1.RESULTS OF DIAGNOSTIC AND OPERATIVE HYSTEROSCOPY WHICH STUDIED IN FIRST MATERNITY HOSPITAL

Trough WHO recommendation hysteroscopy is the golden standart technique of uterine cavity evalution. First Maternity Hospital of Mongolia have been implemented gynecological laparoscopic surgery since 2009, then from 2013 we have started diagnostic and operative hysteroscopy. A hysteroscopy may be done to find the cause of abnormal bleeding or bleeding that occurs after a woman has passed menopause. It also may be done to diagnose infertility. Also a hysteroscopy can be used to remove growths in the uterus, such as fibroids or polyps. We evaluated results of gynecological diagnostic and operative hysteroscopy, which was done in First maternity hospital.We took special questionnaires from 39 women and did prospective analyses.39 patients, who were done hysteroscopy were involved our study from November 2013 to January 2015. 51% of participants were reproductive age women. Under 20 years old participants who underwent diagnostic hysteroscopy due to hyperplasia of endometrium were 3%, they were performed biopsy. From all participants 87,2% performed operative hysteroscopy, 12,8% was diagnostic hysteroscopy. After diagnostic and operative hysteroscopy, 97% of patients had no symptoms, but last 3% of patients had lower abdominal pain. If clarify diagnosis which is approved after diagnostic and operative hysteroscopy 51.3% was displaced IUD, 35.9% was endometrial polyp, 2.6% was hyperplasia endometrium, 10.3% was infertility. Complication was 2.6% through postmenopausal participants if compared with premenopausal women. Diagnostic and operative hysteroscopic procedure has benefits for synehia, septum of uterus, endometrial hyperplasia, abnormal uterine bleeding, submucosal myomectomy. From all participants 87,2% performed operative hysteroscopy, 12,8% was diagnostic hysteroscopy. Hysteroscopy was done when displaced IUD (51.3%), polyp of endometrium (35.9%), hyperplasia of endometrium (2.6%), and infertility (10.3%).Complications after hysteroscopy depends from menopause.

Endometriosis is described as a chronic inflammatory disease, characterized by endometrial-like tissue, found outside the uterine cavity which cause chronic pelvic pain, infertility,dysmenorrhea. The prevalence of endometriosis is difficult to determine accurately but in asymptomatic women, the prevalence of endometriosis ranges from 2- 22 %, depending on the population studied , in infertile women 20-50 % and in those with pelvic pain, between 40-50% (Balasch, 1996; Eskenazi, 2001; Meuleman, 2009).Endometriosis is found 7-10% of reproductive agewomen and 20-90% in with chronic pelvic pain, infertility cases. Pathogenesis of endometriosis is not yet fully understood but one potential cause of the disease is retrograde menstruation which results in the deposition of endometrial tissue into the peritoneal cavity. Today a composite theory of retrograde menstruation with implantation of endometrial fragments in conjunction with peritoneal factors to stimulate cell growth is the most widely accepted explanation for peritoneal endometriosis. Susceptibility to endometriosis is thought to depend on the complex interaction of genetic, immunologic, hormonal and environmental factors. To determine prevalence and severity of clinical symptoms, compare meta-analysis to changes the clinical value of serum CA-125 and peritoneal fluid cytology in women with endometriosis of Ulaanbaatar city. We had selected total of 60 woman with endometriosis which were registered from January to December 2014 in gynecologic clinic of First Maternity Hospital. The research group registered in questionnaire with 28 questions. During the inspection laboratory analysis of serum CA-125, ultrasound analysis and peritoneal fluid cytology were done. Assessment of pelvic pain by means of a 10-point linear analog scale / pain score/ which provided by International Pain Association. The research result was worked out by prospective method. Average age of patients 33.4±8.9. Pain location: Chronic pelvic pain 30%; Dysmenorrhea 28.3%; Dysparunea 10%; Pain during defecation 6,7%; Without pain -25%. Average level of Serum СА125 was 38.13±20.6. Location of endometriosis: adenomyosis - 8.4%, endometrioma-15% endometriotic lesion at cul de suc 68.3%, rectal involment 1.7%, tubal lesion-3.3%, combined 3,3%. 76.7% of surgery for endometriosis have done by laparoscopy and 23.3% by laparotomy. Ultrasound examination result: endometrioma d=0-2cm-1.7%, d= 3-5cm-36.2%, d=6-8cm-10.3%, d=9cm<-12.1%. Cytology result: Peritoneal fluid contains 75% of erythrocytes, mesothelial cells and it needs to further study.CONCLUSION:Most of patient /58.3%/ had chronic pelvic pain and dysmenorrhea. The severity of pain was significantly improved after operative laparoscopy. /p<0.05%/51,7% of patient had infertility problem.Value of serum CA-125 was higher in study group with large sized and not clear content ovarian endometrioma by ultrasound examination. /p<0.05%/The local environment of peritoneal fluid surrounding the endometriotic implant is immunologically dynamic and links the reproductive and immune systems. Peritoneal factors to stimulate cell growth is the most widely accepted explanation for peritoneal endometriosis. Peritoneal fluid contains a variety of free floating cells, including macrophages, mesothelial cells, lymphocytes, erythrocytes, eosinophils and mast cells. In our cytology results: peritoneal fluid contains 75% of erythrocytes, mesothelial cells.

4. RESEARCH ABOUT LAPAROSCOPY ASSISTED VAGINAL HYSTERECTOMY

Hysterectomy is still the most common major operation performed by gynecologists. Having said that the incidence of hysterectomy has increased last 20 years with new minimally invasive technique. Laparoscopy assisted vaginal hysterectomy has advantage like small incision, less operative pain, quicker recovery, less hospital stay comparing to the abdominal open surgery. Objective: To compare the outcome of abdominal hysterectomy and laparoscopic vaginal hysterectomy.The study population comprised all patients who had LAVH and abdominal hysterectomy at First Maternity Hospital, from June 2013 to December 2014.The mean age of both group is same (46.2±5.3). Duration time of LAVH is approximately 2.30±-5.9minutes and of total abdominal hysterectomy is a 1.41±2.7 minute. The general blood loss during LAVH is 74.4±19 ml and during total abdominal hysterectomy is 185.2 ± 67 ml. After LAVH any patient had no complaints. After LAVH the patient stayed at the hospital for 3 days. LAVH took significantly longer operating time than TAH. Blood loss was jess during LAVH. Hospital stay after LAVH was less than TAH. Drug cost is more case LAVH.
5.Diagnosis of intraoperative squash cytology in brain tumors

Background: The intraoperative smear cytology (squash preparation) is fairly accurate, simple, reliabletool for rapid intraoperative diagnosis of neurosurgical biopsies [1]. In the early 1930s, Dr Eisenhardtand Cushing introduced intraoperative cytological investigations for rapid examination of neurosurgicalspecimens and guidance of surgical treatment [2]. Besides rapid decision making during neurosurgicalprocedures, it is also to be ensured that minimum injury is caused to the normal brain structuressurrounding the intracranial neoplasm. It has become necessary for pathologists to train themselves inthe interpretation of cytomorphological features of various central nervous system lesion and used inmany neurosurgical units of all over the world [3]. Thus, to begin doing intraoperative squash cytology inour practice is the aim of this study.Aim: To study the diagnostic accuracy of squash cytology in the intraoperative diagnosis of centralnervous system tumorsMaterials and Methods: The material for this study was obtained from 30 cases of nervous systemneoplasms sent for intraoperative consultation from State Third Central Hospital, Shashtin to the thirdsection of National Center for Pathology between 7th of November 2013 to 28th of March 2014. Squashpreparation was performed on all cases and stained with hematoxylin and eosin. The diagnosis given onsquash cytology was then compared with final diagnosis given on paraffin-embedded sections.Results: Of total 30 cases of primary central nervous system tumors, neuroepithelial tumors are thelargest category of tumors (50%). The accuracy of squash cytology was 82.1%.Conclusion: This is the first study in our country where intra-operative squash cytology in brain tumorwas done and diagnostic accuracy was 82.1% as very good category. There were 4 cases had diagnosticdiscrepancy.

toward an advanced, as our country medical
services are trying to that country’s technology
has been introduced. In our country every
day there is only surgical care necessary to
endocrine gland disorders, including most
common thyroid surgery. It is open and
robotic assistant endoscopic surgery in the
world. But robotic assistant endoscopic
surgery is too expensive and impossible in
our country. Thus we chose Thunderbeat
for open thyroid surgery in especially huge
enlarged goiter and vascularized goiter to try
prevent bleeding, recurrent laryngeal nerve
palsy and post operation hematoma and
other complications
Thunderbeat is new generation instrument
combines an advanced bipolar clamp to the
existing ultrasonic cutter.
Materials and Methods: M 51-years-oldwoman
was admitted to Mongol Hyundae
hospital because of front of neck pain, multi
nodular goiter, tachycardia, sweaty and not
controlling emotion.
Anamnesis: She diagnosed to
Thyrotoxicosis in 2005 and she took
medicine last ten years but not controlling
that poisoning. Status locals: Huge enlarged
goiter in front of her neck
Treatment plan:

2. Operation: Open total thyroidectomy
3. Medicine
4. Wound dressing
5. Observation
We did operation after day of admission
day, Patient is placed in a Semi erect position
with a folded sheet underneath the shoulders
so that the head is sharply angulated backward
on the multifunctional surgical table under
general anesthesia. We used thunderbeat
from muscular to all procedures, dissection
both thyroid gland and isthmus after Kocher
skin incision and sub skin tissue dissected.
There is no complication while procedure
and post operation days
Result: Nodular hyperplasia of thyroid
gland with focci of micro adenomatous
change
Conclusion: Thunderbeat in use open
thyroid surgery first outcomes blood loss
than 30ml, no injury of recurrent laryngeal
nerve and no penetration ligament of Berry
while procedure, second outcomes no
hematoma, painless, wound healing process
faster and short time hospitalization.
