1.A Case of Generalized Syringoma.

A case of syringoma with unusual distribution in a young healthy woman is reported. Physical examination revealed numerous angular or lichenoid papules, varing from skin color to light brown in hue, on the anterior aspect of the neck, upper chest, breast, sternal area, inframammary area and abdomen with grouping tendency. Some papules had fine scales on their surface. The histologic findings were those of charateristic features of syringoma.

2.Histologic Studies on Korean Fetal Skin - The morphological development of the epidermis, hair, sebaceous gland, eccrine sweat gland, and basement membrane.

These studies were undertaken to evaluate the morphological development of the epidermis, hair, sebaceous gland, eccrine sweat gland and basement membrane in the skin of the Korean fetuses. Nineteen fetuses, 4 to 28 weeks of age, ranging in size from 4 to 230mm crown-rump length, obtained from healthy Korean mothers, were used in these studies and histological observation was done in the different fetal weeks and in the various parts of .the body such as face, perioral, scalp, axilla, abdomen, palm and sole. The results are as follows: 1) At 4-5 weeks of fetal age, the epidermis consists of a single layer of undifferentiated cells and by the 6th week it becomes two-layered; the periderm and the stratum germinativum. At 12-13 weeks, the germinative layer proliferates in a centrifugal direction to form a stratum intermedium and also proliferates in a centripetal direction, giving rise to clusters of cells that grow into the subjacent dermis, to form prhair germs and eccrine sweat gIand germs. Keratohyaline granules first make an appearance at 20th week and the stratum granulosum appears at 28 weeks of fetal life. From 20th week onward the periderm gradually disappears and is replaced by a stratum corneum at 28 weeks.

3.Five Cases of Erythema Dyschromicum Perstans.

Five cases of erythema dyschromicum perstans which seem to be the first cases in Korea are described. They are healthy subjects with moderately pigmented skin. Numerous gray-blue macules are seen on the covered part of the body in two patients, but dark brownish macules in other three patients. Histologic findings reveal the characteristic features of hydropic degeneration of the basal cell layer of the epidermis, perivascular infiltrates of lymphocytes and histiocytes and melanin-laden macrophages in the corium in four patients, however, merely pigmentary incontinence into the corium in one patient. We think that there are some problems in cases which show the chracteristic features of erythema dyschromicum perstans histologically but do not have peculiar clinical features, e.g. color of the lesion, especially in Oriental. In this respect, though there may be down for much discussion in three cases which have not typical color of the lesion, we report these cases with typical two cases under the name of erythema dyschromium perstans in order to accumulate more knowledge of these problems

4.A Case of Norwegian Scabies in a Patient with Cushing Syndrome.

We describe here a case of Norwegian scabies in a patient with Cushing syndrorne, the first case in Korea. He had complained of intermittent mild itching for 5 month. Physical examinations revealed the characteristic features of Cushing syndrome and hyperkeratotic brownish papules or/and patches were distributed on the dorsa of hands, axillae, groins, interdigital webs, upper part of the chest and around the neck. We could find numerous burrows and many mites, eggs, lavas and scybala in the affected sites on direct microscopic examination with 25% KOH. Significant laboratory findings were as follows: 1. Total eosinophil count: 0 2. 17-hydroxycorticoids in 24 hr. urine: 10. 1 mg/900 cc 3. Bone X ray series: generalized osteoporosis 4. Mantoux test: negative About 10 days after topical application of 25% benzyl-benzoate lotion combined with 5% sulfur ointment, the skin lesions were cleared. We suspect that the lack of delayed hypersensitivity might attribute to the development of Norwegian scabies in this patient.

5.A Case of Eosinophilic Pustular Folliculitis (Ofuji's Disease) in a Newborn.

We describe a case of eosinophilic pustular folliculitis in a 1-month-old Korean male infant. The patient was suffering from erythematous papules and pustules affecting the scalp, trunk, and extremities, detected at birth. The flare was accompanied by leukocytosis and eosinophila. Histopathology revealed folliculitis with a predominant eosinophilic infiltrate. Lesions showed partial response to topical steroid and disappeared in four weeks. Three months after the resolution of the lesions, he presented a similar clinical picture as the previous episode.

6.Three Cases of Livedoid Vasculitis Improved by Low-dose Danazol.

Livedoid vasculitis is characterized clinically by smooth or depressed ivory-white scars surrounded by hyperpigmentation and telangiectasia with or without preceding purpuric in-filtrated papules and plaques and histologically by intravascular deposition of fibrin. Its pathophysiology is still obscure. Many treatment modalities have been used, but results are not satisfactory. Recently, there have been experiences of successful treatments with danazol, a synthetic androgen. We used danazol in three patients who presented with livedoid vasculitis and were relatively resistant to treatments with aspirin and/or pentoxifylline. After 4 to 8 weeks, there was remarkable clinical improvement in all three patients.

7.A Case of Pruritic urticarial Papules and Plaques of Pregnancy.

We report. herein a case of pruritic urticatial papules and plaques of pregnancy in a Z8-year-old primigravida. She had the urticarial papules and plaques on the abdomen and thighs, which developed at 30th week of pregnancy. She was treated with topical fluorinated steroid and chlorpheniramine, 6mg/ day. The itching sensation was relieved within 24 hours after the therapy and the skin lesions resolved after delivery.

8.Therapeutic Effect of Potassium Iodide in Erythema Nodosum.

We evaluated the therapeutic effect of potassium iodide in fourteen patients with erythema nodosum. The results were as follows: 1) All patients were given a saturated solution of potassium iodide, 600 to 900 mg orall y a day for 3 days to 5 weeks. 2) Three patients were observed initial days, two patients within 9 days. 3) There were no correlations between therapeutic effect of potassium iodide. 4) Specific side effects were not found during the treatment with potassium iodide.

9.A Histopathologic Study of Early Psoriatic Lesions.

In 20 patients with psoriasis, 27 early psoriatic lesions of 1-2mm size (16 papules with scale and 11 maculopapules without scale) and 15 clinically uninvolved skin at distance of 2-4em from the nearest psoriatic lesion were histologically examined in an attempt to obtain information on the mechanism which initiates the development of the skin changes. 10.A Case of Recurrent Melanocytic Nevus.

Recurrent melanocytic nevus forms after incomplete removal of a melanocytic nevus and is chatacterized by somewhat proliferation of atypical melanocytes at the dermoepidermal junction and epidermis or recurrent junctional activity. A 73-yeat-old man had a pigmented lesion on the volar aspect of the right heel, which was excised 20 years ago. Five years thereafter, the pigmented lesion recurred in the previous operation scar and had been excised again. But pigmented lesion reappeared in the scar as an irregular patch of variegated pigmentation of browm, gray and black. Histopathologic findings showed fibrosis in the dermis and proliferation of normal and atypical rnelanocytes in the overlying epiderrnis.
