1.Clinical Trial on the Hypotensive Effect of Arotinolol(S-596) in Essential Hypertension.

Arotinolol was administered orally to 31 patients to evaluate is antihypertensive action. After one week of antihypertensive medication, they were given Arotinolol 30mg daily for 4 week. Routine serum chemistries, electrolytes, urinalysis and electrocardiography were studied before and after Arotinolol. Blood pressure measured in sitting, supine and standing position was 175.3+/-5.4/115.7+/-4.0, 168.0+/-5.2/150.0+/-3.8, 154.4+/-8.5/103.4+/-3.2mmHg in control and 143.8+/-2.8/93.3+/-2.8, 144.6+/-5.4/88.1+/-4.3, 139.7+/-4.6/86.8+/-3.4mmHg in Arotinolol treatment group. The differences between both blood pressure were statistically significant(P<0.01). No laboratory test showed significant difference between both period. There were no significant side effects except 2 cases of transient headache and dizziness respectively, and one case of diarrhea. In summary, Arotinolol apears to reduce blood pressure effectively with minimal side effects.

2.Percutaneous needle aspiration and biopsy of localized pulmonlary lesions

Over a period of recent 12 months, 41 patients who had localized pulmonary lesions, but were not diagnosedpathologically through bronchoscopy, transbronchial biopsy, sputum examination and cervical lymph node biopsy,underwent percutaneous needle aspiration and biopsy with a diagnostic accuracy of 83% concerning malignancy vs.benignity and 78% considering the specific cell types as well. Six patients developed small asymptomaticpneumothorax and the other three develped small amount of hemoptysis; One of them required treatement. There were21 malignancies and 14 benign lesions. Three cases were misinterpreted and five were inconclusive on cytology,maybe due to inadequate material. No material was aspirated in one case due to hardness of the mass, which, later,confirmed chondroarcoma on surgery. The method, problems and complications are discussed. Nowadays, thepercutaneous needle aspiration and biopsy appears to be the procedure of choice in pulmonary coin lesionsespecially when they are smaller, more peripheral, and whenever meastatic neoplasm in suspected.

3.The Association between Influenza Treatment and Hospitalization-Associated Outcomes among Korean Children with Laboratory-Confirmed Influenza.

There are limited data evaluating the relationship between influenza treatment and hospitalization duration. Our purpose assessed the association between different treatments and hospital stay among Korean pediatric influenza patients. Total 770 children < or = 15 yr-of-age hospitalized with community-acquired laboratory-confirmed influenza at three large urban tertiary care hospitals were identified through a retrospective medical chart review. Demographic, clinical, and cost data were extracted and a multivariable linear regression model was used to assess the associations between influenza treatment types and hospital stay. Overall, there were 81% of the patients hospitalized with laboratory-confirmed influenza who received antibiotic monotherapy whereas only 4% of the patients received oseltamivir monotherapy. The mean treatment-related charges for hospitalizations treated with antibiotics, alone or with oseltamivir, were significantly higher than those treated with oseltamivir-only (P < 0.001). Influenza patients treated with antibiotics-only and antibiotics/oseltamivir combination therapy showed 44.9% and 28.2%, respectively, longer duration of hospitalization compared to those treated with oseltamivir-only. Patients treated with antibiotics, alone or combined with oseltamivir, were associated with longer hospitalization and significantly higher medical charges, compared to patients treated with oseltamivir alone. In Korea, there is a need for more judicious use of antibiotics, appropriate use of influenza rapid testing.

4.Total Antioxidant Status in Maternal and Neonatal Plasma According to Delivery Type.

Antioxidants oppose the toxic actions of lipid peroxides and oxygen radicals, and they limit the amount of lipid peroxides formed. Women with normal pregnancies have an increase in oxidative stress and lipid peroxidation when compared with nonpregnant women. The antioxidants also increase progressively with advancing gestation, and the antioxidants in the fetus also increase with advancing gestation, especially during late gestation. The purpose of this study is to investigate the diefference of antioxidant status in maternal and neonatal plasma according to the delivery type. So, we investigated the status of antioxidant in the maternal and neonatal plasma when compared normal vaginal delivery with Cesarian section without labor. This study was done under the hypothesis that labor pain is originated from the hypoxic state of myometrium during uterine contraction. The number of women who were studied was total 56, 24 women were delivered by normal vaginal delivery and 32 women were done by Cesarean section without labor. The blood was sampled during active labor and postpartum 24 hour in the case of normal vaginal delivery, the women who were done Cesarean section being sampled before operation and 24 hours after operation. The neonatal blood was sampled from cord, birth 1 day and 3 day. The antioxidant levels were measured by Total Antioxidant Status (Randox Laboratory Ltd., UK) kit. The results were as follows. 1. The mean maternal plasma antioxidant status was not significantly different according to delivery types and not significantly different when compared antepartum with postpartum (vaginal delivery; antepartum: 1.54+/-0.31 mM/L, postpartum: 1.58+/-0.32 mM/L, Cesarean section; antepartum: 1.55+/-0.29 mM/L, postpartum: 1.56+/-0.33 mM/L). 2. The neonatal antioxidant status was not significantly different between the neonates who were born by vaginal delivery and the neonates who were born by Cesarean section. The neonatal plasma antioxidant status was increased progressively after birth (cord: birth 1 day: birth 3 day=1.46+/-0.25 mM/L: 1.59+/-0.25 mM/L: 1.79+/-0.37 mM/L). Therefore, labor pain was not seems to be significantly affect the maternal and neonatal plasma antioxidant status. And the neonatal antioxidant status was increased for adaptation to the external environment after birth.

5.Understanding the Response Characteristics of X-ray Verification Film.

PURPOSE: This study is intended to understand the sensitometric charac teristics and the emulsion properties of the commercially available CEA TVS film in comparison with the Kodak X-Omat V film. MATERIAL AND METHODS: For this purpose, we have formulated an analytic expression of the characteristic curves for x-ray film exposed to mixed radiation of electrons, photons, and visible light. This mathematical expression was developed based on reaction-rate and target-hit theories. Unlike previous expressions, it relates optical density to emulsion properties such as grain size and silver bromide content. We have also developed a quantity which characterizes the film response to visible light relative to that to photons and electrons. This quantity could be expressed as a function of grain area. Thus, we have developed mathematical expressions and quantities with which the emulsion properties of the films can be revealed based on the sensitometric characteristics. Demonstrating the use of this analytical study, we exposed CEA and Kodak verification films to the mixed radiation of electrons, photons, and visible light, and interpreted the experimental results accordingly. RESULTS: We have demonstrated that: (1) the saturation density increases as the silver bromide content increases, (2) the time required to reach the threshold dose (to which the film begins to respond) when films are exposed to visible light decreases as the grain size increases, and (3) the CEA film contains more silver bromide, whereas the Kodak film contains larger grains. These findings were supported by the data provided by the manufacturers afterward. CONCLUSION: This study presented an analytical and experimental basis for understanding the response of X-ray film with respect to the emulsion properties.

6.Common Misconceptions in People With Epilepsy.

BACKGROUND AND PURPOSE: This study was undertaken to determine the knowledge that people with epilepsy (PWE) have regarding the nature of epilepsy and its management, and also to identify the factors contributing to their knowledge of epilepsy. METHODS: We studied 79 consecutive PWE who visited the outpatient clinic of Seoul National University Hospital using a structured questionnaire consisting of 27 questions in 3 categories. The mean correct response rate was 61%, with 81% believing that brain cells die during a seizure, 29% considering it dangerous to take a bath or shower alone, and more than 70% believing that taking antiepileptic drugs (AEDs) will impair memory and damage the liver and kidneys. RESULTS: The mean overall correct-answer rate was significantly related to gender, length of education, type of seizures, and regularity of hospital visits (all p<0.05). CONCLUSIONS: The level of knowledge deviated significantly from the scientific data, especially in the causes of epilepsy, safety issues, and side effects of AEDs. A large-scale study should identify those PWE with the lowest knowledge of epilepsy, and then develop and implement suitable educational intervention programs to improve their knowledge.

7.Management of complex surgical wounds of the back: identifying an evidence-based approach

Background:
Postoperative dehiscence and surgical site infection after spinal surgery can carry serious morbidity. Multidisciplinary involvement of plastic surgery is essential to minimizing morbidity and achieving definitive closure. However, a standardized approach is lacking. The aim of this study was to identify effective reconstructive interventions for the basis of an evidence-based management protocol.
Methods:
A retrospective review was performed at a single tertiary institution for 45 patients who required 53 reconstruction procedures with plastic surgery for wounds secondary to spinal surgery from 2010 to 2019. Statistical analysis was performed for demographics, comorbidities, and treatment methods. Primary outcomes were postoperative complications, including dehiscence, seroma, and infection. The secondary outcome was time to healing.
Results:
The overall complication rate was 32%, with dehiscence occurring in 17%, seroma in 15% and infection in 11% of cases. Median follow-up was 10 months (interquartile range, 4–23). Use of antibiotic beads did not affect rate of infection occurrence after wound closure (P=0.146). Use of incisional negative pressure wound therapy (iNPWT) was significant for reduced time to healing (P=0.001). Patients treated without iNPWT healed at median of 67.5 days while the patients who received iNPWT healed in 33 days. Demographics and comorbidities between these two groups were similar.
Conclusions
This data provides groundwork for an evidence-based approach to soft tissue reconstruction and management of dehiscence after spinal surgery. Timely involvement of plastic surgery in high-risk patients and utilization of evidence-based interventions such as iNPWT are essential for improving outcomes in this population.
8.Trends in the Charges and Utilization of Computer-Assisted Navigation in Cervical and Thoracolumbar Spinal Surgery

Methods:
Relevant data from the National Readmission Database in 2015–2018 were analyzed, and the computer-assisted procedures of cervical and thoracolumbar spinal surgery were identified using International Classification of Diseases 9th and 10th revision codes. Patient demographics, surgical data, readmissions, and total charges were examined. Comorbidity burden was calculated using the Charlson and Elixhauser comorbidity index. Complication rates were determined on the basis of diagnosis codes.
Results:
A total of 48,116 cervical cases and 27,093 thoracolumbar cases were identified using computer-assisted navigation. No major differences in sex, age, or comorbidities over time were found. The utilization of computer-assisted navigation for cervical and thoracolumbar spinal fusion cases increased from 2015 to 2018 and normalized to their respective years’ total cases (Pearson correlation coefficient=0.756, p =0.049; Pearson correlation coefficient=0.9895, p =0.010). Total charges for cervical and thoracolumbar cases increased over time (Pearson correlation coefficient=0.758, p =0.242; Pearson correlation coefficient=0.766, p =0.234).
Conclusions
The use of computer-assisted navigation in spinal surgery increased significantly from 2015 to 2018. The average cost grossly increased from 2015 to 2018, and it was higher than the average cost of nonnavigated spinal surgery. With the increased utilization and standardization of computer-assisted navigation in spinal surgeries, the cost of care of more patients might potentially increase. As a result, further studies should be conducted to determine whether the use of computer-assisted navigation is efficient in terms of cost and improvement of care.

BACKGROUND: Neutrophils are considered to play critical roles in the development of acute respiratory distress syndrome. Histamine, which is distributed abundantly in lung tissue, increases the rolling of neutrophills via increase of P-selectin expression on the surface of endothelial cells and is known to have some interrelationships with IL-1, IL-8 and TNF-alpha. We studied to investigate the effect of the histamine on the acute lung injury of the rats induced by intratracheal insufflation of TNF-alpha which has less potency to cause lung injury compared to IL-1 in rats. METHODS: We intratracheally instilled saline or TNF(R&D, 500ng), IL-1(R&D, 50ng)or histamine of varius dose(1.1, 11 and 55 microg/kg) with and without TNF separately in Sprague-Dawley rats weighing 270-370 grams. We also intratracheally treated IL-l(50ng) along with histamine(55 microg/kg). In cases, there were synergistic effects induced by histamine on the parameters of TNF-induced acute lung injury, antihistainmes(Sigma, mepyramine as a H1 receptor blockade and ranitidine as a H2 receptor blockade, 10 mg/kg in each)were co-administered intravenously to the rats treated TNF along with histamine(1.1 microg/kg) intratracheally. Then after 5 h we measured lung lavage neutrophil numbers, lavage cytokine-induced neutrophil chemoatt- ractants(CINC), lung myeloperoxidase activity(MPO) and lung leak. We also intratracheally insufflated TNF with/without histamine(11 microg/kg), then after 24 h measured lung leak in rats. Statistical analyses were done by Kruskal-Wallis nonparametric ANOVA test with Dunn's multiple comparison test or by Mann-Whitney U test. RESULTS: We found that rats given TNF, histamine alone(11 and 55 microg/kg), and TNF with histamine(1.1, 11, and 55 microg/kg) intratracheally had increased (P<0.05) lung MPO activity compared with saline-treated control rats. TNF with histamine 11 microg/kg had increased MPO activity (P=0.0251) compared with TNF-treated rats. TNF and TNF with histamine(l.l, 11,, and 55 microg/kg) intratracheally had all increased (P<0.05) lung leak, lavage neutophil numbers and lavage CINC activities compared with saline. TNF with histamine 1.1 microg/kg had increased (P=0.0367) lavage neutrophil numbers compared with TNF treated rats. But there were no additive effect of histamine with TNF compared with TNF alone in acute lung leak on 5 h and 24 h in rats. Treatment of rats with the H1 and H2 antagonists resulted in inhibitions of lavage neutrophil accumulations and lavage CINC activity elevations elicited by co-treated histamine in TNF-induced acute lung injury intratracheally in rats. We also found that rats given IL-1 along with histamine intratracheally did not have increase in lung leak compared with IL-1 treated rats. CONCLUSION: Histamine administered intratracheally did not have synergistic effects on TNF-induced acute lung leak inspite of additive effects on increase in MPO activity and lavage neutrophil numbers in rats. These observations suggest that instilling histamine intratracheally would not play synergistic roles in neutrophil-mediated acute lung injury in rats.
