1.Neurological Presentation of Decompression Sickness Among Koran Civilian Divers.

We retrospectively evaluated 47 patients with decompression sickness (DCS) seen at the Kang Nam General Hospital between January 1987 and June 1989 to describe the characteristcs of neurological manifestations. In 25 patients (53.2%) girdle paresthesia or bandlike tightness was noted just before the onset of neurological symptoms or signs. Progressive limb weakness, urinary retention, alterations in mentality, severe paresthesia were main presenting features in order of decreasing frequency. In the spinal from of DCS lower thoracic segment was most commonly involved. On the contrary to the previous pathological reports posterior columns were relatively spared in view of clinical points. Of 7 patients tested with Somatosensory Evoked Potetial (SSEP), five showed abnormal fidings. Myoclonic jerks of lower limb were the most common late sequelae.

2.Reliability and Validity of Korean Brief Comprehensive Geriatric Assessment Questionnaire.

Background: The purpose of this study was to evaluate the applicability of the Korean Brief Comprehensive Geriatric Assessment Questionnaire through testing the reliability and validity of it. Methods: Questionnaire consisted of items on medical, functional, mental, socioeconomic status and lifestyle was administered as test-retest on 78 elderly outpatients at 6 university hospitals in Seoul, Korea. Weighted kappa value, kappa value, Pearson correlation coefficients and overall percent agreement were calculated to measure reliability. Validity coefficient was estimated by using reliability coefficient. Results: Reliability coefficients and validity coefficients were higher than 0.50 (fair or good) for 28 items (87.5%) among 32. For test- retest reliability, median was 1.00 (range, 0.87-1.00) for overall percent agreement, 0.66 (range, 0.27~1.00) for kappa value, and 0.83 (range, 0.65~1.00) for Pearson's correlation coefficients. Differences of reliability and validity were identified by gender for 19 items (59.4%), and by age group for 12 items (37.5%). Conclusion: The Korean Brief Comprehensive Geriatric Assessment Questionnaire appears to be a reliable and valid screening tool. After additional study is performed, this instrument can be used as a useful tool for comprehensive outpatient geriatric assessment.

PURPOSE: The purpose of this study is to evaluate the psychometric properties of the Korean version of the King's Health Questionnaire(KHQ) in patients with stress urinary incontinence. MATERIALS AND METHODS: Multicenter prospective study was undertaken in 106 patients with stress urinary incontince. Psychometric properties including discriminant validity, convergent validity were evaluated and the Cronbach's alpha coefficients were calculated. Test-retest analysis was performed and the sensitivity to clinical change before and after treatment was also evaluated. RESULTS: The psychometric properties and clinical validity of the KHQ Korean version were confirmed in 106 study population. The KHQ's good reliability was evidenced by Cronbach's alpha coefficients of>0.60, indicating to change in patients' perception of bladder condition in all domains. CONCLUSION: Our analyses confirm excellent psychometric properties for the Korean version of KHQ, which appears to provide a valid and reliable instrument for clinical usages in Korea.

4.Analysis of the Risk Factors That Affect Esophageal Transit Delay When Performing Wireless Capsule Endoscopy.

BACKGROUND/AIMS: Capsule endoscopy has become an excellent diagnostic tool for various small bowel diseases. However, some cases of delayed passage of the capsule in the esophagus without obstruction have been reported. The aims of this study were to analyze the risk factors associated with esophageal transit delay. METHODS: From Nov. 2002 to July. 2008, 141 patients underwent capsule endoscopy. Among them, 3 patients were excluded. The 138 patients were divided into two groups (the delayed esophageal transit time (DETT) group, and the normal esophageal transit time (NETT) group), and we compared their characteristics, including age, gender, the reason for examination, the total transit time and the rate of an incomplete examination. RESULTS: DETT occurred in 7 patients (5.1%). The mean age (61.14+/-0.70 vs. 44.01+/-7.37, respectively, p=0.02) was higher in the DETT groups. No statistically increased risk was found for gender and the indications for the procedure. The DETT group showed a higher rate of incomplete examination than did the NETT group (7/7 vs. 41/131, respectively, p=0.001). CONCLUSIONS: Even though delayed esophageal transit on capsule endoscopy is not a serious complication, it could lead to an incomplete examination. Therefore, checking the chest X-rays after swallowing the capsule can be helpful to notice delayed esophageal transit earlier in the procedure.
