1.Effect of metformin on endothelial function in women with polycystic ovary syndrome

Objective To evaluate the effect of metformin on endothelial function in women with polycystic ovary syndrome.Methods 30 women with PCOS were enrolled.Metformin were taken 500mg three times a day contiguously using 3 months.Before and after treatment flow-mediated EDD function and other clinical metabolic parameters,such as serum endothelin-1,insulin,testosterone,glucose,lipid,waist-hip ratio,body mass index.Results After the treatment of metformin in study group,WHR,BMI and endothelin-1 have no significant difference compared with before the treatment(P>0.05).But the changes of brachial artery diameter and HDL were increasing and LH/FSH,T,insulin,HOMA-IR,TG and LDL were significant decreasing after the treatment in study group compared with before the treatment(P<0.05).There was no significant difference in WHR,BMI,endothelin-1,the changes of brachial artery diameter,LH/FSH,T,insulin,HOMA-IR,TG and LDL between the study group after the treatment with the control group.Meanwhile,HDL were significant increasing in the study group after the treatment compared with the control group(P<0.05).Conclusion There are impaired endothelial function and insulin resistance in women with PCOS.After the treatment with metrormin,the endothelial function and insulin resistance could be improved.
2.The research of low-dose mifepristone on the treatment of anovulatory functional uterine bleeding

Objective To explore the effect of low-dose imfepristone on the endometrial simple hyperplasia.Methods Study group were randomly divided into two groups.43 cases in group mifepristone,43 cases in group norethindrone and control group were 20 cases.The endometrial histopathology and the contents of ER and PR were examined before and after the treatment.The changes of endometrial thickness and hemoglobin were detected,too.Results The endometria were all simple hyperplasia before taking the medicine and turned to proliferative stage after taking mifepristone and norethindrone.There were significant difference compared with taking the medicine before(P<0.05).However,there were no statistical difference between two groups after taking the medicine(P>0.05).The endometrial ER and PR were significantly depressed after taking the medicine compared with before in study group(P<0.05).There were no significant difference between control group and group mifepristone after taking mifepristone(P>0.05).But there were significant difference between controI group and group norethindrone after taking norethindrone(P<0.05).The endometrum were significant turned thin and the contents of hemoglobin were significantly risen up after taking the medicine in study group(P<0.05).But the contents of hemoglobin were significantly risen up after taking the medicine in group mifepristone compared with group norethindrone(P<0.05).Conclusions Low-dose mifepristone can inhibit endometrial simple hyperplasia and turn the endometria from simple hyperplasia to proliferative stage.Low-dose mifepristone may adjust the endometrial ER and PR for inhibitting endometrial hyperplasia and make the expression of endometrial ER and PR decrease.But the contents of ER and PR were more close to normal after taking mifepristone.Low-dose mifepristone can attenuate the endometria and regain the hemoglobin fast.There are seldom side-effects after taking low-dose mifepristone.
3.Research about Bevacizumab combined with FOLFOX protocol to treat metastasis colorectal cancer

The common therapy of colorectal cancer is FOLFOX scheme,which contains flurouracil,leucovorin and oxaliplatin.Numerous clinical trials have demonstrated that bevacizumab combined with FOLFOX scheme in cancer's therapy is safe and effective.But the adverse reactions including hypertension,neurovirulence,gastrointestinal bleeding and perforation are raised up.Scholars have carried out a series of studies for the overall survival times,tumor response rates and survival qualities for the patients with metastatic colorectal cancer which using the joint scheme,but they draw different conclusions the usefulness and safety of the joint scheme still need more RCT and meta-ananlysis to be proved.
4.The antibiotic susceptibility and extended spectrum β-lactamase genotype of clinical Klebsiella pneumoniae

Objective To analyze the antibiotic susceptibility, ESBL genotype of clinical Klebsiella pneumoniae strains isolated from People′s hospital and facilitate the control of resistance spread. Methods Identification and antibiotic susceptibility tests of 1 205 strains from 2001 to 2007 were done by VITEK-2 system.The antibiotic susceptibility results were analyzed by whonet5.3.The ESBL gene was detected by PCR and the Chi-square test was used for statistical analysis.Results The rate of ESBL-producing strains in klebsiella pneumoniae has increased from 2001 to 2007[18.8% (40/213) in 2001, 20.9% (53/253) in 2002, 32.8% (42/128) in 2003, 33.6% (45/137) in 2004, 36.6% (60/164) in 2005, 45.3% (68/150) in 2006 and 45.6% (73/160) in 2007].The SHV gene was the most dominant in ESBL genotypes.There were 83.3% (50/60) ESBL strains in 2005 with SHV gene, 82.3%(56/68) in 2006 and 83.6%(61/73) in 2007.The rated of strains with CTX-M gene were increasing.There were 26.7%(16/60) ESBL strains with CTX-M gene in 2005, 36.7%(25/68) in 2006 and 54.8%(40/73) in 2007.The isolates with more than one type of ESBL gene were increasing.There were 45%(27/60) ESBL strains in 2005 with two types of ESBL gene, and no one had more than two types of ESBL gene in that year.There were 47.9%(35/73) ESBL strains in 2007 with two types of ESBL gene.In 2007 there were 9.6%(7/73) and 2.7%(2/73) ESBL strains with three types and four types of ESBL gene respectively.There was a statistical difference between the antibiotic resistance rates of cefotaxime, ceftriaxone and ceftazidime in SHV-gene-phore strains (χ2=13.22, P<0.01).The strains with SHV gene were more resistant to cefotaxime than ceftriaxone and ceftazidime.There also was a statistical difference of the antibiotic resistance rate of cefotaxime, ceftriaxone and ceftazidime between strains with TEM gene (χ2=9.91, P<0.01) and CTX-M gene (χ2=34.84, P<0.01) respectively.None of the strains with CTX-M gene was sensitive to cefotaxime, and they were more resistant to ceftriaxone than ceftazidime.The strains with TEM gene were more resistant to cefotaxime than ceftriaxone and ceftazidime.There were statistical differences of the antibiotic resistance rate to cefotaxime (χ2=29.65, P<0.01), ceftriaxone (χ2=20.26, P<0.01) and ceftazidime (χ2=20.26, P<0.01) between the strains with SHV gene only and strains with SHV and CTX-M gene concurrently.There were also statistical differences of the antibiotic resistance rates to cefotaxime (χ2=11.01, P<0.01), ceftriaxone (χ2=9.93, P<0.01) and ceftazidime (χ2=7.01, P<0.01) between the strains with SHV gene only and strains with SHV and TEM gene concurrently.The antibiotic resistance rates to cefotaxime (χ2=11.54, P<0.01), ceftriaxone (χ2=17.58, P<0.01) and ceftazidime (χ2=14.11, P<0.01) were statistically different between the strains with SHV gene only and strains with SHV and OXA gene concurrently.The antibiotic resistance rates to ceftazidime (χ2=23.61, P<0.01) were statistically different between the strains with CTX-M gene only and strains with SHV and CTX-M gene concurrently. There was no statistical difference in antibiotic resistance rates to cefotaxime (χ2=3.55, P<0.01) and ceftriaxone (χ2=3.35, P<0.01) between the strains with CTX-M gene only and strains with SHV and CTX-M gene concurrently. The antibiotic resistance rates to ceftazidime (P=0.01) were statistically different between the strains with only TEM gene and strains with SHV and TEM gene concurrently, and there was no statistical difference of the antibiotic resistance rates to cefotaxime (P=0.29) and ceftriaxone (P=0.26) between the strains with TEM gene only and strains with SHV and TEM gene concurrently. ConclusionsThe producing rate of ESBL is increasing year after year and the SHV type of ESBL is the dominant one.Strains with more than one type of ESBL gene are increasing.The antibiotic resistance rates to cefotaxime, ceftriaxone and ceftazidime are statistically different between strains with same ESBL genotype.
5.Postoperative recurrence of pelvic solitary fibroma with bladder metastasis: a case report

Solitary fibroma often occurs in the pleura, but rarely outside the pleura. Extrapleural solitary fibroma has atypical clinical symptoms and difficult imaging diagnosis. Here, we report a case of solitary fibroma recurred and metastasized to the bladder after pelvic surgery. He was treated with palliative resection in our hospital. The patient died of systemic metastasis 3 years after operation.

7.Effect of REZ-I cochlear implantation on residual hearing in adult patients.

OBJECTIVETo evaluate the effect of cochlear implantation with REZ-I straight electrodes on residual hearing of postlingually deafened adults, and to explore the audiologically safety and injury characteristics of cochlear implantation.
METHODSSixteen unilateral REZ-I (22 channels) cochlear implantation recipients from September 2009 to December 2009 were picked out. Their pre-and post-implantation audiometry data including pure-tone audiometry (PTA), auditory steady-state responses (ASSR), auditory brainstem responses (ABR) and distortion product otoacoustic emissions (DPOAE) were retrospectively analyzed, in order to compare the change between pre- and post-implantation residual hearing.
RESULTSAmong the 12 recipients who had some measurable residual hearing before implantation, 5 (41.6%) patients had conserved some measurable hearing but the other 7 (58.4%) recipients had lost all measurable hearing after implantation on the implanted side. The implanted ears had an average PTA threshold drop of 9.5 dB HL and a statistically significant difference between pre- and post-implantation (P < 0.05) PTA thresholds in the frequencies of 250 Hz, 500 Hz, 1000 Hz, 2000 Hz and 4000 Hz. Compared to non-implanted ears, the drop in 500 Hz and 1kHz had a statistically significant difference between pre- and post-implantation PTA thresholds (P < 0.05). The ASSR residual hearing threshold elevation were statistically significant (P < 0.05) between pre- and post-implantation ASSR at 250 Hz and 500 Hz on the implanted side, while the The ASSR residual hearing threshold elevation were statistically significant (P < 0.05) at 500 Hz when compared to non-implanted side. The difference of residual hearing between pre- and post-implantation was not statistically significant for both DPOAE and ABR.
CONCLUSIONThere will be a certain degree of damage to residual hearing of the implanted side following REZ-I cochlear implantation.

8.Cutaneous permeation kinetics and pharmacodynamics of topical lidocaine gel in rat.

AIMTo study the cutaneous permeation kinetics and pharmacodynamics of lidocaine gel.
METHODSThe concentration of lidocaine in dermis following topical application in rats was determined by the cutaneous microdialysis technique and related parameters were calculated; the pharmacodynamics of the gel was evaluated by electric stimulation method with EMLA (eutectic mixture of local anesthetics) cream as a control.
RESULTSThe peak of percutaneous absorption kinetic profile of lidocaine gel across rat skin occurred at 1.25 h; the onset time of local anesthetic action of lidociane gel was similar to that of EMLA, but both the duration and depth of anesthetic effect were superior to EMLA cream.
CONCLUSIONLidocaine gel showed good anesthetic effect. The minimum effective concentration of lidocaine in dermis is 12 mg.L-1.

9.Gene coding and mRNA expression of vascular endothelial growth factor as well as microvessel density in brain of plateau zokor: comparison with other rodents.

Vascular endothelial growth factor (VEGF) plays an important role in tissues angiogenesis. The adaptation of animals to hypoxic environment is relative to the microvessel density (MVD) in tissues. To further explore the adaptation mechanisms of plateau zokor (Myospalax baileyi) to the hypoxic-hypercapnic burrows, the VEGF mRNA and the MVD in cerebral tissues of the plateau zokor were studied. Total RNA was isolated from liver, and VEGF cDNA was obtained by RT-PCR, then the VEGF cDNA was cloned and sequenced. The coding sequence of plateau pika (Ochotona curzniae), rat (Rattus norvegicus) and mouse (Mus musculus) VEGF cDNA are obtained from GenBank, and the nucleotide and amino acid sequence homology of plateau zokor VEGF cDNA coding sequence with that of plateau pika, rat and mouse were analyzed and compared by using of bioinformatics software. The VEGF mRNA was detected by real-time PCR, and the MVDs in cerebral tissues of the plateau zokor, plateau pika and Sprague-Dawley (SD) rat were measured by immunohistochemical staining. The results showed that the open reading frame of the plateau zokor VEGF was 645 bp, and the coding sequence of the plateau zokor VEGF cDNA shared 92.1%, 93.6% and 93.8% nucleotide sequence homology to that of the plateau pika, rat and mouse, respectively. The deduced amino acid sequence of the plateau zokor VEGF cDNA was composed of 188 amino acids and the amino acids from 1 to 26 were signal peptide sequence. The plateau zokor VEGF188 was 90.2%, 94.9% and 94.4% homologous to that of plateau pika, rat and mouse. The level of VEGF mRNA in brain of the plateau zokor was significantly lower than that of SD rat, but there was no obvious difference in VEGF mRNA level between plateau zokor and plateau pika. The MVD in brain of the plateau zokor was markedly higher than that of plateau pika and SD rat. In conclusion, plateau zokor enhances its adaptation to the hypoxic environment by increasing the MVD. The level of VEGF mRNA in the brain of plateau zokor is lower than that of SD rat, which may be as a result of inhibition by the higher concentration of carbon dioxide in the burrow.

10.Study on protective and haemodynamic effects of Danshen Tongluo capsule on rats with myocardial infarction.

To study the protective effect of Danshen Tongluo capsule on rat hearts in the myocardial infarction (MI) model. After being fed with high fat diets for one month, the SD rats were randomly divided into the sham group and the model group according to the left ventricular ejection fraction (EF). The MI model group was duplicated by ligating coronary artery and then divided into five groups: the model group, the positive control group (model + captopril), the small dose group (model + Danshen Tongluo), the medium dose group (model + Danshen Tongluo) and the high dose group (model + Danshen Tongluo). Four weeks later, changes in myocardium ultra-structure were observed by hemodynamics, cardiac ultrasound and electron microscope. The results showed: (1) All doses of Danshen Tongluo capsule could significantly reduce the left ventricular end diastolic pressure (LVEDP) (P <0.01), increase the maximum internal pressure of the left ventricle (+ dp/dtmax), and lower the drop rate of left ventricular pressure (- dp/dtmax), with statistical significances in medium and high dose groups; (2) The B ultrasound results showed increase in EF and left ventricular shortening fraction (FS) in all dose groups of Danshen Tongluo capsule; (3) The medium dose group showed significant decrease in myocardial infarction index (P <0.01) and injured and fractured myofilament and sarcomere of ischemic myocardium in myocardial ultra-structure; All of Danshen Tongluo capsule-treated groups revealed reduction in myocardial injury and myocardial infraction area. The study preliminarily proves that Danshen Tongluo capsule can improve hemodynamic function, and has a protective effect on myocardial ischemia.
