BACKGROUND: It has been explained in many big sample experiments that community comprehensive prevention from the risks of cardiocerebrovascular disease can reduce its morbidity and mortality. But, it is required more samples to verify the evaluations of the process and effect of intervention.OBJECTIVE: To understand the effect of community comprehensive prevention on cardiocerebrovascular disease and hygienic costs demanded and summarizes the suitable patterns of comprehensive prevention on cardiocerebrovascular disease in countryside.DESIGN: Healthy people in community were taken as the objects and community intervention was designed.SETTING: Department of Community Hygienic Service of People's Hospital in Guzheng Town, Zhongshan City, Guangdong Province and Neurological Institute of Guangzhou Medical College.PARTICIPANTS: Totally 32 000 inhabitants were collected from 8 villages in Guzheng Town, Zhongshan City, Guangdong Province from March 1992 to March 2002, aged varied from 20 to 74 years, of which, 14 600inhabitants were male and 17 400 inhabitants were female.METHODS: The intervention of community-based prevention was applied on cardiocerebrovascular disease, including managementof hypertension, smoking control, rational nutrient, nationwide body building and diabetic management.MAIN OUTCOME MEASURES: [1] Awareness, attitude and behavior related to cardiocerebrovascular disease. [2] Management rate and control rate of hypertension. [3] Morbidity and mortality of cerebral apoplexy.RESULTS: [1] By 10-year intervention, awareness rate of hypertension in whole group was 70.29%. [2] Management rate and control rate of hypertension were increased yearly, in which, the management rate was increased from 25.66% in 1997 to 80.50% in 2002, the total control rate was increased from 4.43% in 1997 to 45.80% in 2002, the control rate of blood pressure <160/95 mm HG (1 mm Hg=0.133 kPa) was 58.80%and that <140/90 mm Hg was 32.80%. [3] The systolic and diastolic pressures in intervention community were decreased averagely, in which the D-values are 7.23 mm Hg and 3.92 mm Hg respectively. [4] The awareness, attitude and behavior related to cardiocerebrovascular disease were enhanced remarkably. [5] Morbidity and mortality of cerebral apoplexy were in tendency of decreasing yearly, from 146.90/100 000 and 108.63/100 000 in 1997 to 105.83/100 000 and 69.90/100 000 in 2002 respectively, by 41.07/100 000 and 38.73/100 000 respectively in 6 years and the age of incidence was postponed. [6] It was indicated in economic analysis that the average ratio of profit to cost between 1997 and 2001 was 2.32.CONCLUSION: Community-based comprehensive prevention on cardiocerebrovascular disease improves the awareness, attitude and behavior in community group and reduces morbidity and mortality of cerebral apoplexy.

The priority of the modernization of Chinese medicine at present is the standardization of Chinese medicine practice, including syndrome differentiation, disease diagnosis, standardized therapeutic program, etc. For this reason, the primary work is to establish a concise disease-syndrome differentiation and treatment system (CDSDTS) under the guidance of Chinese medicine theory, holding the core advantages and effectiveness of Chinese medicine, and aiming at creating a simplified theoretical and practical program facilitating for operation and spreading. It is not a mere simplification or careless reduction, but a normalized program of treatment according to the essence of Chinese medicine. Although the therapeutic effect might be somewhat lost, it can be widely approved, thereby, it would make contributions for the inheritance and development of Chinese medicine.

OBJECTIVE To study the audiological and etiological characteristics of infants failed to pass hearing screening. METHODS 126 infants received audiological diagnostic tests, including auditory brainstem response(ABR),40 Hz auditory event related potential(40 Hz AERP),distortion product otoacoustic emissions(DPOAE),tympanometry and acoustic reflex. The degrees and types of the hearing loss, and etiological characteristics were analyzed. RESULTS Among 126 infants (252 ears),61 were diagnosed with sensorineural hearing loss(48.41%),48 were conductive hearing loss(38.09%),and 17 were found to have normal ABR thresholds(13.49%). The hearing loss was associated with various factors, including history of infection during pregnancy(21 cases),threatened abortion(9 cases),pregnancy with age at or over 35(6 cases),extension of pregnancy(7 cases),history of systematic diseases(10 cases),history of neonatal jaundice(13 cases),history of asphyxia and hypoxia(18 cases),premature and low birth weight neonates(8 cases),neonatal diseases (8 cases),family history of deafness(5 cases),craniofacial deformity(3 cases),central nervous system disorder(6 cases),and 9 cases were second child. CONCLUSION The infants who failed to pass hearing screening have various etiology characteristics in hearing loss. The infants associated with risk factors were mostly found to have sensorineural hearing loss.

Acta Physiologica Sinica 2002;54(1):33-37
To investigate the mechanisms of the inhibition of osteoblastic differentiation by dexamethasone (DEX), the effects of different doses of DEX on the activity of alkaline phosphatase (ALP), the synthesis of osteocalcin (OC) and the expression of collagen type I were observed in the cultured rat osteoblasts. The LIM mineralization protein-1 (LMP-1) mRNA, a positive regulator of osteoblasts, was semi-quantified by RT-PCR. The results showed that a lower dose (10(-9) mol/L) of DEX could enhance the activity of ALP, the synthesis of OC and the expression of collagen type I. However, a higher dose (10(-7) mol/L) of DEX inhibited them and down-regulated the expression of LMP-1 mRNA in osteoblasts. It is suggested that DEX stimulates osteoblast differentiation at lower dose, while at higher dose it inhibits osteoblast differentiation. The inhibitory action of DEX on osteoblast differentiation might be mediated by the down-regulation of LMP-1 mRNA.
