both the patients and clinicians. Increasing researches have indicated that MSED is associated with not only unhealthy living habits and psychological factors but also the early progression of endothelial, metabolic and endocrine diseases. The diagnosis and treatment of MSED should be based on the relevant guidelines, with consideration of both its specific and common features. The therapeutic principle is a combination of integrated and individual solutions aimed at the causes of the disease. Drug intervention should be initiated if psychological therapy fails. Negligence of MSED may affect the quality of life of the patients and their partners, and what's more, might delay the management of some other severe underlying diseases. Adequate attention to the early diagnosis and treatment for MSED is of great significance for a deeper insight into the etiology of ED, the prevention of potential cardiovascular and metabolic diseases, and the improvement of the overall health of males.

9.Hyperbaric oxygenation and penis erectile dysfunction.

Relaxation and contraction factors influencing penile erection are produced and released by the central and peripheral nerves as well as intracavernosal sinus gap and vascular endothelial cells. Aging, diabetes, cardiovascular disease and spinal cord injury can influence these factors. Further researches of hyperbaric oxygenation (HBO) on the erectile dysfunction (ED) can provide some theoretical evidences for the clinical treatment of ED.
