Ejaculatory incompentence is not able to ejaculate intravaginally. Psychological causes are generally associated with an inability to ejaculate intravaginally. The treatment of ejaculatory incompetence is very difficult and complicated. The treatment of retrograde ejaculation is much more difficult. Such patients have a very low volume ejaculate but numerous sperm can be found in the urine and on physical examination, testicles are of normal size. Sperm recovery from the urine could be used for AIH. The treatment for some men with paraplegia who do not ejaculate with a vibrator. Background: Ankle arthroscopy has been used in Viet Duc Hospital for 4 years and now has become a routine surgery with about 200 patients, who has had ankle arthroscopy to reconstruct the anterior cruciate ligament or operate ragged meniscectomy. Objectives: Evaluate the result of ankle arthroscopy in Viet Duc Hospital and review the indication and surgical technique of ankle arthroscopy in the literature. Subject and methods: Retrospective study on 10 patients with ankle arthroscopy in Viet Duc Hospital from May 2006 to May 2007. Results: 5 patients with impingement, 4 patients with OCD lesion and 1 case of an old displaced fracture of lateral malleolar. Postoperative evaluation with Olerud Molander, ankle function improves at least 10 points and all the patients are satisfied with the results. Conclusions: Ankle arthroscopy gives good result and can be recommended for other cases.

Background: Laparoscopic cholecystectomy is the first laparoscopic surgery that was performed at Cho Ray hospital in September 1992. In 2005, Quang Tri general hospital has performed successfully the first case of laparoscopic appendectomy. Objectives: To access the preliminary results of application on endoscopic surgery in Quang Tri general hospital. Subjects and method: A prospective study was conducted on 140 patients with laparoscopic surgery or retroperitoneal surgery, was performed in Quang Tri general hospital from September, 2005 to April, 2006. Results:Among 140 patients was operated, there was only one case of postoperative intestinal obstructive complication (accounted for 0.7%), no case of death. Operative aged was between 11 and 70 years old. The average surgical time was 65 minutes for cholecystectomy, 35 minutes for appendectomy, 85 minutes for ureterolithotomy, 80 minutes for gynecological diseases. 2 cases of cholecystectomy and appendectomy changed open surgery. Conclusion: Laparoscopic surgery is a safe and effective method. It can be developed in provincial hospitals. Quang Tri general hospital has successful preliminary developed the new technique.

Objective: To evaluate the effectiveness and safety of electroacupuncture in conjunction with additional medications in providing analgesia and stabilizing hemodynamic parameters during total thyroidectomy. Methods: This randomized controlled trial included 100 patients who underwent a total thyroidectomy between October 2022 and October 2023 at the Vietnam National Hospital of Acupuncture. The patients were randomized into two groups. The electroacupuncture analgesia (EA) group received EA stimulation at five acupuncture points: Hegu (LI 4), Neiguan (PC 6), Shuitu (ST 10), Quepen (ST 12), and Yifeng (SJ 17), while the control group received a bilateral superficial cervical plexus block. Primary outcomes included the level of analgesia and perioperative vital signs in both groups. Additionally, pain thresholds and serum β-endorphin levels were measured before and after electroacupuncture in the EA group. Results: Complete analgesia (Level A) was attained in 86% and 76% of the patients in the EA and control groups, respectively, with no significant difference between the two groups (P = 1.00). In the EA group, the mean pain threshold after receiving EA doubled (648.7 (77.4) g/s vs. 305.3 (45.3) g/s, P < .001), and the mean serum β-endorphin level increased by approximately 13.5 pg/mL (P < .001). All patients remained hemodynamically stable throughout the surgery. Conclusion EA, in conjunction with additional medications that stimulate five acupuncture points, LI 4, PC 6, ST 10, ST 12, and SJ 17, was well tolerated and effectively maintained a suitable level of analgesia and hemodynamic stability during total thyroidectomy.

Background: HIV/AIDS pandemic really become danger to mankind on the earth. Objectives: Description epidemiological characteristics of HIV/AIDS patients. Studying clinical expression, learning aboutdemands of infected patients and community care for infected patients. Subjects and method: 71 HIV/AIDS people in Thua Thien Hue were studied in 2006. Using descriptive method to give epidemiological characteristics and behaviour of HIV/AIDS people. People were determined HIV/AIDS infection based on the criteria of Ministry of Health. The information were collected by target study. Results: HIV/AIDS people mainly from the age of 20 to 39 (83,10%), males were 52,10% and in various levels of education and all kinds of occupation. The rate of sexually transmitted infection was 84,5% and infection transmission was 15,5%. The rate of sexually transmitted infection within the last 12 months was 57,7%. 63,4% of infected people used condom while having sex with their spouses or partners. 1,4% of infected people using drug infection within the last month. 38% of the infected people had manifestations of AIDS, 77,8% of HIV/AIDS were treated with antiviral drugs. Families and communities had positive attitudes and behaviors to the HIV/AIDS people. Infected people being remoted from the society was 12,7%, 88,7% of them receiving supports from their communities such as disease treatment, spirit and material supports and job opportunities. Conclusion: Strengthen communication activities to change behavior of HIV/AIDS people and in communitiy to reduce alienation, discrimination for infected people. Execute socialization in care and support for HIV/AIDS people.

Background: There are many methods used in epidemiological studies of tuberculosis (TB) bacteria but Spoligotyping method is widely used with high accuracy, simple procedure, and carried out on strains containing a little of IS6110 segment \r\n', u'Objectives: To improve Spoligotyping technique and apply this technique to classify Mycobacterium tuberculosis\r\n', u'Subjects and method: Subjects and methods: The study included 12 medical waste samples collected at Thai Binh Hospital of Tuberculosis and 19 samples obtained from the The Hanoi Institute for Tuberculosis and Lung Diseases. Spoligo model of 31 samples were analyzed based on Spotclust and SpoIDB4 database and divided up into family and subfamily.\r\n', u'Results: Spoligotyping technique has good results with the PCR product amplified 40 cycles and presented the film in 18 hours. Obtained results in 31 medical waste samples belong to 4 families: Beijing, EAI, T1 and H3-LAM9. EAI and Beijing are dominant families with 45.16% and 38.7%), respectively. T1 and H3-LAM9 are 12.9% and 3.22%, respectively. The number of samples in the study is little but the obtained rate of different spoligo models of strains are quite diversified (41.9%)\r\n', u'Conclusion: This result is relatively appropriate with previous studies on the distribution of EAI and Beijing families in Vietnam and the world. Background: Situation of extremity injury caused by traffic accidents and occupational accidents are increasing in Vietnam. Therefore, the prevention and fight against these accidents are one of the key objectives to reduce the burden on the families of victims as well as for society. Objectives: To outline situation of extremity injury patients caused by traffic accident, treated in Viet Duc hospital from 2000-2004. Subjects and method: A retrospective study was conducted on 19.845 patients (14.562 males, 5.283 females, aged from 1-97 years old). The subjects were divided into 3 main groups: alone upper extremity injury, alone lower extremity injury and multiple fractures. Results: Extremity injury was common seen in people of working age (78.1%), men more than women (2.76/1 ratio), left foot more than right foot and it was common been in multiple fracture situation due to complex mechanisms of injury (50.3%), alone extremity injury (49.7%). The lower extremity injury was more common seen than upper extremity injury. Open fracture injury (27.3%), in which open fractures in lower extremity were more common seen. Open fractures in both 2 leg bones was the most common seen, accounted for 57.4% among total of open fractures. Amputation accounted for 8.3% among all case of open fractures and 2.3% among all case of extremity injuries. Conclusion: Extremity injury (lower extremity and upper extremity) caused by traffic accidents is common seen. The average, there are 3 emergency surgeries due to open fracture injury were performed daily in Viet Duc hospital per a total of about 11 traumatic emergency surgeries.

Background: Acute Respiratory Infection (ARI) is a common disease in developing countries. Morbidity and mortality of ARI are high, especially among children under 15 years old. Objectives: To describe socio-graphic factors, seasonal patterns, risky areas and determine the morbidity and mortality rates of acute respiratory infections in Thai Binh province. Subjects and method: This retrospective study reviewed the medical records of 4,585 hospital admitted patients who were diagnosed with ARI including upper and lower respiratory infections such as sore throat, pharyngitis, bronchitis, pneumonia and bronchitis-pneumonia at 8 district hospitals and 1 provincial hospital in Thai Binh province during 2002-2005. The selected medical records were based on the available check list and two standard screening tests. Results:Morbidity and mortality of ARI in Thai Binh province were 61.6 and 0.52, respectively. ARI mainly occurred among children under 5 years old, of which the highest mortality was among those under 12 months of age. Male children were at higher risk of acquiring ARI, but less prone to death than female. Occupation did not significantly associate with the risk of ARI. The morbidity increased sharply during inter-season, e.g. March and October. Thai Binh city, Kien Xuong and Tien Hai district were reported with the highest morbidity in accompany of the high mortality as consequences. Conclusion: The prevention and control methods were recommended to annually focus on the male children aged less than 5 years old during March and October in Thai Binh city, Kien Xuong and Tien Hai district.

Background: The combination medical between army and pepople in protection, healthcare for people and army in the whole country was a great policy of The Party and State. Objectives: Evaluate result of combination between armed forces in epidemic control and preventive medicine programs at mountainous and coastal areas of Thua Thien Hue in 2006. Studying knowledge, attitude, practice of army medical officers and men canvassed masses about epidemic control and preventive medicine programs. Subjects and method: Officers, men, army medical officers in 41 communes (28 coastal communes and 13 mountainous communes) of 5 district have combination medical between army and pepople. Using epidemiology method to description, analysis, cross-sectional study. Results: In 2006, home-flies spraying was carried out in 18 out of 20 markets; 1,030 all size restaurants and 18 residential areas of 5 districts. The number of chemically treated latrines in high epidemic areas was accounted fof 15,1%. A population-based latrine construction project was initiated to build 838 new latrines, contributing to minimize the invironmental pollution. 9,17% of waterwells and water tanks were hygienically treated; 11,434 people used clean water source; and 46,436 people washed their hands with soap. Studying knowledge, attitude, practice on preventive medicine programs of army medical officers showed very good results. Conclusion: The combination medical between army and pepople in preventive medicine, epidemic control gave wide efficiency for mountainous and coastal communes of Thua Thien Hue. It actively supported for Health station of 28 coastal communes and 13 mountainous communes in preventive medicine work.

Background: Child health care practices in the context of households and community is an important factor to improving the health of children. Some indexes were proposed by the World Health Organization to evaluate these practices. Objectives: To describe the knowledge and practice of pregnant women, under-5-year-old children\u2019s health care and assess the application of the survey toolkit in evaluating various indexes of healthcare practice at family and community levels. Subjects and method: By employing a structured interview, this cross-sectional survey has been conducted in Tu Liem district - Hanoi city and Tien Hai district \u2013 Thai Binh province. 120 mothers of under-5-year-old children participated in the survey. Results:98% of mothers had at least 3 antenatal visits during pregnancy. The rate of complete tetanus vaccination reached 43.3% in Tu Liem and 65% in Tien Hai. Prevalence of infant with low birth weight for age accounted for 8.3% in both districts. The percentage of children given complementary feeding at age of 6 to 9 months as the National Nutrition Program recommendations was only 30%. 65.7% and 68.5% of under-2-year-old children in Tu Liem and Tien Hai were breastfed within the first 30 minutes after delivery. There were a small proportion of mothers who gave complementary food either too early or too late, 8.3% and 8.4% in Tu Liem, 6.7% and 6.6% in Tien Hai, respectively. More than 90% of children were given normal feeding as usual during last their illness, but only 50% of children were given more fluid than usual. 98.2% of mothers knew 2 signs to immediately bring the child to health facilities. The most common injuries in under-5-year-old children were animal bites (62.5%) and falling (31.3%), however only 62.5% of mothers knew at least two measures of injury prevention. Conclusion: The toolkit for collecting data of practical index assessment can be applied at communes.
