1.Comparison of the curative effect of proximal femoral nail anti -rotation and dynamic hip screw in the treatment of intertrochanteric fracture

Objective To compare the clinical curative effect of proximal femoral nail anti rotation ( PFNA) and dynamic hip screw fixation in the treatment of the femoral intertrochanteric fracture,and provide reference for the development of clinical surgery scheme.Methods 74 patients with femoral intertrochanteric fracture in our hospital were selected and randomly divided into observation group and control group,37 cases in each group.The control group was treated with dynamic hip screw fixation,and the observation group was treated with PFNA therapy.Opera-tive time,bleeding volume and wound healing were compared between the two groups.The time of bone healing and complications were compared between the two groups after operation 1,3,6 months, functional recovery of hip was evaluated after operation by Harris score in the two groups.Results Intraoperative bleeding in the observation group was (115.68 ±72.38)mL,the operation time was (43.26 ±11.47)min,which were significantly less than the con-trol group(t=9.62,7.11,all P<0.05).Bone healing time of the observation group was (11.82 ±1.37) weeks, which was significantly less than the control group(t=3.46,P<0.05).The incidence rate of postoperative complica-tions of the observation group was 8.11%,which of the control group was 13.51%,the difference was not statistically significant (P>0.05).1 month after surgery,the Harris score of the observation group was (71.39 ±4.38)points, which was significantly higher than the control group (t=3.17,P<0.05).Postoperative 3,6 months,Harris hip score had no statistically significant difference between the two groups ( all P >0.05 ) .Conclusion PFNA had less surgical trauma,early functional exercise for patients after surgery by comparison with dynamic screw fixation.It could promote fracture and hip joint function rehabilitation.
2.Surgical modality of superior mesenteric artery syndrome: experience of 21 cases

Objective To explore the cause of and treatment for superior mesenteric artery syndrome (SMAS). Methods Clinical data of 21 patients from 1992 to 2002 with SMAS were analyzed retrospectively. Results Three cases of SMAS recovered with nonoperative treatments, eighteen recovered after surgical therapy including lysis and downward movement of the ligament of Treitz and extensive mobilization of the duodenum in 4 cases (Type Ⅰ), lysis and Roux-en-Y duodenojejunostomy in 9 cases (Type Ⅱ), side to side duodenojejunostomy in one (Type Ⅲ), and Billroth-Ⅱ gastrectomy in 2 cases (Type Ⅳ), and anterior side to side duodenojejunostomy or Roux-en-Y reconstruction in 2 cases (Type Ⅴ). Conclusion Correct diagnosis and appropriate surgery for SMAS lead to satisfactory outcomes.
3.The effects and the mechanisms of exogenous fatty acids on colorectal cancer cells

Objectives: To observe the effects and the mechanisms of exogenous fatty acids on colorectal cancer cells(LoVo). Methods:The effect of exogenous fatty acids on LoVo cells was determined by 5 diphenyl tetrazolium bromide(MTT) assay.The content of MDA was examined to evaluate the extent of lipid peroxidation. Results:The growth of LoVo cells was inhibited by polyunsaturated fatty acids(PUFAs).The inhibitory effect of saturated fatty acids and monounsaturated fatty acids on LoVo cells was not showed.None of the fatty acids was found to fibroblast cells(HLF).PUFAs caused the significant rising of intracellular MDA content. Conclusions:PUFAs could inhibit the growth of colorectal cancer cells.The strengthening of lipid peroxidation may be one of the mechanisms.
4.The change in metabolism of fatty acids in breast cancer cells and its significance

Objectives:To observe the changes in metabolism of fatty acids in breast cancer cells and the effects of inhibiting fatty acid synthase(FAS) on the growth of breast cancer cells. Methods:By RT PCR,the expression of FAS mRNA in breast cancer cell line was examined.The growth inhibition of breast cancer cells by specific FAS inhibitor, cerulenin, was determined by MTT assay.Flow cytometric analysis was used to study the changes of cell cycle. Results:FAS mRNA expression in MCF 7 cells was high.We found that cerulenin caused dose and time dependent inhibition of growth of MCF 7 cells.The growth inhibition of MCF 7 after 48 hours exposure of cerulenin at 2.5,5,10 and 20 mg/L was ( 43.47 ?4.58)%,(62.92?2.68)%,(81.93?0.91)% and (67.7?12.27)%( P
5.Microsurgical Treatment for Pituitary Adenoma via Single-nostril Transsphenoidal Approach

Objective: To explore the effect of single-nostril transsphenoidal approach on pituitary adenoma. Methods: We retrospectively analyzed 46 cases of pituitary tumors treated with single-nostril transsphenoidal approach and the effects and complications of surgery.Dunng the surgery,a nasal speculum was inserted through right nostril slowly towards the anterior wall of sphenoid sinus.A nasal mucosa incision of about 1.5cm was made in the right nasal cavity at the level of the middle nasal turbinate.With a fracture of the bony septum,a space was developed between the bilateral nasal mucosa and bony septum to the sphenoid sinus.Then,the face of the sphenoid sinus was exposed.The remainder of the bony septum,the anterior sphenoid sinus wall,and the sphenoid mucosa were removed.The antenor sphenoidotomy should be less than 1.5cm wide.After confirming the tumor by dural puncture,a cross incision of dura was made and the tumor was removed.The saddle was usually Collapsed and visible after total tumor removal.When the tumor was resected,sevaral gelatin sponges were stuffed into the Surgical cavity to stop bleeding. Results: Thirty-four cases had total resection and 12 cases had subtotal resection.No deaths or disability occurred.Hormone levels in almost all patients were improved.Seventeen cases had a sign of diabetes insipidus.Electrolyte disturbance occuwed in 5 cases.NO postoperative cerebrospinal fluid rhinorrhea was observed. Conclusion: Single-nostril transsphenoidal approach has many advantages in treating pituitary adenomas such as simplified approach,brief technology and high security.
6.Selection Pressure on Haemagglutinin Genes of H9N2 Influenza Viruses from Different Hosts

Positive selection and differential selective pressure analyses were carried out to study Haemagglutinin (HA) genes of H9N2 influenza viruses from different hosts in this paper. Results showed that, although most positions in HAs were under neutral or purifying evolution, a few positions located in the antigenic regions and receptor binding sites were subject to positive selection and some of them were even positively selected at the population level. In addition, there were always some positions differentially selected for viruses from different hosts. Both selection pressure working on HA codons and positions differentially selected might account for the extension of the host range and adaptations to different hosts of H9N2 influenza viruses.
7.Positive Selection Analysis of VP1 Genes of Worldwide Human Enterovirus 71 Viruses

Human enterovirus 71 viruses have been long circulating throughout the world. In this study, we performed a positive selection analysis of the VP1 genes of capsid proteins from Enterovirus 71 viruses. Our results showed that although most sites were under negative or neutral evolution, four positions of the VP1 genes were under positive selection pressure. This might account for the spread and frequent outbreaks of the viruses and the enhanced neurovirulence. In particular, position 98 might be involved in neutralizing antibodies, modulating the virus-receptor interaction and enhancing the virulence of the viruses. Moreover, both positions 145 and 241 might correlate to determine the receptor specificity. However, these positions did not display much difference in amino acid polymorphism. In addition, no position in the VP1 genes of viruses isolated from China was under positive selection.

9.Effects of MRI in the diagnosis and prognosis of cervical spinal cord injury without fracture or dislocation

BACKGROUND: In the elderly, the cervical vertebrae will appear with different degrees of spinal stenosis caused by various factors. Notably, the incidence of cervical spinal cord injury without fracture or dislocation is high, accounting for 3%-16% of cervical spinal cord injury.OBJECTIVE: To explore the efficacy of MRI in the diagnosis and prognosis of cervical spinal cord injury without fracture or dislocation, and to analyze the influencing factors for surgical effectiveness.METHODS: Fifty-one patients with cervical spinal cord injury without fracture or dislocation in Zibo Central Hospital from August 2014 to June 2016 were analyzed retrospectively. All the patients underwent anterior, posteror, or anterior-posterior fusion, respectively, and then the intraoperative ligament injury and MRI images were compared. The age, injury type, spinal cord injury type revealed on MRI, time from injury to hospital, signal length of spinal cord injury,American Spinal injury Association grade, degree of cervical stenosis, whether hormone used or not within 8 hours after injury, time from injury to surgery, and surgical approaches were analyzed based on univariate analysis and multivariate analysis by Logistic regression analysis.RESULTS AND CONCLUSION: (1) MRI showed more comprehensive damage of the spinal cord and intervertebral disc ligament complex than the intraoperative findings (χ2=5.966,P < 0.05), and the cervical spinal cord injury was related to the injury of the corresponding intervertebral disc ligament complex (χ2=1.53, P > 0.05). (2) Univariate analysis results showed that the spinal cord injury type revealed on MRI, signal length of spinal cord injury, American Spinal injury Association grade, degrees of cervical stenosis, whether hormone used or not within 8 hours after injury, and time from injury to surgery were related to the functional recovery of spinal cord (P < 0.01). (3) Multivariate analysis results indicated that the type revealed on MRI, signal length of spinal cord injury and degrees of cervical stenosis were significantly related to the functional recovery of spinal cord (P < 0.05). (4) These results suggest that MRI has a high tissue resolution, so it plays an important role in the diagnosis of cervical spinal cord injury without fracture or dislocation.Moreover, the degrees of spinal cord injury and spinal stenosis, as well as operation time are main factors for curative efficacy.
10.Application of T grain technique to the diagnosis of lung disease and analysis of its image quality.

OBJECTIVETo compare the image quality of T grain green sensitive film (TML-1) and Lanex Gd(2)O(2)S rare earth intensifying screen with that of XK-1 blue sensitive film and calcium tungstate (CaWO(4)) intensifying screen, and to study the application of T grain technic to the diagnosis of lung diseases.
METHODS160 coal miners were randomly selected to take both TML-1 and XK-1 chest film of high kV radiographs at the same time. Silver halide granule, fluorescence of intensifying screen, radiographic parameters, the density at different points in the lung and chest radiographs were observed.
RESULTSSilver grains in TML-1 film were more homogeneous in distribution than in XK-1 film. Luminous intensity of Lanex Gd(2)O(2)S rare earth intensifying screen was brighter than CaWO(4) intensifying screen in the same exposure. The exposure doses of TML-1 film was reduced to one third of XK-1 film. The density of chest radiographs was 0.24 to 2.74 in TML-1 film, and 0.30 to 2.60 in XK-1 film. There were greater exposure latitude and more informations in TML-1 film. By apertured-disc observation, the fine structure of lung in TML-1 film was clearer than in XK-1 film, the shape was more concrete and reliable, visualizability was stronger.
CONCLUSIONT grain technique may obviously improve the clearness and resolution of image, and enhance the transmission of information, as well as increase the diagnostic informations.
