A case of 76-years-old male patient with nasal bleeding as the first symptom in our hospital, who was finally diagnosed as tsutsugamushi disease. This old man was bited by insect in farmland 2 days before the symptom occurred. PE: Left thigh and right buttock have eschar, with splenomegaly. Routine blood test: WBC (decrease) 3.9 x 10(9)/L, RBC (decrease) 3.86 x 10(9)/L, PLT (decrease) 41 x 10(9)/L, HGB (decrease) 117 g/L; Chest CT: lung interstitial pneumonia, a small amount of bilateral pleural effusion. Oxk-ag 1:320. The patient was discharged after treatment with chloramphenicol for 8 days.

OBJECTIVE: To investigate the cause of epistasis in patients irradiated for nasopharyngeal carcinoma. METHOD: Seventy nasopharyngeal carcinoma cases with epistasis after radiotherapy were retrospectively analyzed. RESULT: The causes of epistasis were local nasopharyngeal carcinoma recurrent 34.3% (24/70), pseudoaneurysm 14.3% (10/70) and nasopharyngeal crust 51.4% (36/70). Seven cases with local recurrent, 3 cases with pseudoaneurysm and 1 cases with nasopharyngeal crust and osteoradionecrosis died of intractable epistasis. CONCLUSION Epistasis in patients irradiated for nasopharyngeal carcinoma is dangerous complication, especially in cases with local recurrent and pseudoaneurysm. Nasopharyngeal crust must be treated actively.

The factor VIII gene is located on the X chromosome,making haemophilia A, a sex-linked disorder. Thus on pedigree grounds all daughters of such patients are obligate carriers. One female case of haemophilia A was reported and related literatures were reviewed.

OBJECTIVE: To evaluate the etiology and management of delayed epistaxis after endoscopic surgery. METHOD: To retrospectively analyze the clinical data of 11 cases for epistaxis after nasal endoscopic surgery. To compare their precipitating factors, their surgical approach, time and site of bleeding and management in order to find the intrinsic rules. RESULT: The precipitating factors, bleeding sites and treatments varied among patients. CONCLUSION Delayed epistaxis after endoscopic surgery can not be neglected. There may be some precipitating factors. The surgical approach may be related to the bleeding site. It is better to treat the epistaxis using the endoscope to explore the bleeding site and to give corresponding intervention.
