3.Preventing and Controlling Bleeding in Gastric Endoscopic Submucosal Dissection.

Although techniques and instruments for endoscopic submucosal dissection (ESD) have improved, bleeding is still the most common complication. Minimizing the occurrence of bleeding is important because blood can interfere with subsequent procedures. Generally, ESD-related bleeding can be divided into intraprocedural and postprocedural bleedings. Postprocedural bleeding can be further classified into early post-ESD bleeding which occurs within 48 hours after ESD and late post-ESD bleeding which occurs later than 48 hours after ESD. A basic principle for avoiding intraprocedural bleeding is to watch for vessels and coagulate them before cutting. Several countertraction devices have been designed to minimize intraprocedural bleeding. Methods for reducing postprocedural bleeding include administration of proton-pump inhibitors or prophylactic coagulation after ESD. Medical adhesive spray such as n-butyl-2-cyanoacrylate is also an option for preventing postprocedural bleeding. Various endoscopic treatment modalities are used for both intraprocedural and postprocedural bleeding. However, hemoclipping is infrequently used during ESD because the clips interfere with subsequent resection. Bleeding that occurs as a result of ESD can usually be managed easily. Nonetheless, more effective ways to prevent bleeding, including reliable ESD techniques, must be developed.

Injection of N-butyl-2-cyanoacrylate has been used successfully for treatment of gastric variceal bleeding. Bacteremia after injection of N-butyl-2-cyanoacrylate is well known, however, the method for diagnosis of infected endovascular injected material has remained uncertain. This is the first case reporting use of F-18 FDG PET/CT in detection of the source of infection after control of endoscopic bleeding with N-butyl-2-cyanoacrylate.

Bronchopleural fistula(BPF) occurs as a postoperative complication in 2 to 5 percent of pulmonary resection. The detection of BPF is generally difficult and various diagnostic methods have been utilized to identify the site of the fistula in order to treat it adequately. Closure of these BPF can be surgical intervention or bronchoscopic application of various sealing agents. We report an experience with use of bronchoscopic instillation of n-butyl-2-cyanoacrylate(Histoacryl ) for closure of a postpneumonectomy BPF.

6.Bonding strengths of 2-octyl cyanoacrylate, N-butyl-2-cyanoacrylate, and nylon 10-0: In experimental corneal perforations in porcine eyes

OBJECTIVE: To determine the bonding strength of 2-octyl cyanoacrylate (Dermabond) compared with N-Butyl-2-cyanoacrylate (Histoacryl) and nylon 10-0 (Alcon,) in sealing experimentally induced corneal perforations in cadaver porcine eyes.
METHODS: This is a single-blind, randomized, physical experimental study involving 78 freshly enucleated porcine eyes in which perforations of 3.0 and 5.1 mm were made in the cornea and randomly sealed with either interrupted nylon 10-0 (n=13), Dermabond (n=13), or Histoacryl (n=13). Intraocular pressures were raised by injecting normal saline into the anterior chamber and postsealing leaking pressures were measured using a precalibrated manometer attached to the anterior chamber maintainer. Fishers Exact Test was used to determine the difference in proportion of eyes that leaked, and Wilcoxon signed ranked test to compare the mean leaking pressures.
RESULTS: In the 3.0 mm group, the proportion of eyes that leaked in Dermabond (2/13, 15.4 percent) and Histoacryl (1/13, 7.7 percent) were comparable (p=1.00). Proportion of leak in nylon 10-0 (13/13, 100 percent) was significantly higher (p0.001). Mean leaking pressures of Dermabond (79.5 mm Hg) and Histoacryl (88.0 mm Hg) were higher compared with that of nylon 10-0 (61.44 mm Hg) (p 0.05). In the 5.1 mm group, proportion of eyes that leaked in Dermabond (4/13, 30.8 percent) and Histoacryl (2/13, 15.4 percent) were comparable (p=0.07 AND P=0.10).
CONCLUSION: The bonding strengths of Dermabond and Histoacryl are comparable and greater than that of nylon 10-0. Both are effective for 3.0 mm and 5.1 mm corneal perforations.

7.Clinical Experience with Combination of Selective Embolization and Surgery for Treatment of Congenital Vascular Malformation(CVM)

As for treatment of congenital vascular malfomation (CVM), intra-arterial embolization provide transient symptomatic relief but recurrence of the lesion occurs in most cases and surgical excision alone has great potential risk such as massive bleeding, functional deformity and explosive recurrence. Combination of selective embolization of vascular pedicle and immediate surgical excison offers the best chance for definite results. We report here two cases of sucessful treatment of CVM with combination of N-butyl-2-cyanoacrylate (NBCA) glue in embolization and surgery.

8.N-utyl--yanoacrylate (Histoacryl) in the Treatment of Esophageal Variceal Bleeding: Comparison with Band Ligation.

BACKGROUND/AIMS: Endoscopic sclerotherapy and band ligation have been well documented beneficial therapeutic options of esophageal variceal bleeding. But acute variceal bleeding is refractory to sclerotherapy in upto one-hird of patients, and rebleeding occurs in 30% to 50%. Recently alternative endoscopic modality, N-utyl--yanoacrylate (Histoacryl) injection is performed in intravariceal sclerotherapy but its efficacy and safty are not clearly established. We evaluated the efficacy of Histoacryl on esophageal variceal bleeding and compared with that of endoscopic band ligation in the present study. MATERIALS/METHODS: From March 1994 to March 1998, ninety seven patients with endoscopically documented esophageal variceal bleeding were enrolled. Histoacryl injection (Histoacryl group, n=33) or endoscopic band ligation (EVL group, n=64) was done for esophageal variceal bleeding. We evaluated the rebleeding rate and in-ospital mortality in both groups. RESULTS: Baseline characteristics were similar but active bleeding on first endoscopic session was significantly higher in Histoacryl group (Histoacryl group; 90.7%, EVL group; 26.6%, p=0.002). Successful hemostasis was done at 87.9% in Histoacryl group, 95.3% in EVL group (not significant). There were no significant differences on early rebleeding rate (18.2% vs 23.4%), late rebleeding rate (39.4% vs 37.5%) and in-ospital mortality (24.2% vs 15.6%) between Histoacryl group and EVL group. There were no technique-elated fatal complications at Histoacryl injection group. CONCLUSION: Therapeutic efficacy of Histoacryl injection was similar to the endoscopic band ligation in patients with esophageal varix bleeding in terms of hemostasis and rebleeding. Histoacryl is effective therapeutic option for esophageal variceal bleeding as well as gastric variceal bleeding.

9.Experimental Embolization Using Histoacryl Blue (N-butyl 2-cyanoacrylate) in Pig Rete Mirabile.

PURPOSE: This study was performed to evaluate any difference in the penetration of the acrylic glue into the pig rete mirabile according to the difference of the concentration of the glue mixture. MATERIALS AND METHODS: Three domestic pigs (12-15kg) were used. After obtaining a normal angiography, two different concentrations of 1:1 and 0.5:1 of Histoacryl Blue mixture with Lipiodol were administrated into pig rete mirabile and then anglographic and pathologic findings were analyzed after taking a postembolization angiography. RESULTS: The degree of the penetration of the glue mixture into the pig rete was different in the anglographic and pathologic findings according to the glue mixture ratio. In case of using 0.5:1 mixture of glue and Lipiodol, glue penetrated into the smaller vessels with size of about 80 micro rn than in case of using 1:1 mixture of glue and Lipiodol. CONCLUSION: Because the polymerization time of glue mixture will be different according to the concentration of glue mixture, a mixture with thin glue concentration seemed to penetrate deeply into the pig rete.

10.Experimental strabismus surgery using N-butyl-2-cyanoacrylate (Histoacryl(R)).

Strabismus surgeries using n-butyl-2-cyanoacrylate and 6-0 vicryl suture material were performed in 16 albino rabbits. Both eyes of 16 rabbits were used. The superior and inferior rectus muscles were recessed and resected 5 mm. In 12 rabbits, n-butyl-2-cyanoacrylate was used and those rabbits were divided into 3 groups, each group (4 rabbits, 8 eyes) was sacrificed 1, 2, and 3 weeks after operation respectively. The eyes were enucleated to examine the tensile strength and pathologic findings of the attached area. In 4 rabbits, 6-0 vicryl suture material was used as a control group in this study. This group was sacrificed and examined 3 weeks following the operation. In the cyanoacrylate groups, there were statistically significant differences and a chronologically increasing tendency of tensile strength between groups. Between the cyanoacrylate and the suture material group at 3 weeks post operation, there was no statistically significant differences in the tensile strength. Pathelogicallyr extraocular muscle bundles revealed patchy areas of interstitial inflammatory infiltration and fibrosis in the cyanoacrylate groups. Capillary telangiectasia and muscle fiber regeneration were commonly observed. The findings in specimens 3 weeks after operation revealed rather mild changes, suggesting healed lesions. In the suture material group, mild inflammatory reactions were found.
