1.Expression of Foxp3~+ lymphocytes in breast carcinoma tissues and their clinic significance

Objective To investigate the expression of Foxp3~+ lymphocytes in breast carcinoma tissues and their correlation with other pathological factors,and to investigate the mechanism of action of Treg cells.Methods The expression of Foxp3~+ lymphocytes in the breast cancer tissue and non-cancerous tissue was detected by flow cytometry (FCM) in 30 breast carcinoma patients, and its correlation with other pathological factors was statistically analyzed by multiple linear regression analysis.The expression of TGF-β and IL-10 in the lymphocytes infiltrated in breast cancer tissue and non-cancerous tissue was measured by immunohistochemistry, and their correlation with the expression of Foxp3~+ lymphocytes was statistically analyzed by linear correlation dependability analysis. Results There was significant difference in the expression of Foxp3~+ lymphocytes between the malignant and non-cancerous breast tissues(P<0.05),and it was positively correlated with the clinical stage,blood vessel invasion and the matter of axillary lymph node metastasis(P<0.05). The expression of IL-10 in the tumor infiltrating lymphocytes was positively correlated with the expression of Foxp3~+ lymphocytes(P<0.05).Conclusion The expression level of Foxp3~+ lymphocytes is correlated with invasion and metastasis of breast carcinoma, and the IL-10 secreted by Foxp3~+ lymphocytes may be involved in this effect.Foxp3~+ lymphocytes can be used as an assistant marker for prediction and new therpeutic target of breast cancer.
2.Epidemiology investigation on arsenism from drinking water along Huai'he River and the surrounding area of Hong'ze lake in Huai'an city of Jinngsu province in 2008

Objective To explore the distribution of water with high level arsenic and prevalence of arsenism along Huai'he River and the surrounding area of Hong'ze lake in Huai'an of Jiangsu. Methods Wate rsamples were collected and tested in 2008 from 18 villages of 6 towns according to history data in 3 counties like Xuyi,Jinhu and Hongze. Samples having arsenic level higher than 0.05 mg/L were investigated by epidemiological method and the patients were diagnosed by Standard of Diagnosis for Endemic Arsenism. Results All 5199 water samples were determined,and 260 water samples were exceeding the national drinking water quality level (0.05 mg/L) in 3 counties,the rates of exceeding diagnosis were 5.6%(247/4454),0.7%(4/597),6.0%(9/148) respectively. Total detected rate of endemic arsenic disease was 5.94%(128/2155). The detected rates of age group of 0 ~ ,20 ~,30 ~ ,40 ~ ,50 ~ ,60 ~ ,70 ~ ,80 ~ were 2.86%(1/35),2.11%(2/95),1.26%(3/239),3.10%(16/516),5.53% (32/579),10.07%(41/407),11.84%(27/228),10.71%(6/56) respectively. The detected rate of male (9.10%,78/857) was higher than that of female(3.85%,50/1298,χ~2 = 25.46,P < 0.01). Conclusions Huai'he River and the surrounding areas of Hong'ze lake like Xuyi,Jinhu and Hongze are identified existing endemic arsenic disease area. The prevention of arsenism should be strengthened in these areas.

4.Application of titanium mesh and free forearm flap for reconstruction of maxillary defect resulted from tumor resection.

OBJECTIVETo investigate the application of titanium mesh and free forearm flap for reconstruction of maxillary defect resulted from tumor resection.
METHODSFrom 2004 to 2008, 19 cases with maxillary tumor underwent tumor resection. The defects were reconstructed immediately with titanium mesh for bony defects and free forearm flap for oral mucosa defects.
RESULTS16 cases achieved satisfactory functional and cosmetic results. The speech assessment was good without oronasal reflux. The titanium mesh was exposed and infected in 3 cases. Then the meshes were taken out and the defects were covered with the forearm flap to close the oronasal fistula. The midface was slightly depressed with no functional morbidity.
CONCLUSIONSIt is simple and practical to reconstruct maxillary defect with titanium mesh and free forearm flap. Both the functional and cosmetic results are satisfactory.

6.Reconstruction of oral soft tissue defects with free anterolateral thigh flap.

OBJECTIVETo assess the clinical features and therapeutic efficacy of anterolateral thigh (ALT) flaps for the intraoral defects reconstruction.
METHODSThe clinicopathologic data of 67 cases with oral tumors were obtained from School of Stomatology, Nanjing University Medical Center from Dec. 2008 to Dec. 2010. All the patients underwent the simultaneous tumor resection and intraoral defects reconstruction with free anterolateral thigh flaps. The defects included the tongue, buccal, gingival, mouth floor, and so on. The descending branch of lateral femoral circumflex artery was anastomosed to the external maxillary artery or superior thyroid artery; the vein was anastomosed to the common facial vein or external jugular vein. The flaps were divided into three types: musculocutaneous ALT flap, fasciocutaneous ALT flap and thinned ALT flap.
RESULTSThere were 38 male patients and 29 female. The anterolateral thigh flaps included 35 musculocutaneous flaps, 17 fasciocutaneous flaps and 15 thinned flaps. The success rate was 98.5% (66/67). Partial necrosis happened in one case with diabetes, which healed after debridement and dressing. 1 flap was totally necrosis. Double venous anastomosis was performed in 41 flaps, and one venous anastomosis was performed in 26 flaps. 8 patients required operative exploration in the perioperative period including 6 flaps with thrombotic events (5 flaps were complete survival after the salvages, and 1 flap was failure) , 1 flap with hematoma, and 1 flap with twisting of perforator. The follow-up period ranged from 2 to 24 months( mean, 8.7 months). The result was satisfied. The donor sites were closed directly in all patients, and the wounds healed uneventfully.
CONCLUSIONSThe free anterolateral thigh flap is an ideal soft tissue flap for the intraoral defects reconstruction with good functional result at recipient area and less morbidity at the donor site.

7.Design and application of self-service terminal for hyperbaric oxygen therapy

Objective To design a self-service terminal for hyperbaric oxygen therapy in order to improve the orderliness and efficiency of hyperbaric oxygen therapy.Methods The hardware included mainframe and cabinet,and the software used human-computer interface.Network database was connected with ADO technology,and Delphi 7.0,Pascal compiling code were applied to code compilation.Results The self-service terminal involved the patient into the information chain of hyperbaric oxygen therapy to enhance the working efficiency and therapy orderliness.Conclusion The terminal behaves well in hardware,software,network database and human-computer interaction,and thus is worthy promoting to medium and large hyperbaric oxygen chambers.

Objective To design and develop an information management system based on network database for hyperbaric oxygen therapy to solve the problems in repeated operation,excessive data,copying and etc.Methods Network database was used to establish the information storage unit,design software and hardware architecture as well as develop information treatment units for medical terminal,self service,hand-hold intelligent terminal,appointment App program and etc.Results The system realized rapid printing and recognition of patient information,quick response of server,high accuracy of charging module,and high efficiency of self service terminal and appointment registration.Conchusion The system optimizes hyperbaric oxygen therapy flow,enhances working efficiency of medical staffs and gains high patient satisfaction,and thus is worthy promoting clinically.
9.Correlation between inclination of occlusal plane and masticatory path in skeletal malocclusion

Objective To evaluate the characteristics of and correlation between the inclination of occlusal plane (IOP) and masticatory path in young adults with skeletal malocclusion.Methods Thirtythree individuals were eligible for this study based on the inclusion criteria.Individuals were grouped by ANB angles.There were ten,six and seventeen cases categorized as group Ⅰ ( normal occlusion with skeletal Class Ⅰ ),Group Ⅱ ( skeletal Class Ⅱ ) and Group Ⅲ ( skeletal Class Ⅲ ),respectively.The following parameters of sagittal anatomical landmarks were studied by lateral cephalometric analysis:ANB,MP-FH,OP-CP (IOP),and OP-FH.Mandibular movement of the incisal point during unilateral-sided gum-chewing was recorded by the BioEGN mandibular kinesiography analyzer and both maximal distances and direction (OP-opening/closing angles ) in sagittal plane were analyzed.Correlation between IOP and maximal distances,direction of masticatory path were analyzed,respectively.Statistical analysis was performed with PASW 17.0 software.Results IOP varied among three groups:group Ⅰ ( - 1.78 ± 5.16) °,group Ⅱ( 0.13 ± 2.31 ) °,group Ⅲ ( - 5.28 ± 4.31 ) ° ( P < 0.05 ).Maximal distances of masticatory path in Group Ⅰ,Ⅱ and Ⅲ were as follow:Vertical length:(20.30 ± 8.92),( 17.60 ± 3.29),and ( 18.25 ± 4.09),Anterior-Posterior width:(2.99 ±7.90),(9.60 ±5.80) and (4.67 ±4.64) mm,Lateral width:(8.79 ±2.22),(7.11 ± 2.03 ) and ( 8.47 ± 1.70 ) mm,respectively.The variation among groups was not ofstatistical significance (P > 0.05 ).OP-opening/closing angles of masticatory path in each level were found increasing from group Ⅱ,group Ⅲ to group Ⅰ.There was significant difference between Group Ⅱ and Group Ⅰ in each level of OP-opening angle.OP-opening/closing angles were not correlated with IOP in group Ⅰ and group Ⅱ ( P > 0.05 ).IOP and OP-opening/closing angles were shown to be positively correlated in group Ⅲ with statistical significance (P < 0.05 ).Conclusions The discrepancy of craniofacial structure has an impact on IOP.Skeletal malocclusion and IOP have no significant influence on the maximal distances of masticatory paths.The direction of masticatory path is influenced by IOP in skeletal malocclusion.
10.Study on the relationship between hepatitis C virus infection and sharing injection equipment, sexual behavior among injecting drug users.

OBJECTIVETo study hepatitis C virus (HCV) transmission through different modes of sharing injection equipment and sexual behavior among injecting drug users (IDUs) in Liangshan of Sichuan province.
METHODSA community-based survey was conducted to investigate past and current demographic data, injection equipment sharing patterns and sexual behavior of IDUs. Blood samples were also taken to test for HCV. The survey was conducted between Nov 8 and Nov 29, 2002. 379 subjects were screened through outreach recruitment and peer informing. SPSS (11.5) was used for data analysis.
RESULTSHCV prevalence was 71.0% (269/379). Needles or syringes sharing in the past three months and past syphilis infection were strongly associated with HCV transmission after univariate analysis using chi-square test. Trend analysis indicated that HCV infection rate increased along with the increase of needles or syringes sharing, sharing of rinse water and the number of peers sharing the equipments. Data from multivariate logistic regression showed that sharing of needles or syringes and history of syphilis infection were significantly associated with HCV transmission. No significant difference was found between HCV infection and sexual behavior after univariate analysis using chi-square test.
CONCLUSIONFurther sero-epidemiological prospective cohort studies should be conducted to clarify the relationship between different modes of sharing injection equipment, sexual behavior and HCV infection.
