PURPOSE: To report a case of episcleral cartilaginous single tissue choristoma attached to pterygium with literature review. METHODS: A-65-year old woman had a pterygium. we found hard and movable mass attached to pterygium over medial rectus muscle when we excised pterygium. Excisional biopsy was done. RESULTS: This mass was confirmed as episcleral cartilaginous single tissue choristoma.

4.US findings of thyroid carcinomas developed in multinodular goiters.

Primary role of ultrasound in patients with thyroid nodule is to determine the multiplicity and detect occult carcinoma. We analyzed US findings of 53 thyroid carcinomas with multiple nodular lesions from january 1988 to december 1991. The results were as follows: 1. 109 malignant nodules in 53 cases and 24 benign in 23 were comfirmed. 2. The nature of the masses were solid in 72 malignant nodules (74.2%), and complex in 25 (25.8%) of which 19 were predominantly solid. 3. The echo pattern of the solid and solid predominant masses were hypoechoic in 78 malignant nodules (80.4%), hyperechoic in 10 (10.3%), and isoechoic in 3 (9.3%) 4. Internal punctate calcifications within the masses were observed in 31 malignant nodules (31.9%). 5. Halo sign was present in 16 malignant nodules(16.5%). 6. The preoperative sonographic diagnosis of thyroid carcinoma was made in 39.4% of 53 cases with multiple nodules on US. In conclusion, the number, and halo formation of nodules were insignificant to differentiate the benign nodule from the malignant. The heterogeneous hypoechoic nodules with size greater than 4cm, multiple stippled calcifications, displacement of the trachea or other surrounding structures of combined lymph node enlargements seem to indicate the possibility of malignancy.

5.A Case of Cutis Marmorata Telangiectatica Congenita.

Cutis marmorata telangiectatica congenita(CMTC) is a rare congenital cutaneous vascular anomaly. The major skin findings are persistent, fixed cutis marmorata, telangiectasia, and phlebectasia. In approximately 50 % of patients, the disorder tends to be associated with various con-genital anomalies. We describe in this report a 4-month-old male infant with CMTC. The patient had red or violet marbled patches, telangiectases, and atrophy on the right arm present at birth. During the follow up period of 12 months, there was no change in the cutis marmorata pattern and telangiectases, whereas the atrophy has been less prominent than at the initial visit.

6.A Case of Subcutaneous fat Necrosis Associated with Pancreatitis.

We report a case of subcutaneous fat necrosis associated with pancreatitis that may be the first case in Korean hterature as for we know. The patient was a 41-year-old man wbo presented erythema nodesum-like erythematous subcufaneous nodules on both lower extremities. Histopathologic findings showed subcutaneoms focal fat necrosis and ghoat-like cells. Granular basophilic material was deposited in and around the necrotic fat cells and stained positively with von Kossa stain. The lesioas subsided gradwdly without any specific treatment, as the underlying pancreatitis was ameliorated.

7.Flow Cytometric Analysis of DNA patterns in Carcinoma of the Uterine Cervix.

OBJECTIVE: Flow cytometric deoxyribonucleic acid (DNA) content analysis has been shown to be of prognostic importance in some cancers. In this study, DNA patterns obtained by flow cytometry in patients with cervical cancer have been shown to be related to a prognostic importance in cervical carcinoma. METHODS AND MATERIALS: We retrospectively reviewed 59 cervical cancer patients who admitted to the department of obstetrics and gynecology, Samsung Medical Center firom January 1995 to May 1997. RESULTS: A total of 59 archival paraffin-embedded blocks were proc 42.4 percent were found to be DNA content aneuploid (DNA-A) and 57.6 percent DNA content diploid (DNA-D). The S-phase fraction was significantly increased with stage. The incidences of aneuploid and relative DNA index were not significantly increased with the stage(P=0.280) and age of patients(P=0.088). The results had no significant differences between the squamous cell carcinoma and adenocarcinoma, and no relationship between the groups with keratinizing cell type and nonkeratinizing type(P=0.409). The DNA ploidy patterns were not significantly correlated with metastasis to lymph node(P=0.179), tumor size(P=0.109), invasion depth(P=0.361) and recurrent group(P=0.285). High S-phase fraction had a tendency to correlate with the invasion depth(P=0,011), size(P=0.027), lymph node metastasis(P=0.039) and recurence(P=0.099) of cervical cancer. CONCLUSION: Flow cytometric DNA content analysis is being used more frequently in the management of different malignant tumors. Our study shows that DNA content analysis may not serve as an important biological indicator in determining the prognosis in cervical carcinoma, but High S-phase fraction has been connected with more aggressive tumors.

8.Effects of Ultra-Marathon Running on Pro-inflammatory, Anti-inflammatory and Endothelial Function Factors in Athletes with Exercise-induced Hypertension.

Running for a long distance in runners with exercise-induced hypertension (EIH) increases high blood pressure (BP) which causes myocardial burden, inducing elevation of cardiac markers. It is assumed to be associated with endothelial dysfunction, but has not been studied so far. The purpose of this study is to exam the responses of anti-inflammatory, inflammatory, and endothelial regulation factors to 100 km ultra-marathon of athletes who have EIH. For this study, we obtain and analysis interleukin-6, tumor necrosis factor-α, interleukin-10 (IL-10), monocyte chemoattractant protein-1, creatine kinase and nitric oxide (NO) from 100 km ultra-marathoners before and right after the 100 km race. There were 40 volunteers who had ever won the 100 km marathon race before more than one time. They had the graded exercise test with measurement of BP for assessing EIH. Among them, eight athletes were categorized to normal control group (NCG) and 10 athletes were assessed by EIH group (EIHG). The graded exercise test with treadmill was performed for the maximal strenuous exercise and BP. EIH was diagnosed as <140/90 mm Hg in normal and ≥210 mm Hg in maximal exercise. Marathoners including EIHG (n=10) and NCG (n=8) participated in the race and blood extracted from them before the race and right after the race. As results, EIHG had lower IL-10 than NCG during resting states and after the 100 km race EIHG had significantly lower NO concentration than NCG. Therefore, the NO in the endothelial is inhibited during strenuous endurance exercise, resulting in vasoconstriction with increasing the BP.

10.Secondary Adrenal Insufficiency Initially Misdiagnosed as Depression: A Case Report.

The abnormalities in Hypothalamic-pituitary-adrenal(HPA) axis are associated with many psychiatric symptoms including depression. We present a report of a 71 year old man who was admitted to the psychiatric department presenting symptoms of headache, avolition, loss of energy, psychomotor retardation, poor appetite, insomnia, anxiety resulting from adrenal insufficiency and hypopituitarism. Hypothyroidism and electrolyte disturbance were managed and headache, insomnia, anxiety, GI symptoms were improved. But he remained in anergic state. After discharge, he was readmitted to infection department with high fever and drowsy mentality. Adrenal insufficiency was recognized and he was treated with corticosteroid replacement therapy. Finally his diagnosis was made as panhypopituitarism and overall symptoms were resolved. In this case, we showed how the atypical symptoms resulting from hypopituitarism develop and progress. Hypothyroidism, adrenal insufficiency, and growth hormone deficiency resulting secondarily from panhypopituitarism were associated with various nonspecific symptoms such as loss of energy, fatigue, insomnia, weight loss, decreased appetite etc. In clinical situation, differential diagnosis with depression is needed when clinicians were met a patient with these nonspecific symptoms. It is important that laboratory tests and differential diagnosis with endocrine diseases should be conducted, especially in geriatric patients with nonspecific symptoms like anergia, fatigue, poor appetite and so on.
