2.Diagnostic Values of Segmental Somatosensory Evoked Potentials(SEPs) in the Unilevel/Unilateral Lumbosacral Radiculopathy.

OBJECTIVE: To evaluate the diagnostic values of segmental somatosensory evoked potentials (SEPs) in the unilevel/unilateral lumbosacral radiculopathies. METHOD: Thirty-nine radiculopathy patients of whom diagnosis was confirmed by the surgery or selective nerve root injection and 20 subjects with no evidence of radiculopathy were included in the study. Before the treatment, superficial peroneal nerve segmental SEPs and sural nerve segmental SEPs were performed. Sensitivities and specificities were delineated from the several diagnostic criteria. RESULTS: The analysis of side-to-side mean cortical P1 latency difference (>2.0 SD) reveals a segmental SEPs sensitivity for L5 radiculopathies of 12.1% and sensitivity for S1 radiculopathies of 0%. The analysis of side-to-side mean percent amplitude difference (2.0 SD) reveals a segmental SEPs sensitivity for L5 radiculopathies of 9.1% and sensitivity for S1 radiculopathies of 0%. The analysis of side-to-side mean cortical P1 latency difference (>3 ms) and the side-to-side mean amplitude reduction (>50%) show the similar results. The specificities of L5 radiculopathies and S1 radiculopathies are variable from 40.0% to 93.3%. Sensitivities of segmental SEPs are lower than needle EMG. It is not root specific, even the SEP is not obtainable unilaterally. CONCLUSION: The diagnostic values of segmental somatosensory SEPs are questionable in the unilevel/unilateral lumbosacral radiculopathies which are confirmed by the surgery or selective nerve root injection.

Nine-month-old female infant was seen with a 7-month history of a nodule on the right temporal scalp, which had gradually increased in size. Stroking of the lesion resulted in urtication and blistering and there were no other cutaneous lesions. The histology showed subepidermal bulla formation and a dense infiltration of mast cells in the papillary and reticular dermis. We present an infant with solitary mastocytoma on the scalp, a rare site.

6.Spontaneous Lumbar Artery Bleeding and Retroperitoneal Hematoma in a Patient Treated with Continuous Renal Replacement Therapy.

Rupture of the lumbar artery is usually associated with trauma but rarely has been reported in association with anticoagulation. We present a 71-year-old man who developed spontaneous rupture of the lumbar artery leading to a retroperitoneal hematoma while receiving continuous renal replacement therapy (CRRT). The bleeding was confirmed by computed tomography and angiography and was controlled successfully using selective angiographic embolization. We suggest that spontaneous retroperitoneal bleeding should be considered in a case of sudden decrease in hemoglobin in a CRRT patient.

7.A Case of Ossification in the Phthisis Bulbi.

Phthisis bulbi, resulting from repeated ocular inflammation and infection or ocular trauma, causes various degeneration of ocular tissue as well as changes in ocular external shape. It shows the impression of scleral thickening with calcification of ocular tissue and an unusual ossification as well. Having observed 31-year-old patients of phthisis bulbi and band keratopathy caused by repeated operation after ocular trauma years ago, we extracted the tan brown colored shell-like firm tissue in posterior segment and report sclerotic impression of some of ocular tissue and histopathologic finding of the typical ossificationof ocular tissue during the course evisceration.

8.Six Cases of Congenital Absence of the Vagina Managed by McIndoe Operation.

Mullerian agenesis or dysgenesis, rare developmental anomaly presenting as primary amenorrhea, is characterized by absence of vagina and absence or rudimentary development of the uterus. Anatomic anomalies of the axial skeleton and kindney often accompany this psychologically devatating condition. There are various methods of surgical treatment for congenital absence of the vagina. The six cases reviewed in this paper were all treated with McIndoe technique. All of 6 women experienced satisfactory sexual activity and were satisfied with vaginal depth following vaginal creation using a split thickness skin graft technique. Despite the existence of several alternative methods, the McIndoe technique is a relatively simple and highly successful procedure and hence is still prefered by many surgeons.

10.The Effect of Retinoic Acid on Experimentally-Induced Proliferative Vitreoretinopathy in the Rabbit.

The proliferative vitreoretinopathy was a complication followed by operation of rhegmatogenous retinal detachment. It was the mot, comon cause of a failure of retinal detachment surgery. It was characterized by the growth of cellular fibrous membrance in detached both retinal side and vitreous and also developed by giant retinal dialysis, posterior segmental trauma, excessive cryotherapy, endophthalmitis, retianl vascular disease. In order to prevent and treat of proliferative vitreoretinopathy, various methods of operation and drugs have been researched. We executed the experiment using the rabbit to observe the effect of retinoic acid that is known by inhibition of migration and proliferation of retinal pigment epithelial cell and fibroblast in vitro. With 121 eyes of rabbit, we induced the proliferative vitreoretinopathy by injecting of human retinal pigment epithelial cell, human fibroblast, and rabbit fibroblast into eyeball of rabbits. The extent of proliferative vitreoretinopathy was compaired by injecting those cells with the group that was injected by retinoic acid and control. The result was that in those cell groups, the extent of proliferative vitreoretinopathy was significantly higher in the rabbit fibroblast group than the other two groups(P<0.05). And in the groups that were injected retinoic acid, when subconjuctivaly injected(0.3mg/0.3ml), proliferative vitreoretinopathy was effectively suppressed and when intravitrealy injected (0.05mg/0.1ml), vitreoretinopathy was more increased than the control group. This result was probably caused by high concentration of retinoic acid in vitreous and further evaluation with various concentration of retinoic acid is needed. In conclusion, we recommend a rabbit fibroblast and subconjunctival injection of retinoic acid for the study on the suppressive effect of proliferative vitreoretinopathy.
