1.Comparison of the Efficacy and Safety of Low Molecular Weight Heparin and Rivaroxabanon in the Preven-tion of Deep Venous Thrombosis after Spinal Surgery

OBJECTIVE:To compare the efficacy and safety of low molecular weight heparin and rivaroxaban in the preven-tion of deep venous thrombosis after spinal surgery. METHODS:Totally 276 patients with high risk factors after spinal surgery were randomly divided into control group and observation group. The patients in the two groups were used elastic stockings to physi-cally prevent deep venous thrombosis from the postoperative first day to continue 3 months;based on it,control group was subcuta-neously injected with 1 low molecular weight heparin from the postoperative first day,once a day. Observation group was orally treated with 1 Rivaroxaban tablet,once every night. The treatment course for 2 groups was 2 weeks. The clinic data was observed, including clinical efficacy,superficial femoral vein diameter(FSV),popliteal vein diameter(POPV),prothrombin time(PT),fi-brinogen degradation products(FIB)and D- dimer(DD)before and after treatment,incidence of deep venous thrombosis and ad-verse reactions. RESULTS:There were no significant differences in the total effective rate and deep venous thrombosis and inci-dence of ADR between 2 groups(P>0.05). After 3 months,compared with before,there were no significant differences in the FSV and POPV between 2 groups (P>0.05);POPV in control group was significantly lower than before and observation group, there was statistical significance(P<0.05). After 1 week,compared with before,there was no significant difference in the PT be-tween 2 groups(P>0.05);the FIB and D-D were significantly higher than before,there was statistical significance(P<0.05), however,there were no significant differences between 2 groups(P>0.05);after 4 weeks,PT in 2 groups was significantly high-er than before,there was statistical significance(P<0.05),however,there were no significant differences between 2 groups(P>0.05);FIB and D-D were significantly higher than before,and observation group was lower than control group,there was statisti-cal significance(P<0.05). CONCLUSIONS:Both efficacy and safety of low molecular weight heparin and rivaroxaban in the pre-vention of deep venous thrombosis after spinal surgery are good. However,rivaroxaban is better than low molecular weight heparin in improving blood coagulation function.
2.Clinicopathological features of 13 cases of small intestine gastrointestinal stromal tumors

Objective To investigate the clinical and pathological features,immune phenotype,diagnosis,differential diagnosis of small intestine gastrointestinal stromaI tumors (GIST). Methods Clinical manifestations,pathological fea-tures and immunohistochemical characteristics were analyzed in 13 cases of small intestine gastrointestinal stromal tu-mors with review of the relative literature. Results Of 13 cases,age ranged from 34 to 76 years(mean 52 years),includ-ing 8 males and 5 females. There were 8 cases in jejunum,3 cases in ileum,2 cases in duodenum. In 13 patients,1 case was low risk,4 cases were middle risk,and 8 cases were high risk The diameter of tumors was 1.6-18 cm. Of 13 cases,there were 6 cases of spindle cell type,3 cases of epithelioid cell type and 4 cases of mixed cell type.Immunohis-tochemically,tumor cells were positive for CD117,DOG-1,Vimentin,but negative for Desmin, S-100. CD34 and SMA were positive in 5 cases,nestin was positive in 10 cases. The positive rate of Ki-67 was from+8%to+30%. Conclusion Small intestinal stromal tumors are rare tumors of alimentary tract, the prognosis was significantly worse in gastric GIST. Surgery is the principal and effective procedure for the tumors.
3.Analysis of monoclonal antibody binding sites in ovine prion protein.

Binding sites of five monoclonal antibodies were obtained by reinforceable method of overlapping recombinant prion protein and synthetic peptide. Overlapping peptides of PrP core were expressed in Escherichia coli by insertion of serial PCR amplicons of ovine PrP gene fragments into pET32a. The expressed fusion peptides were then tested for the binding activity to PrP monoclonal antibodies in Western blotting. The binding sites of 5 monoclonal antibodies of ovine PrP were located respectively as follows: 2H3 in 199 aa-213 aa, 4C6, 5F11 and 7F11 in 139 aa-168 aa and 7F1 in 214 aa-227 aa. There oligo peptides were synthesized and used in ELISA test for more accurate localization of the binding sites. The binding sites of 4C6, 5F11 and 7F11 were further confirmed to be in 149 aa-158 aa. This conclusion may contribute to the research for pathogenesis and diagnostic method of scrapie and bovine transmissible spongiform encephalopathy.

4.Construction and application of competency-based portfolios for medical students

Objective:To retrospectively construct competency-based portfolios for medical students to analyze the factors influencing competency self-assessment, and to explore an approach to efficient portfolio construction.Methods:The participants were randomly selected among medical graduates of 2015 in Shantou University Medical College. Through records collection and an online questionnaire survey, portfolios were built for the participants based on their development data during eight years from college entrance, college education, to post-graduation medical education. The correlation between variables was determined using Kendall's tau-b correlation coefficients. The inter-group differences were analyzed using the Mann-Whitney U test. Results:A total of 126 usable questionnaires for effective developmental portfolios were collected. There were a total of 208 indicators collected, including 79 questionnaire indicators (including 36 for competency self-assessment). The reliability coefficient (Cronbach's α) of the questionnaire was 0.984. The factors related to competency self-assessment were identified. Conclusion:This study provides a basis for the implementation of portfolio assessment, which can promote students' self-assessment and competency development. Optimizing the indicator system, building an online platform, increasing participants' participation motivation, and emphasizing self-reflection and feedback will help improve the efficiency of developmental portfolio construction and its performance.
