For valgus impacted four part fracture of the proximal humerus, surgical stabilization and early mobilization of the joint can produce the best clinical outcomes. 4.Management of Displaced Intra-articular Calcaneal Fracture.

Recently, open reduction and internal fixation has been the treatment of choice for displaced intra-articular calcaneal fractures for many orthopaedic surgeons. However controversy still surrounds the optimal treatment with regard to whether displaced intra-articular calcaneal fractures should be treated operatively or conservatively. Conservative treatments include use of splint, rest, leg elevation, icing, use of analgesics and early mobilization. Operative treatment is open reduction and internal fixation, performed through an extensile lateral approach with interfragmentary screws and application of a neutralization plate. We reviewed the question of whether operative treatment by open reduction and internal fixation provides a benefit compared with conservative treatment for displaced intra-articular calcaneal fractures.

5.Nurturing the seeds of evidence-based practice: Early ambulation among cardiac surgery patients

Background:
Cardiovascular disorders continue to be the most prevalent cause of
morbidity and mortality in the Philippines and worldwide. Surgical treatments used to
manage cardiovascular disorders (unfortunately) have multiple complications. As part of
the health care team, nurses need to develop interventions that are safe, scientifically
grounded, and cost-effective in order to counteract these complications. One of the nursing
interventions that can be implemented is early ambulation.
Aim:
To search, appraise, and synthesize the best evidence surrounding early ambulation
among cardiac surgery patients.
Methods:
This study employed an evidence-based review method suggested by Melnyk
and Fineout-Overholt (2005). Systematic literature search was done to the following
databases: Cochrane, Cumulative Index of Nursing and Allied Health Literature (CINAHL),
Joana Briggs Institute (JBI), MEDLINE, National Guideline Clearinghouse (NGC), and
Nursing/Academic edition. Post-operative patients who have undergone coronary artery
bypass surgery (CABG), percutaneous coronary intervention (PCI), and transfemoral
cardiac catheterization (TCC) are included in this study. The final review also included six
articles.
Results and Discussion
Findings show that early ambulation among patients who
underwent coronary artery bypass surgery (CABG), percutaneous coronary intervention
(PCI), and transfemoral cardiac catheterization (TCC) improves patient care outcomes
(i.e., reduce complications such back pain, puncture bleeding, and urinary discomfort,
improves general well-being, and decrease health care costs). Parameters for early
ambulation (ranges from 3 hours to 24 hours) and late ambulation (ranges from 12 hours to
48 hours) are used in the study reviewed. The non-randomized comparative study found
that the complication rate in the early ambulation group is not increased compared to the
late ambulation group (test for non-inferiority p= 0.002). Randomized controlled trial found
out that early ambulation among cardiac surgery patients could reduce back pain
(OR=0.19, 95% CI: 0.08-0.45, p<0.001), decrease urinary discomfort (OR=0.35, 95%
CI:0.14-0.90, p=0.03) for very or unbearable urinary discomfort, and general-well being
(p=0.0005 for vitality scale and p=0.014 for the total general well-being). Furthermore,
early ambulation group reported decrease in hospitalization costs (less charge of $105 or
Php 5,040).

6.Ipsilateral Femoral Segmental and Tibial Fractures : A Case Report.

For these fractures, surgical stabilization and early mobilization of joint produce can be the best clinical outcomes. 7.Anterior Interbody Fusion Using Kaneda Device in Unstable Thoracolumbar Spinal Lesions.

Recently anterior spinal device in the treatment of the unstable thoraco-lumbar spine has been used. We have experienced 11 patients of the unstable thoraco-lumbar spine, which were composed of 8 patients with burst fracture and 3 patients with tuberculous spondylitis. The 11 patients with neurological deficit were treated with an one stage anterior operation consisting of anterior decompression through vertebrectomy, realignment with Kanedae device. No patient showed neurologic deterioration after surgery. The anterior spinal instrumentation with Kaneda device afford enough stability to enable early ambulation with good alignment and solid fusion.

8.Studies on the Intrasinusal Pyelolithotomy: II. Clinical observation on intrasinusal pyelolithotomy.

Following results were obtained through comparative investigation between lumbar and posterior vertical incision in the intrasinusal pyelolithotomy on the 28 cases of renal stones. 1. There was no difference in operation time between lumbar and posterior vertical incisions for renal stones. 2. Blood loss during operation by posterior vertical incision, as compared with lumbar incision, was reduced to a half. 3. While narcotic analgesics were used postoperatively in whole cases of lumbar incision, mild narcotic analgesics were used in a few cases of posterior vertical incision. But there was no necessity of the use of narcotic analgesics in whole cases of posterior vertical incision. 4. There was no difference in the degree of postoperative urine leakage between lumbar and posterior vertical incisions for renal stones. 5.In the posterior vertical incision for renal stones, as compared with lumbar incision, postoperative ambulation day was reduced to a half and postoperative hospitalization period was also shortened to one third. Considering above mentioned results in the posterior vertical incision operative in vasion was minimal by approach into retropritoneal space without cutting muscle layers, in addition early ambulation and discharge were possible. Especially by performing intrasinusal pyelolithotomy in the state of pyelolithotomy in situ, reoperation was possible in the case of recurrence of stones. I think posterior vertical incision seems to be a reasonable method for surgical management of renal stones.

9.Breakage of Cephalomedullary Nail Used in the Treatment of Proximal Femur Fractures: Case Report.

Internal fixation using a cephalomedullary nail as treatment for proximal femur fracture has recently been popular for early ambulation and rehabilitation. However metal breakage at the lag screw insertion site was reported due to non-union, delayed-union, and early weight bearing. In our orthopedic department, we experienced 2 cases of nail breakage at the lag screw insertion site, therefore we report on evaluation of the cause of metal failure and prevention of complications with literature review.

10.Pediculo-Iliac Screw Fixation in Vertically Unstable Posterior Pelvic Ring Injury.

Herein, we report 5 cases of vertically unstable posterior pelvic ring injuries treated by pediculoiliac fixation. We enrolled 5 patients (male 3, female 2) with vertically unstable posterior pelvic ring injuries treated by pediculo-iliac fixation. Prior to and following the surgery, radiologic results were compared using Matta and Saucedo's method and the clinical results before and after surgery were compared using the Postel score. The outcomes of radiological evaluation were anatomic reduction in 3 cases and nearly anatomic in 1. The mean postel score at last follow up was very good in 4 cases and poor in 1 case. Lumbosacral pediculo-iliac screw fixation enables early ambulation and it is considered a useful method.
