4.Inward malfomation of elbow and its surgical treatment in children

Study was performed on 48 subjects with inward deformity of elbow after a supracordylar fraction on the arm, form January 1999 to June 2002 at the Department of Surgery, National Institute of Pediatry. Results showed that this deformity was occurred commonly in children of 6-11 years old of age, especially in boys. Humeral ostectomy was indicated in the case of deformity of cubitus varus of > 15o and > 6 months after the trauma. The opening of the lower end of brachial bone and the campbell classical technique of excison of the muscle did not mineralize did not limit the motions of the joints of elbow. Kirschner screw was fixed temporarily and removed 12-14 days after the operation

6.Treatment of refractory lateral epicondylitis with modified Nirschl surgical technique.

OBJECTIVETo investigate the clinical effects of modified Nirschl surgical techniique in treating refractory lateral epicondylitis.
METHODSFrom March 2009 to January 2011,21 patients (21 elbows) with refractory lateral epicondylitis were treated in our hospital. There were 8 males and 13 females,ranged in age from 25 to 59 years with an average of (48.3+/-13.4) years and the duration time from 8 to 33 months with an average of (17.1+/-7.7) months;affected position in dominant sides of 16 cases and non-dominant sides of 5 cases. The patients had already received multiple non-operative treatments. Modified Nirschl surgical technique was performed,and operative origination from origin of musculus extensor carpi radialis brevis to discard process with small incision, the process place of extensor digitorum communis would be removed. The incisions were nursed by ice compress for 2 days after operation. Range of motion (ROM) and strengthening exercise of elbow joints started at the 1 week after plaster slab fixation; ROM and strengthening exercise of wrist joints also started at the 2 week after fixation. The pain, power of gripping and patient staisfaction were recorded after operation. Verhaar scaling were used to estimate the recovery.
RESULTSSeventeen patients were followed up from 13 to 22 months with an average of 16.3 months. According to Verhaar standard, 15 cases obtained excellent results and 2 good. No postoperative complication such as instability was found.
CONCLUSIONModified Nirschl surgical technique is an effective method in treating refractory lateral epicondylitis but correct to diagnosis and exclusion the coexisting diseases,accurate removal the process are important guarantee.

7.Clinical study of movable external fixation for elbow tuberculosis.

OBJECTIVE:
To compare clinical effect of movable external fixation and fusion fixation for the treatment of elbow tuberculosis.
METHODS:
From October 2013 to June 2019, 52 patients with elbow tuberculosis treated with standard antituberculosis therapy were divided into movable external fixation group and fusion fixation group according to treatment methods. In group A, there were 25 patients, including 11 males and 14 females, aged from 24 to 75 years old with an average of (42.81± 9.01) years old; the courses of diseases ranged from 2 to 9 months with an average of (3.96±1.45) months. In group B, there were 27 patients, including 15 males and 12 females, aged from 23 to 77 years old with an averageof (44.08±7.44) years old; the courses of diseases ranged from 2 to 7 months with an average of (3.88±1.67) months. All patients were performed focus debridement. Intraoperative blood loss, operative time were compared between two groups. VAS score before operation, 2 weeks and 12 months after operation were applied to evaluate pain relieve;Mayo elbow performance score (MEPS) before operation, 1 and 12 months after operation were used to evaluate clinical effect;changes of erythrocyte sedimentation rate (ESR) and Creactive protein, CRP) before operation, 3 weeks after antituberculosis therapy, 1 week and 6 months after operation were compared between two groups.
RESULTS:
All patients were followed up from 12 to 20 months with an average of (13.50±4.85) months. No mixed infection and recurrence of tuberculosis occurred. There were no statistical differences in intraoperative blood loss and operative time(
CONCLUSION
For elbow tuberculosis, movable external fixation and fusion fixation have equal effect in operative time, amount of bleeding and control of tuberculosis infection indicator. Movable external fixation need earlier functional exercise, not conducive to pain relief at early stage, which may be better than fusion fixation, it is worth clinical promoting.

9.Surgical exposures of the distal humeral fractures: An anatomical study of the anterior, posterior, medial and lateral approaches.

PURPOSE:
Exposure of the articular surface is the key to the successful treatment of intra-articular fractures of distal humerus. Anterior, posterior olecranon osteotomy as well as medial and lateral approaches are the four main approaches to the elbow. The aim of this study was to compare the exposure of distal articular surfaces of these surgical approaches.
METHODS:
Twelve cadavers were used in this study. Each approach was performed on six elbows according to previously published procedures. After completion of each approach, the exposed articular surfaces were marked by inserting 0.5 mm K-wires along the margins. The elbow was then disarticulated and the exposed articular surfaces were painted. The distal humeral articular surfaces were then closely wrapped using a piece of fibre-glass screen net with meshes. The exposed articular surfaces and the total articular surfaces were calculated by counting the number of meshes, respectively.
RESULTS:
The average percentages of the exposed articular surfaces for the anterior, posterior olecranon osteotomy, medial and lateral approaches were 45.7% ± 2.0%, 53.9% ± 7.1%, 20.6% ± 4.9% and 28.5% ± 6.3%, respectively.
CONCLUSION
The anterior and posterior approaches provide greater exposures of distal humeral articular surface than the medial and lateral ones in the treatment of distal humeral fractures.

10.Development and innovation of arthroscopic techniques.

In this special issue, several papers related to clinical application of arthroscopic surgery are published, which present the current situation of arthroscopic techniques in our country. As a technique of minimally invasive orthopedics, arthroscopy is used in examination, diagnosis and treatment of joint diseases, which is the trend of modern surgery. Arthroscopy has been widely used in surgical procedures in knee diseases, and now it expands the scope of application to other joints, such as shoulder, elbow, wrist, hip, ankle and even smaller joints. The application scope of arthroscopy has spread from arthroscopic debridement to reparative and reconstructive surgery. Some innovation has been made in extra-articular surgeries. Researchers conduct in depth study and exploration in the basic and clinical application of arthroscopy. Arthroscopic surgery has become an indispensable branch of modern orthopedics.
