Spitz nevus is a variant of melanocytic nevus which is histopathologically defined as large spindle and/or epithelioid cells. Angiomatoid Spitz nevus is a rare histologic variant of desmoplastic Spitz nevus characterized by prominent vasculature. We present a case of angiomatoid Spitz nevus, celluar type, that has not been reported before. We provide another example to show the remarkable diversity of Spitz nevus.

4.Four Cases of Spitz Nevus.

Spitz nevus is a benign tumor of melanocytic origin. The lesion is a usually asymptomatic, solitary, reddish or pinkish, hairless, dome-shaped, smooth-surfaced, and firm papule or nodule, measuring less than 1.0cm in the greatest diameter. In most instances, it is encountered most commonly on the face and extremities in the first two decades of life. The histologic picture of the lesion reveals numerous uniform nests of spindle cells and epithelioid cells with sparse pigmentation in the dermis. We report herein four cases of Spitz nevus with a review of literatures.

6.A Case of Multiple Agminated Spitz Nevi Showing Desmoplastic Changes.

We report a case of multiple agminated Spitz nevi in a 16-year-old boy who presented with multiple nodules on the right side of his face. His past history revealed that there had been erythematous and brown colored papules on the same site at the age of four. At that time one lesion was excised and histopathological findings showed fairly well circumscribed spindle and epithelioid cell nests consistent with a Spitz nevus. Twelve years later, he presented with multiple agminated brown to black colored nodules on the face. Microscopic evaluation revealed nevus cells diffusely distributed throughout the dermis showing no or little junctional activity. In addition, these cells were embedded in a strikingly desmoplastic stroma which consisted of acellular collagen fiber. Immunohistochemical staining for S-100 protein was positive. To our knowledge this is the first report of multiple agminated Spitz nevi in Korea.

7.A Case of Primary Cutaneous Perivascular Epithelioid Cell Tumor.

Perivascular epithelioid cell tumors (PEComas) are a rare, recently described group of mesenchymal tumors, which is composed of distinctive perivascular epithelioid cells. Primary cutaneous PEComas are composed of focally nested or trabecularly arranged groups of epithelioid cells with clear to eosinophilic cytoplasm and round to oval nuclei with obvious nucleoli. Here, we describe the case of a 27-year-old Korean man who was presented with a solitary, cutaneous nodule on his left popliteal fossa. Physical examination revealed a solitary, 1.2x1.0 cm-sized erythematous protruding nodule on his left popliteal fossa. Immunohistochemical evaluation showed that the epithelioid tumor cells were positive for HMB-45 and desmin. These clinical and histological findings were consistent with a diagnosis of primary cutaneous PEComa.

8.A Case of Spitz Nevus on the Ear Helix.

Spitz nevi, benign melanocytic tumors, have some histologic resemblance to malignant melanoma. They usually appear as asymptomatic, solitary, reddish to pinkish, hairless, dome-shaped, round to oval, smooth-surfaced papules or nodules, measuring less than 1.0 cm in diameter. Histologically, these lesions display striking nests of large spindle cells and epithelioid cells, usually extending from the epidermis to the reticular dermis. Here in, we report a case of Spitz nevus on the helix of the right ear of a 23-year-old man which was treated with Er:YAG laser.

9.A Case of Pigmented Epithelioid Cell Nevus.

Pigmented spindle cell nevus was described as a variant of spindle and epithelioid cell nevus (Spitz nevus). However, it hs been classified as a separate disease because of the presence of large pigmented cells confined to the epidermis and papillary dermis. Pigmented epithelioid cell nevus is considered as a counterpart to pigmented spindle cell nevus, because it has similar clinicopathologic features to those of pigmented spindle cell nevus, except for tumor cells being entirely composed of large epithelioid cells. Herein, we report a case of a 31 year-old woman with a deeply-pigmented nodule which was entirely composed of large epithelioid cells, and showed many histologic findings of pigmented epithelioid cell nevus.

10.A Case of Atypical Spitz Nevus.

A 6-year-old boy presented with a fast-growing, 0.8x0.7 cm-sized, dome-shaped, erythematous nodule on the back. Histopathologically, the lesion showed a spitzoid melanocytic proliferation from the dermo-epidermal junction to the reticular dermis. Melanocytes arranged in nests were epithelioid and spindle-shaped with abundant eosinophilic cytoplasm. The histopathological and clinical features were consistent with atypical Spitz nevus in that they showed no Kamino body, prominent cellular density, more than three mitoses per high power field, a high Ki-67 level, large size, and the lesion was located at an unusual location. Herein, we report a case of an atypical Spitz nevus on the back of a 6-year-old boy.
