1.The Characteristics of Suicide Attempts and Psychosocial Risk Factors in Correctional Institutions.

OBJECTIVES: Suicide is one of the most common causes of deaths in correctional institutions. This study aimed to examine the characteristics of suicide attempts and its related psycho-social factors in correctional institutions. METHODS: This study examined the characteristics of 101 suicide attempts from 2006 to 2007 in the two regional correctional headquarters. Thirty-seven male inmates (43 suicide attempts) and 40 matched controls were included in interviews and reviews of personal records. Psychiatric illnesses were examined using the Structured Clinical Interview for DSM-IV and medical outcomes of their suicide attempts with the Lethality Scale of Diagnostic Interview for Genetic Studies. RESULTS: Over a half of suicide attempts occurred in solitary cells and the most common method was hanging. Medically, 70% of the suicide attempts were more than severe. Poor social support, a lifetime history of suicide attempts, and incarceration were associated with suicide attempts. Additionally, psychiatric illnesses were more likely to increase the risk of suicide. CONCLUSION: This study implies that mental health issues and monitoring systems are important to reduce suicides in the correctional system. Regular checks and management of suicide risk and mental illness are crucial to prevent suicide in correctional institutions.

2.Epidemiologic Trends of Rotavirus Infection in the Republic of Korea, July 1999 through June 2002.

BACKGROUND: Rotavirus is the most common cause of childhood diarrhea worldwide. Although rotavirus is also the leading cause of infant and childhood diarrhea in Korea, much remains unknown about the trends of rotavirus infection by month and geographic region in Korea. To monitor epidemiologic trends of rotavirus infection, a laboratory-based rotavirus surveillance network was established in 2002. This is the first nationwide, multicenter evaluation of rotavirus epidemiology in Korea. METHODS: The rotavirus test results were collected retrospectively from eight network laboratories, from July 1999 to June 2002. Four laboratories used latex agglutination, three used immunochromatography, and one used enzyme-linked fluorescent assay for the detection of rotavirus antigen. RESULTS: Of 10, 441 stool specimens, 2, 496 (23.9%) were positive for rotavirus. During the 3-year period, the rotavirus season began in December-January, and ended in April-May. The rotaviruspositive percentage of summer, autumn, winter, and spring was 11.5%, 10.0%, 32.8%, and 30.0%, respectively. A few hospitals revealed summer epidemics. The rotavirus positive rate in each hospital varied from 15.3% to 44.2%. A common feature of the three hospitals showing the lowest rotavirus-positive percentage (i.e. <20%) was their large size (>800 beds). The secondary care hospitals showed a higher positive proportion (27.5%) compared with tertiary care hospitals (21.1%). CONCLUSIONS: Overall, the rotavirus-positive percentage among all diarrheal specimens was similar to that of other developed countries. The results of this study showed that the autumn epidemic of the rotavirus has declined or disappeared and the peak season for rotavirus has shifted to late winter/early spring in Korea.

3.Transplacental Fetal Therapy for a Case of Fetal Supraventricular Tachycardia which Caused Fetal Hydrops.

A case of fetal supraventricular tachycardia which caused fetal hydrops was diagnosed at 29 weeks of gestation by fetal echocardiography. Transplacental fetal therapy with ma-ternal intravenous digoxin administration resulted in restoration of normal fetal sinus rhythm and disappearance of fetal hydrops on 7th day after initiation of treatment when the mat-ernal serum digoxin level was 2.11 ng/mL. The fetus showed normal sinus rhythm when evaluated by fetal echocardiography during the remainder of pregnancy with maternal oral digoxin maintenence. At birth, the infant did not show any cardiac arrhythmia and hydropic appearance.

4.Clinical Characteristics of ESRD Patients with Severe Hyperparathyroidism Who Undertook Surgical Parathyroidectomy.

BACKGROUND: Medical treatments such as restriction of phosphate, phosphate binder use, and active vitamine D therapy have been widely used for hyperparathyroidism in ESRD patients, and surgical parathyroidectomy should be considered in patients with uncontrolled hyperparathyroidism. METHODS: A retrospective study was performed in 24 ESRD patients with severe and uncontrolled hyperparathyroidism despite of medical treatment who undertook surgical parathyroidectomy in Severance hospital from 1990 to 1999. RESULTS: Sixteen patients had total parathyroidectomy with immediate autotransplant, 7 patients had subtotal parathyroidectomy and only 1 patient had minimally invasive parathyroidectomy. An excellent short-term control of hyperparathyroidism was achieved in all patients after parathyroidectomy. Preoperative bone and joint pain improved in 16 of 19 patients. Muscle weakness and pain improved in 11 of 17 patiens, malaise improved in 8 of 10 patients and pruritus improved in 10 of 13 patients. In addition, clinical laboratory finding improved after parathyroidectomy. No clinical differences were seen between 16 patients who undertook total parathyroidectomy with immediate autotransplant and 7 patients who undertook subtotal parathyroidectomy. Recurrence of hyperparathyroidism ocurred in 5 of 24 patients with 4 nodular hyperplasia and 1 diffuse hyperplasia in pathologic finding. The less degree of attenuated response of intact PTH levels immediately after operation was observed in 5 recurrent cases. CONCLUSION: Good results were obtained after parathyroidectomy. We believe that histologic subtype and the attenuated response of intact PTH after surgical parathyroidectomy could be possible predictors of the recurrent hyperparathyroidism.
