1.A Cases of Endodermal Sinus Tumor in Sacrococcygeal Region

A endodermal sinus tumer (Teilum) is a highly malignant germ cell tumor showing a selective overgrowth of yalk sac endoderm intimately associated with the extraembryonic mesoblast. Several cases of the tumor originating from the ovaries have been reported in the literature. We present a case of the tumor with a rare origin from the sacrococcygeal region.

2.The Result of the Tension Band Internal Fixation

The principle of the tension band technique is to counteract the tensile forces acting across the fracture site and to convert them into compressive forces. We have experienced 37 fractures treated with tension band technique from 1975 to 1978 and the results are as followings; 1. 37 Cases of fracture were treated by tension band technique. Eight cases out of 37 were olecranon fractures. 16 cases were patellar fractures. 10 cases were malleclar fractures. and three cases were trochanteric fractures of the femur. 2. As post-operative immobilization, applied cast splint in almost of the cases except application of hip spica cast in trochanteric fracture. 3. The average duration of immobilization in each cases, 12 days in olecranon, two weeks in patella, four weeks in malleolus and six weeks in greater trochanter respectively. 4. The radiological union was obtained in seven weeks in olecranon, six weeks in patella, eight weeks in malleolus and six weeks in greater trochanter. 5. As complication, one case of nonunion in olecranon, one case of infection and, one case of wire breakage in patella and one case of traumatic arthritis in malleolus were noted, which needed further surgery. 6. In conclusion, tension band wiring is considered as a good method for the fractures involving joing, olecranon, patella, and malleolus because the technique has advantages such as rigid internal fixation, shortening the duration of immobilization and fracture healing and early mobilization of the neighboring joints.

3.Weighting the items of Korean Activities of Daily Living(K-ADL) Scale and its Validity.

BACKGROUNDS: Though K-ADL was developed and validated, weighting each item of K-ADL is needed to evaluate the severity of functional disability as a whole. METHODS: Nominal group technique was used to weight individual items of K-ADL. Relevant experts were recruited; one specialist in geriatrics, one expert in elderly health service, one doctor expert in questionnarie, one nurse and two social welfare workers and one caregiver who work in nursing home for elderly, one nurse working at dementia care center. At the first round, each expert assigned the weights of each item. At the second round, those experts met, discussed, and re-evaluated each weighting. Using the newly developed weighted score, we measured its validity compared to brain-disability grade and three experts' decision of severe dysfunction. RESULTS: For inability to perform task, 7 points were given to 'dressing'and 'washing face', 6 points to 'bathing', 9 points to 'feeding', 8 points to 'transfer and toileting'. If any assistance is needed, 7 points were scored to 'dressing', 4 points were scored to 'washing face', 'feeding', 'transfer' and 'toileting', 2 points were scored to 'dressing'. Correlation coefficient between weighted K-ADL sum and brain-disability grade was -0.665(p=0.000). CONCLUSIONS: We assigned differential weighting to each of K-ADL and found that summing weighted K-ADL was excellent in some validity test.

4.Clinical significance of Lumbar Myelography

A series of 84 cases operated upon for herniated intervertebral disc of lumbar region was studied with respect to the diagnostic value of myelographic examination. Various clinical signs including level of tenderness, sensory impairment, motor weakness, door-bell sign and Lasegue test, were reviewed. The clinical diagnosis derived from these signs was correct in 54.8%. The plain X-ray film showed narrowing of disc space in 44 cases. As to the myelographic findings, mode and level of indentation, positive and negative findings were analyzed and comparisons were made with operative findings. Among 84 cases, 47 cases showed lateral indentation, 32 cases central and 5 cases complete block. As to the level, L4~5 disc space was dominant. As a whole, the myelographic findings accurately corresponded with the operative findings in 77.4%, 65 cases. However, diagnostic accuracy was varied according to level, i.e., in case of L4~5 disc herniation, 45 cases among 53 cases showed correct myelographic diagnosis, while only 10 cases among 17 cases showed correct myelographic diagnosis in L5~S1 disc herniation. In addition, if electomyography and discography are added as diagnostic aid, the preoperative diagnostic accuracy will be higher but those are not available in our circumstance.

5.Discrepancies in Myeolography

Among 157 cases operated upon, under the diagnosis of herniated intervertebral disc, we experienced 34 cases of discrepancies between myelographic and operative findings. We analysed these cases in the respect of patterns, causes and clinical significance of these discrepancies and the results were as follow; 1. Thirty four cases (21.7%) among 157 cases operated upon showed discrepancies between myelographic and operative findings. 2. These 34 cases consisted of 17 cases of level discrepancies, 9 cases of false positive and 8 cases of false negative. 3. The main cases of level discrepancies was indentation due to bony spur with/without adhesion. 4. The main cause of false positive was degenerative changes of the spine. In the detection of this false positive, plain X-ray of lumbosacral spine give important clue. In false positive, explorative paitial laminectomy and removal of bony spur or adhesiolysis was indicated because the effect of the spur or adhesion was same as that of herniated disc. 5. The causes of false negative were giant canal, congenitally narrow cul de sac, high cul de sac and far laterally seated disc herniation. In false negative, plain X-ray of lumbosacral spine afforded little diagnostic aid. In case of being compatible to disc herniation at a certain level through clinical signs, normal myelographic finding has less meanings and exploration is to be performed according to the clinical signs.

6.A Case of Acquired Syphilitic Osteomyelitis of the Tibia

Acquired syphilis rarely manifest itself in the skeleton, causing localised osteoscopic pain, commonly in the abscence of general systemic complaints. Diagnosis of bone syphilis can easily be made by clinical history, course of the disease, roentgeno-graphic findings, serological tests, therapeutic test of antiluetic treatment and, most reliably, by biopsy. We experienced a patient with syphilitic osteomyelitis of tbe tibia who had a history of sexual contact and the diagnosis was confirmed by serological tests and biopsy.

7.A Case of Gastritis Cystica Polyposa, Presenting as a Submucosal Tumor - like Lesion.

Gastritis cystica polyposa (GCP) is a lesion characterized by all the histological features described for hyperplastic polyps, And there is s marked proliferation of muscular elements and entrapment of numerous epithelial cysts. These lesions have been described at gastroenterostomy stomas, at peptic ulcer edges, and in association with carcinoma. It is rare that GCP presenting as a submucosal tumar-like lesion develops in the absence of above mentioned associated conditions. We had experienced a 69 year-old man with GCP. Barium X-ray and endoacopic findings showed submucosal tumor in antrum, and endoscopic ultrasonograpic findings showed markedly thickened mucosal layer with scattered hypoechoeic areas and the submucosal layer well preserved. GCP was confirmed by histological examination of resected stomach.

9.A Clinical Study on the Cardiac Toxicity of Adriamycin.

To clarify the exact role of endomyocardial biopsy in the diagnosis and monitoring of adriamycin-induced cardiotoxicity and to observe the actual relationship between pathologic changes and cardiac dysfunction, a cross-sectional clinical study was conducted. Echocardiography was used to evaluate cardiac dysfunction in 18 patients who had received chemotherapy including adriamycin(mean dose : 410mg/m2 of B.S.A.) without clinical evidence of congestive heart failure, and in 19 normal controls. Six patients receiving adriamycin underwent 7 transfemoral endomyocardial biopsy procedures, and the specimens were evaluated by light and electron microscopy for evidence of drug-related cardiotoxicity. Indexes of cardiac systolic function obtained by M-mode echocardiography(left ventricular dimension, excursion of interventricular septum and left ventricular posterior wall, shortening fraction and ejection fracton) did not show any statistically significant difference between patients who received adriamycin and normal controls. In transmitral flow-velocity curves recorded by Doppler echocardiography with a 2.25MHz probe, the patients showed less E peak velocity and decreased E/A ratio compared with normal controls, which suggests left ventricular diastolic dysfunction in the patients who received adriamycin. All the specimens of the endomyocardial biopsy showed significant pathologic changes of adriamycin indnced cardiotoxicity which was characterized by myofibrillar loss and vacuolization of the cytoplasm. In 2 specimens, pathologic grade was II, while 5 specimens showed pathologic changes of grade III and further chemotherapy with adriamycine was not done in thse 5 cases. From these results it is suggested that pathologic changes precede the clinical onset of congestive cardiomyopathy in the patients receiving adriamycin and left ventricular diastolic dysfunction occurrs before ejection fraction falls to subnormal levels. We conclude that sequential endomyocardial biopsy is absolutely indicated for exact diagnosis and monitoring of adrinamycin-induced cardiotoxicity to prevent the development of irreversible and often fatal cardiomyopathy.

10.Autophagic failure promotes the exocytosis and intercellular transfer of alpha-synuclein.

The accumulation of abnormal protein aggregates is a major characteristic of many neurodegenerative disorders, including Parkinson's disease (PD). The intracytoplasmic deposition of alpha-synuclein aggregates and Lewy bodies, often found in PD and other alpha-synucleinopathies, is thought to be linked to inefficient cellular clearance mechanisms, such as the proteasome and autophagy/lysosome pathways. The accumulation of alpha-synuclein aggregates in neuronal cytoplasm causes numerous autonomous changes in neurons. However, it can also affect the neighboring cells through transcellular transmission of the aggregates. Indeed, a progressive spreading of Lewy pathology among brain regions has been hypothesized from autopsy studies. We tested whether inhibition of the autophagy/lysosome pathway in alpha-synuclein-expressing cells would increase the secretion of alpha-synuclein, subsequently affecting the alpha-synuclein deposition in and viability of neighboring cells. Our results demonstrated that autophagic inhibition, via both pharmacological and genetic methods, led to increased exocytosis of alpha-synuclein. In a mixed culture of alpha-synuclein-expressing donor cells with recipient cells, autophagic inhibition resulted in elevated transcellular alpha-synuclein transmission. This increase in protein transmission coincided with elevated apoptotic cell death in the recipient cells. These results suggest that the inefficient clearance of alpha-synuclein aggregates, which can be caused by reduced autophagic activity, leads to elevated alpha-synuclein exocytosis, thereby promoting alpha-synuclein deposition and cell death in neighboring neurons. This finding provides a potential link between autophagic dysfunction and the progressive spread of Lewy pathology.
