1.Distribution of A.baumannii infections and drug resistance in Ningbo

Objective To investigate the distribution and antibiotic resistance of A.baumannii during the last five years in our hospital,the basis for the reasonable clinical use of antibiotic was provided to doctor.Methods The strains of A.baumannii isolated from clinical specimens during 2009-2013 were analyzed by VITEK-32 system,the antibiotic resistance was analyzed by WHONET5.4 software.Results A.baumannii strains were mainly isolated from sputum,accounting for 77.3%;the majority of the strains were isolated from ICU,accounting for 42.3%;the resistance rates of A.baumannii to antibiotics appeared increasing,and over 70% except cefoperazone-sulbactam and imipenem.Conclusion A.baumannii is the major pathogen,and detection rate of A.baumannii is very high,antibiotic resistance status of A.baumannii is very serious.The management of antibiotic application should be strengthened,and the occurrence and prevalence of antibiotic resistant bacteria should be strictly controlled to prevent outbreak and epidemic of nosocomial infection.
2.Design, synthesis and anti-platelet aggregation activities of ligustrazine-tetrahydroisoquinoline derivatives.

Abstract: Fifteen novel ligustrazine-tetrahydroisoquinoline derivatives were designed and synthesized according to the association principle of pharmaceutical chemistry. The structures were identified by IR, NMR and ESI-MS. The inhibitory activities of platelet aggregation induced by ADP and AA have been measured by Bron method. Preliminary pharmacological results showed that compounds 7g, 7h and 7n had potent inhibitory activity against platelet aggregation induced by AA, and the compound 7o showed significant inhibitory activity against platelet aggregation induced by ADP.

3.A totally laparoscopic peritoneal free flap for reconstruction of hand.

Management of defects on the hand and foot with exposed tendons remains a major challenge for plastic surgeons. Here, we present a case of hand reconstruction with a totally laparoscopic peritoneal flap. The anterior rectus sheath was preserved in situ. The peritoneal free flap supplied by peritoneal branches of the deep inferior epigastric artery was retrieved by laparoscopy to cover the soft tissue defect of the hand. The defect of the dorsal hand was 17 cm ×12 cm. The peritoneal flap measuring 22 cm × 15 cm survived completely without any complications. A following split-thickness skin graft offered the suc- cessful wound closure. Motor and sensory function improved gradually within the first year follow-up. The totally laparoscopic peritoneal free flap is a good choice for reconstruction of the soft tissue de- fects accompanied by exposed tendons on the hand and foot.

6.Long-term outcomes of endoscopic submucosal dissection for gastric stromal tumors

Objective To explore the long-term outcomes of endoscopic submucosal dissection (ESD) of gastric GISTs.Methods Data of 25 patients with gastric stromal tumors,who underwent ESD,were reviewed in terms of clinical characteristics,histopathologic results,complications and long-term outcomes.Results Twenty-five patients underwent one-time complete removal of lesion, with a average tumor size of 3.0 cm (range,0.6 to 7.6 cm).The average time of operation was 65 minutes (range,15 to 154 minutes).one case was observed with intraoperative hemor-rhage of 200 mL blood,which was successfully managed by hemostatic forceps.Perforation with incidence rate of 16%occurred in 4 cases and was closed well with clips.Mucosal mechanical laceration of esophagus occurred in 1 case when the big tumor was taken out.The average length of hospi-talization was 7.3 days (range,4 to 21 days).A follow-up for 30 months (range,13 to 54 months) showed no tumor recurrence or metastasis. Conclusion ESD is an effective and safe endoscopic procedure to remove gastric GISTs for long-term outcomes in patients with no metastasis.
7.Phylogenetic analysis of enterovirus 71 isolated from patients with hand, foot and mouth disease in Guangdong and Fujian provinces, 2000-2001.

OBJECTIVETo identify enterovirus 71 (EV71) strains isolated from patients with hand, foot and mouth disease (HFMD) in Guangdong and Fujian provinces from 2000 to 2001 by using phylogenetic analysis.
METHODSAll 25 samples were first tested for enteric viruses by RT-PCR using enterovirus specific primers EV-1 and EV-2, and then were identified for EV71 by RT-PCR using EV71 specific primers 159S and 162A. The amplicons of 485bp segment (part of the VP1 gene) were cloned into pGEM-T and sequenced. A phylogenetic tree was constructed by comparison of the sequences with other 12 EV71 strains isolated from China, Japan, Hungary, and the United States including the prototype BrCr.
RESULTSThe positive rate of EV71 was about 20%. The sequence analysis showed that the new isolate (GZH2000) shared 94%-96% nucleotide identity with three strains isolated in 1998 and 2000, and 91% with a strain isolated in 1987 from Chinese mainland, but shared only 82%-84% homology with EV71 isolates studied abroad.
CONCLUSIONSEV71 is one of the important pathogens of HFMD in south China. The strains isolated from mainland were closely related with most isolates from Taiwan, but different from most EV71 strains reported abroad. The symptoms of EV71 infection in mainland were not as intensive as those described in Taiwan's outbreak.

8.Effect of supplemented danggui shaoyao powder and its disassembled prescriptions on lymphocytes proliferation in aged rats.

OBJECTIVETo study the effect of supplemented Danggui Shaoyao San (DSS) and its disassembled prescriptions on function of lymphocyte in aged rats.
METHODSRats aged 18 months were administered with decoction of DSS and its disassembled prescriptions separately in groups for 3 weeks to prepare drug containing serum. Samples of ConA induced rats splenic lymphocytes proliferation was treated respectively with prepared drug serum, for testing their effect on lymphocyte proliferation using methyl thiazolyl tetrazolium (MTT) method, and isoprel (ISO) and propranolol were taken as the controls.
RESULTSThe A value of lymphocyte proliferation in the drug serum treated groups was significantly different from that in the control groups (P < 0.01), which could be reduced markedly after treatment with ISO, but could restore to the level before ISO treatment by adding drug serum or propranolol.
CONCLUSIONDSS and/or its disassembled prescriptions could raise the lymphocyte proliferation in aged rats significantly, it also shows antagonizing effect against the inhibition of ISO on lymphocyte proliferation.

9.Expression of p120ctn and its significance in non-small cell lung cancer.

OBJECTIVETo investigate the expression of p120(ctn) in non-small cell lung cancer (NSCLC) and its correlation with the clinical and pathologic parameters.
METHODSImmunohistochemistry (S-P method) was used to detect the expression of p120(ctn) in 143 NSCLC cases. The variation of protein expression was further analyzed in 36 cases by Western blot. The correlation with clinical and pathologic parameters was studied.
RESULTSImmunohistochemically, normal bronchial cells showed membranous expression for p120(ctn), while NSCLC was characterized by cytoplasmic or diminished membranous staining. The rate of abnormal p120(ctn) expression was 79.7% (114/143). There was a significant correlation between abnormal expression of p120(ctn) and tumor differentiation, clinical stage, lymph node metastasis and poor prognosis (< 0.05), but not histologic typing. Western blot showed that the total amount of p120(ctn) in normal bronchial cells was significantly higher than that in NSCLC. The p120(ctn) isoform 1 (120,000) and isoform 3 (100,000) were expressed in normal lung tissue, while there was a reduced expression or absence of isoform 1 in NSCLC.
CONCLUSIONThe expression of p120(ctn) is abnormal in NSCLC; p120(ctn) may serve as a useful prognostic marker for NSCLC.

10. Clinical outcomes of modified reattachment of superior peroneal retinaculum for recurrent peroneal tendon dislocation

Objective:
To investigate clinical outcomes of modified reattachment of superior peroneal retinaculum (SPR) for patients with recurrent peroneal tendon dislocation.
Methods:
A total of 24 cases with recurrent peroneal tendon dislocation from December 2012 to June 2017 were treated with modified reattachment of SPR. There were 20 males and 4 females. The average age was 24.9±9.3 years (14-48 years). The average BMI was 23.18±3.50 kg/m2 (15.8-32.2 kg/m2). A 4-5 cm incision was made along the lateral margin of the fibula and curved distally around the fibular tip in line with the peroneal excursion. The superior peroneal retinaculum, peroneus longus and peroneus brevis were exposed. The periosteum and SPR were stripped from the fibula. The false pouch was formed. Two suture anchors were inserted into the postero-lateral ridge of the lateral malleolus without damaging the cartilaginous ridge, after which the SPR was reattached to the lateral malleolus with the anchored suture. The inner layer of the false pouch was incised, while the outer layer (periosteum) was sutured with the SPR in a pants-over-vest style. The following items, including American Orthopaedic Foot and Ankle Society (AOFAS) score, visual analogue scale (VAS), rate of return to previous sports level, time to return to sports activity, complication, and patients satisfaction were evaluated preoperatively and at the final follow-up.
Results:
The average operation duration was 36.1±8.8 min (20-51 min). The blood loss was 1-10 ml, average 4.1±2.7 ml. The follow-up was carried out in 22 cases for mean 33.9±15.7 months (13-61 months). AOFAS score was improved from 77.8±7.8 points to 95.5±4.3 points significantly (
