2.A Case of Oropharyngeal Teratoma with Cleft Palate.

The oropharyngeal teratoma is a relatively uncommon tumors which can present as airway obstruction in neonates. We experienced a case of oropharyngeal teratoma which was attached to soft palate and combined with cleft palate and caused respiratory distress necessitated orotracheal intubation. We report the one case of oropharyngeal teratoma treated with endoscopic excision with a brief literatures.

3.A Study of Airborne Microbes in the NSICU According to Number of Persons Who Pass through Every Hour.

PURPOSE: The purpose of this study was to analyze the colony count of airborne microbes contamination every hour in the Neurosurgical Intensive Care Unit (NSICU) in order to identify the relationship of colony count to person-visits. METHODS: Data were collected during from 11:00 a.m. September 5 to 11:00 a.m. September 6, 2002. This study used blood agar & nutrient agar and handtally counter (USA) for collection of airborne microbes and number of person-visits. Data was analyzed using the SPSSWIN 10.0 with means, Pearson correlation coefficient, and simple regression. RESULTS: The result of this study are as follows. Total colony count of airborne microbes for 24 hours in the NSICU was 4,609. Total number of person-visits to the NSICU was 15,347. The highest scores for the total colony count in different areas of the NSICU was the rear door, followed by the preparation room, and the front entrance, while the lowest count was in the isolation rooms. There was a statistically significant relationship between colony count and number of person-visits to the NSICU. The most frequently airborne microbes in the NSICU were Micrococcus, CNS, Staphylococcus Micrococcus, Aureus. CONCLUSION: These findings indicate that the number of person-visits in hospitals influences total colony count of airborne microbes. This study contributes to assessment of biological indoor air quality in hospital and in the development of an NSICU care plan.

4.Quantitative Measurement of Dysphonia Severity in Patients With Stroke With Unilateral Vocal Cord Palsy

Unilateral vocal cord palsy (UVCP) is frequently observed in patients with stroke. This study aimed to evaluate the association between objective dysphonia severity and the classification of UVCP in patients with stroke by objectively and quantitatively measuring their phonetic function. We recruited patients with UVCP diagnosed using laryngoscopy after stroke.Subgroups were divided according to UVCP type, and the dysphonia severity index (DSI) and maximum phonation time (MPT) were measured to objectively evaluate dysphonia. The DSI and MPT were compared between subgroups using analysis of variance with Tukey’s honest significant difference post hoc test. In total, 103 patients with stroke and UVCP were recruited. We found that a higher UVCP severity possibly had to do with lower DSI and MPT values. We objectively confirmed that phonetic function was worse in patients with stroke with higher UVCP severity, and the DSI and MPT tests can be helpful in determining the severity and need for additional evaluation.
5.Indoleamine 2,3-Dioxygenase in Hematopoietic Stem Cell-Derived Cells Suppresses Rhinovirus-Induced Neutrophilic Airway Inflammation by Regulating Th1- and Th17-Type Responses

Asthma exacerbations are a major cause of intractable morbidity, increases in health care costs, and a greater progressive loss of lung function. Asthma exacerbations are most commonly triggered by respiratory viral infections, particularly with human rhinovirus (hRV). Respiratory viral infections are believed to affect the expression of indoleamine 2,3-dioxygenase (IDO), a limiting enzyme in tryptophan catabolism, which is presumed to alter asthmatic airway inflammation. Here, we explored the detailed role of IDO in the progression of asthma exacerbations using a mouse model for asthma exacerbation caused by hRV infection. Our results reveal that IDO is required to prevent neutrophilic inflammation in the course of asthma exacerbation caused by an hRV infection, as corroborated by markedly enhanced Th17- and Th1-type neutrophilia in the airways of IDO-deficient mice. This neutrophilia was closely associated with disrupted expression of tight junctions and enhanced expression of inflammasomerelated molecules and mucin-inducing genes. In addition, IDO ablation enhanced allergenspecific Th17- and Th1-biased CD4 + T-cell responses following hRV infection. The role of IDO in attenuating Th17- and Th1-type neutrophilic airway inflammation became more apparent in chronic asthma exacerbations after repeated allergen exposures and hRV infections. Furthermore, IDO enzymatic induction in leukocytes derived from the hematopoietic stem cell (HSC) lineage appeared to play a dominant role in attenuating Th17- and Th1-type neutrophilic inflammation in the airway following hRV infection. Therefore, IDO activity in HSC-derived leukocytes is required to regulate Th17- and Th1-type neutrophilic inflammation in the airway during asthma exacerbations caused by hRV infections.
6.In-vivo Studies on Effect of Lipo-PGE1 on Neoangiogenesis of Composite Graft in a Rabbit Model.

PURPOSE: The survival of composite graft is dependent on three steps, (1) plasmatic imbibitions, (2) inosculation, and (3) neovascularization. Among the many trials to increase the survival rate of composite graft, prostaglandin E1 (PGE1) has beneficial effects on the microcirculatory level with vasodilating, antithrombotic, anti-inflammatory and neoangiogenic properties. Lipo-PGE1 which is lipid microspheres containing PGE1 had developed to compensate the systemic and local side effects of PGE1. This study was proposed to determine whether Lipo-PGE1 administration enhanced the survival of composite graft through neovascularization quantitatively in a rabbit ear model. METHODS: Fourteen New Zealand White Rabbits each weighing 3~4 kg were divided in two groups: (1) intravenous Lipo-PGE1 injection group and (2) control group. A 2 x 1 cm sized, full-thickness rectangular composite graft was harvested in each auricle. Then, the graft was reaaproximated in situ using a 5-0 nylon suture. For the experimental group, 3 microgram/kg/day of Lipo-PGE1 (5 microgram/mL) was administered intravenously through the marginal vein of the ear for 14 days. The control group was received no pharmacologic treatment. On the 14th postoperative day, composite graft of the ear was harvested and immunochemistry staining used Monoclonal mouse anti-CD 31 antibody was performed. Neoangiogenesis was quantified by counting the vessels that showed luminal structures surrounded by the brown color-stained epithelium and counted from 10 random high-power fields (400x) by independent blinded observer. Statistical analysis (Wilcoxon Signed Ranks test for nonparametric data) was performed using SPSS v12.0, with values of p<0.05 considered significant. RESULTS: The mean number of the microvessels was 15.48 +/- 8.65 in the experimental group and 9.82 +/- 7.25 in the control group (p=0.028). CONCLUSION: The use of Lipo-PGE1 facilitated the neoangiogenesis, resulted in the improvement of the survival rate of graft. On the basis of this results, we could support wider application of Lipo-PGE1 for more effective therapeutic angiogenesis and successful survival in various cases of composite graft in the human.

OBJECTIVES: Schizophrenia has been considered to be characterized by an abnormality in attention, especially in the executive control. Emotion is an important component of the executive control. The aim of this study was to investigate the influence of emotion on the executive control in patients with schizophrenia. METHODS: Participants were 20 healthy controls and 19 subjects with schizophrenia. They viewed full-color pictures selected from the International Affective Picture System. During each trial, an emotional picture, which was either positive or negative, lit up on either the left or right side. Participants were instructed to respond to the emotional valance of each stimulus by pressing a button with their left or right index finger, while ignoring its presented side. RESULTS: There was a group difference in the response time, and patients with schizophrenia exhibited an impairment in the executive control of emotional information. However, there was no difference in the response time between the emotional conditions. In the patient group, the missing rate in the positive emotional condition was correlated with the severity of social anhedonia, whereas the missing rate in the negative emotional condition was correlated with the severity of positive symptoms. CONCLUSION: Patients with schizophrenia have a deficit in the executive control of positive emotional information as well as negative emotion, but it may be due to different underlying mechanisms.

9.Pulmonary Coccidioidomycosis in Immunocompetent Patient.

Coccidioidomycosis is a fungal infection caused by the soil fungus, Coccidioides immitis, which is endemic to the south-western United States. However, the incidence of coccidioidomycosis has recently increased due to the increase in overseas travel to endemic areas. We report a case of pulmonary coccidioidomycosis diagnosed in an immunocompetent person. A 28-year-old female, who had lived in Phoenix, Arizona, USA for 2 years, was admitted for an evaluation of persistent cough with fever lasting for 2 weeks. The chest X-ray and Chest CT revealed multifocal patchy consolidation and ground-glass opacity in both lungs as well as multiple enlarged right hilar and paratracheal lymph nodes. A percutaneous needLe biopsy of the main mass-like consolidation confirmed mature spherules of Coccidioides immitis in lung tissue. Pulmonary coccidioidomycosis should be considered in patients presenting with persistent cough with fever and a history of travel to or immigration from an endemic area.

10.Expression of pS2/TFF1 Protein in Normal Colonic Mucosa, Adenoma and Adenocarcinoma.

BACKGROUND: The trefoil factor 1 protein (pS2/TFF1) is a candidate tumor-suppressor protein, and it is a pleiotropic factor involved in the organization and homeostasis of the gastrointestinal tract and various inflammatory or neoplastic diseases. The purpose of this study was to assess the expression of pS2/TFF1 and its clinicopathologic relationship, including the p53 and Ki-67 labeling index, in colorectal carcinogenesis. METHODS: The expression of pS2/TFF1 protein was evaluated immunohistochemically in 45 samples of normal colonic mucosa, 43 samples of adenoma and 186 samples of colorectal carcinoma. RESULTS: pS2/TFF1 protein was expressed weakly in 37.8% of normal colonic mucosa samples, and it had a weak to strong expression in 48.8% of adenomas and 28% of colorectal adenocarcinomas. pS2/TFF1 expression in carcinoma was slightly increased in the poorly differentiated group compared with the well to moderately differentiated group (p=0.059). Interestingly, mucinous carcinoma (4/4) and signet ring cell carcinoma (2/3) showed significant increase of pS2/TFF1 expression. pS2/TFF1 expression was inversely correlated with the p53 protein expression and the Ki-67 labelling index (p<0.05). There was no significant correlation with the tumor size, metastasis or pathologic staging. CONCLUSIONS: Overexpression of pS2/TFF1 expression in colorectal adenocarcinoma was inversely correlated with the Ki-67 labelling index and the p53 expression in cancer. These results suggest that pS2/TFF1 protein may contribute as tumor suppressor factor in colorectal adenocarcinoma.
