1.Two Cases of Hyperinsulinemic Hypoglycemia.

Hyperinsulinemic hypoglycemia is a relatively rare disease in childhood period except neonate, but hypoglycemia due to delicate imbalance between glucose production & consumption is evoked easily and left permanent damage to brain at these period. The definition is that serum insulin level is above 10microU/ml when blood sugar level is below the 40mg/dl and so I/G ratio is higher than 0.4. The clinical manifestations are irrtability, frequent feeding and seizures etc. and there is no specific pancreatic pathology in most cases. We experienced 2 cases of hyperinsulinemic hypoglycemia with pancreatic hyperplasia and pancreatic adenoma each other. The diagnosis was made on clinical manifestations, laboratory results, radiologic and pathologic findings. We reported these cases with brief review of literature.

2.Comparison of the depression and anxiety between the elderly in the home for the aged and those in the community.

This survey was conducted from January 1991 to May 1991. Two hundred and eight residents in 4 government supported homes for the aged and hundred and eleven living in the community in Taegu, Korea, were examined to evaluate the depression and the anxiety of the aged with combined anxiety and depression scale (CADS) and somatization symptom check list (SCL). There are no significant differences between residents in the home for the aged and those living in the community on the total scores of CADS and SCL. However, in the case of the total score of CADS of the female subjects in the home for the aged were significantly higher than those of the community residents. The elderly in the home for the aged tended to have pure depression, while community residents were likely to have anxiety and depression. Fifty-two subjects of home for the aged and sixty-nine of the community scored over 50 points of CADS, which indicates considerable depression or anxiety. In psychosocial factors, the subjects who in the following situations had statistically significant higher scores than others. The results were as follow. Poor health, unhappiness, unsatisfaction to the past occupation, pessimistic thought in future view for both group and unsatisfaction of the relationship with familiar people for the elderly in the community. The SCL scores of two groups subgrouped by under 49 and 50 on CADS showed significant differences between each subgroup on all of the SCL items. It could be suggested that somatic symptoms for the aged is a sign of depression.

4.Solitary Mastocytoma on the Scalp.

Nine-month-old female infant was seen with a 7-month history of a nodule on the right temporal scalp, which had gradually increased in size. Stroking of the lesion resulted in urtication and blistering and there were no other cutaneous lesions. The histology showed subepidermal bulla formation and a dense infiltration of mast cells in the papillary and reticular dermis. We present an infant with solitary mastocytoma on the scalp, a rare site.

5.A Clinical Study of the Ankle Fractures

A clinical analysis was done on 129 patients (131 cases) of the ankle fractures that were admitted and treated in Orthopedic Department, Inchon Christian Hospital from January 1973 to December 1980. The following results were obtained: l. Of the 129 patients, male was 104 patients and female 25 patients, the average age of the patients was 37. 2. The main cause of the injuries was traffic accident (38.2%), and the remain causes were industrial accident (28.2%), fall down (18.3%), slip down (11.5%), and sports injury (3.8%) in order. 3. The most common type by Lauge-Hansen classification was supination-external rotation type (37.4%). 4. In method of treatment, operative treatment was done in 72 cases and nonoperative in 59 cases. 5 The average duration of the cast immobilization in nonoperative treatment was 8 weeks and operative 7 weeks. 6. The better result was obtained by operative treatment than nonoperative one. 7. The most poor result was noticed in pronation-external rotation type according to Lauge-Hansen classification. 8. We agree that the accurate reduction of the lateral malleolus make the talus in good anatomic position in ankle joint. 9. We agree that Lauge-Hansen classification is useful in diagnosis and treatment of ankle fracture.

10.Causative Organisms of Urinary Tract Infection in Children, and their Antibiotic Susceptibility.

PURPOSE: In caring patients with urinary tract infection(UTI), it is prominent to know causative agents and their antimicrobial susceptibilities, of which domestic data appear somewhat different from those reported in the literature. The study was done to learn the distribution of pathogens causing UTI and their antimicrobial susceptibilities in Korean children. METHODS: One hundred forty two inpatients diagnosed with symptomatic UTI in the Department of Pediatrics, Hanyang University Hospital from 1997 to 2002 were retrospectively enrolled to analyze relevant information obtained from medical records, including causative agents of UTI and their antimicrobial susceptibilities. RESULTS: The average age of the study patients was 1.9 years with the peak in infants younger than 1 year of age(105 patients), of which 84 patients were younger than 6 months of age. The male to female ratio was 4:1 in <1 year of age and 1:2.1 in > or =1 year of age. Isolated organisms in urine cultures included Escherichia coli 83.1%, Klebsiella pneumoniae 6.3%, Proteus mirabilis 2.1%, and their distributions were not different, whether suprapubic specimen(100 patients) or non-suprapubic specimen(42 patients) was cultured. Antimicrobial susceptibility of E. coli was above 90% against ceftriaxone, ceftazidime, amikacin, 40-65% against trimethoprim-sulfamethoxasole, tobramycin, gentamicin, and 16.9% against ampicillin. CONCLUSION: E. coli was isolated in 83.1% of children with UTI, which is comparable to the data in the literature. The fact that susceptibility of E. coli was low to ampicillin, gentamicin, and trimethoprim-sulfamethoxasole indicates that the empiric and prophylactic choices of antimicrobials for UTI in Korean children should be investigated further.
