This paper explored the current conceptual framework of disability and rehabilitation, and constructed theoretical structurewith three layers of international convention, resolutions and policies, and domestic laws and operational tools. The characteristics of community-based rehabilitation (CBR) had been discussed with inclusive development approach. Some recommendations had been proposed forthe development of rehabilitation and work programs of rehabilitation for all in China.

OBJECTIVE: Microgravity culture is an emerging cultural technology,which can be used in many fields such as tissue engineering and cell culture,etc. Microgravity bioreactor was used for cell culture in simulated microgravity environment.DATA SOURCE: We search the PubMed database from January 1995 to June 2002, using the key words "microgravity,simulated microgravity,bioreactor, tissue engineering, cell culture, rotating cell culture system" and the language was confined to English.STUDY SELECTION: Inclusion criteria: ① Research of cell culture in simulated microgravity condition. ② Research of tissue engineering of microgravity. ③ Randomized controlled empirical study. ④ Establishment of the simulated microgravity environment. Exclusion criteria: ①Quite obsolete literatures. ② Repeated research.DATA EXTRACTION: A total of 288 articles on reparation of spinal cord injury were collected, according to the inclusion criteria 19 articles were included, and the other 269 articles were excluded because they were repetitive and duplicated researches.DATA SYNTHESIS: ①Cultivation in simulated microgravity had its particular feature and practicability: Shearing force formed by the turbulent flow was avoided; Culture fluid was admixed by rotating; There exists Zero headspace; Anchorage-dependent cell could grow on the carriers; Sufficient oxygenation was provided. ② Cell growth in simulated microgravity environment: cells were prone to aggregate, and much higher cell density was generated. ③ Three-dimensional culture system of animal cells was constructed with microgravity bioreactor to satisfy organ surrogate or requirement of investigation in vitro. ④ Angioid tubiform fabric was generated in the three-dimensional tissue structure constructed with microgravity bioreactor, which was used to generate the blood vessel pipage of tissue engineering. ⑤ Problems needing to solve: Once separated from the microgravity environment, whether the function of three-dimensional structure constructed with microgravity bioreactor can be still remained for long term or not and the structure will be kept invariably or not after transplanted and integrated to parasitifer tissue need further research.CONCLUSION: Technique of simulated microgravity culture is a original tissue culture technology. The central technique is the construction of three-dimensional culture system of animal cells, which can provide a favourable cultural surrounding for the growth and metabolism of mutiplicate cells and tissue blocks, carry out the tissue culture of high density, and keep the specificity of histological differentiation of cultural cells.

Objective To explore the value and the most reasonable procedure of EN by naso-intestinal tube in digestive tract disease after operation.Methods Data of 25 cases of indigestive tract disease subject to EN through a naso-intestinal tube after operation were reviewed and analysed.Results Insertion of naso-intestinal tube was completed in all cases.Nasal feeding could be completed in 24 cases;postoperative complications included 1 case of leakage,1 case of arrhythmia and heart failure and 4 cases of fat liquefication.Conclusion EN by naso-intestinal tube in digestive tract disease was favorable for reduction of postoperative risk.It could be applied in almost all pa- tients when the doctor had grasped the technique skillfully.

Objective To develop a new culture system in which hepatocytes can restore in-vivo like function. Methods Sandwich configuration was used for the culture of hepatocytes, and protein synthesis as well as P 450-b ,albumin mRNA expression by in situ hybridization was studied. ResultsStable and gradually increasing protein synthesis and high level of P 450-b , albumin mRNA expression were observed. In contrast, protein secretion of hepatocytes cultured in conventional single collagen culture system was low and decreased until approximately zero seven days after, and mRNA expression of P 450-b ,albumin was low. Conclusions Sandwich configuration in which cell polarity of hepatocytes is reestablished like in vivo situation.

Objective To investigate the effects of ?-lipoic acid on the proliferation and activation of NF-?B induced by high glucose (HG) in rat mesenteric cells (MCs). Methods The rat mesenteric cells were cultured in the medium with normal glucose (5.6mmol/L, NG), high glucose (25mmol/L, HG), HG+100 ?mol/L ?-lipoic acid, or HG+200?mol/L ?-lipoic acid and HG+PDTC (a NF-?B inhibitor). Activation of nuclear factor-?B (NF-?B) of rat mesenteric cells were measured by electrophoretic mobility shift assay (EMSA). The cell proliferation was assessed by MTT. OBJECTIVETo compare the efficacy difference in the treatment of functional dyspepsia between acupuncture at the acupoints selected by pattern/syndrome differentiation and domperidone.

METHODSSeventy cases were randomized into an acupuncture group (35 cases) and a western medication group (35 cases). In the acupuncture group, Zusanli (ST 36) and Neiguan (PC 6) were selected. Taichong (LR 3) and Neiting (ST 44) were added for excess syndrome while Gongsun (SP 4) and Yinlingquan (SP 9) were added for deficiency syndrome. A pair of electrodes was attached to one acupoint and an assistant point (2 mm next to the acupoint centripetally) and stimulated with disperse-dense wave at 2 Hz/100 Hz, once a day. In the western medication group, domperidone was prescribed for oral administration, 10 mg each time, three times a day. In the two groups, the treatment of 5 days made one session and 4 sessions were required totally. Nepean dyspepsia index (NDI) was compared after treatment, 1, 2, 3, 4 and 5 months after treatment between the two groups respectively.

RESULTSThe score of symptom and score of life quality in NDI after treatment and at each follow-up time point were improved obviously in the acupuncture group as compared with those before treatment (all P < 0.01). In the western mediation group, the score of symptom and the score of life quality in NDI after treatment and in follow-up of 1, 2 and 3 months were improved obviously as compared with those before treatment (all P < 0.01), but the differences were not significant in follow-up of 4 and 5 months (both P > 0.05). Compared with the western medication group, the symptom score of NDI was reduced obviously after treatment and in each time point of follow-up in the acupuncture group (P < 0.05, P < 0.01), and the score of life quality was increased obviously (P < 0.05, P < 0.01).

CONCLUSIONAcupuncture at the acupoints selected by pattern/syndrome differentiation and domperidone are effective in the treatment of functional dyspepsia. Domperidone is unsatisfactory in the long-term effect, but acupuncture achieves the positive short-term and long-term effects on functional dyspepsia.

AIM: To evaluate the diagnostic accuracy of macular ganglion cell - inner plexiform layer ( GCIPL ) measurements using high- definition optical coherence tomography ( Cirrus HD - OCT ) ganglion cell analysis algorithm for detecting early and moderate to severe glaucoma.
METHODS:Twenty normal control persons, 26 patients with early glaucoma and 29 patients with moderate to severe glaucoma were enrolled in this study. Macular GCIPL, optic nerve head ( ONH ) parameters and peripapillary retinal nerve fiber layer ( RNFL ) thickness were measured in each subject. Then all measured results of each parameter were calculated using SPSS17. 0. Areas under the receiver operating characteristic curves ( AUC) of each parameter were calculated to compare the diagnostic accuracy for detecting early and moderate to severe glaucoma.
RESULTS: For detecting early glaucoma, AUC of average RNFL and seven clock value of RNFL were the biggest ( 0. 871 and 0. 896 respectively ), the AUC of parameters in GCIPL were also significant, among them,
the average GCIPL showed bigger AUC(0. 847) than the minimum GCIPL (0. 812). For diagnosing moderate to severe glaucoma, the AUC of rim area was 0. 992, which was bigger than that of average RNFL ( 0. 991 ). The minimum GCIPL showed bigger AUC ( 0. 983 ) than the average GCIPL (0. 967). For early glaucoma diagnosis, the sensitivity of average RNFL was the highest (76. 9%), while the average GCIPL has the highest specificity (93. 5%).
CONCLUSION: AS a new diagnostic parameter for detecting glaucoma, GCIPL shows similar diagnostic potential compared with RNFL. For early glaucoma diagnosis, average RNFL is the most important parameter, while screening early glaucoma, average GCIPL should be paid more attention.

ObjectiveTo determine whether there is correlation between the pathological classification and urinary level of transforming growth factor(TGF)-β1,macrophage inflammatory protein (MIP)-1α,and Neutrophil gelatinase-associated lipocalin (NGAL) of lupus nephritis.MethodsELISA was used to test the levels of urinary level of TGF-β1,MIP-lα and NGAL.The correlation between these three urinary markers and the pathological classifications,Austin score,histological semi-quantitative score and clinical data were analyzed.Comparisons between groups were performed with t-test,ANOVA and X2 test.Correlation analysis was performed with Pearson analysis.Results①There was significant difference in urinary TGF- β1,MIP1α,and NGAL levels when compared the lupus nephritis group,SLE patients without renal damage group and the normal control group [TGF-β1(351±219),(92±60),(74±29) pg/ml],[MIP-1α(18.0±15.5),(8.5±2.3),(7.1±1.9) pg/ml],[NGAL (1.104±0.519),(0.181±0.030),(0.146±0.024) ng/ml],while there was no significant difference between lupus nephritis (LN) group and patients with other glomerulonephritis.② There was significant difference in the three urinary markers when stratified the LN patients based on the pathological classifications,however,there was no significant difference between the LN group and other glomerulonephritis group.③ Analysis of the correlation between the three urinary markers and semiquantitative histopathological score of the LN patients showed that the urinary level of TGF-β1 and MsMI,GCI,TCI,VCI was closely related to each other,the urinary MIP-1α was related to and endol,and the dGAI and VCI was closely related, while the urinary NGAL was closely related with endol,dGAI and MsHI.The correlation analysis between the three urinary markers and acute and chronic pathological index score of the LN patients showed that TGF- β1 was correlated with the chronic index(r=0.89,P＜0.01 ),the MIP-lα and NGAL were significantly correlated with the activity index(r=0.71,P＜0.01 ; r=0.60,P＜0.01 ).Conclusion There is a positive correlation between the urine TGF-β1 level and chronic kidney disease.The urinary MIP-lαand NGAL are associated with active renal damage.

Objectives To study the significance of using kappa value in surgical clinical teaching.Methods The interns in Department of General Surgery were included,and were divided into routine teaching group and targeted teaching group randomly. Kappa statistic was used to compare the consistency of physical examination.Results The results showed that Morris's point tenderness was grasped more efficiently in targeted teaching than routine teaching group.Conclusion With the help of kappa value teachers can find the teaching items more correlated with training and give students targeted teaching so as to improve the teaching effect.
