With development of immunochemical methods for anlysis of serum proteins, quantitative determination of serum immunoglobulin levels and purification of them have been made possible in recent years. There are numerous reports about immunoglobulin levels in various diseases. A search of the literature reveals very few reports of quantitative immunoglobulin determinations in allergic dermatoses. The present study was conducted to find quantitative changes of serum immunoglobulins in a few allergic dermatoses. To quantify the IgG, IgA and IgM, one hundred and two sera were analysed form 30 normal control group, 30 contact dermatitis patients, 12 stopic dermatitis patients and 30 urticaria patients by modified Oudin's capillary tube method. The results are as follows. 1.In contact dermatitis the IgG was significantly increased to compare with control group(p<0.01) 2.In atopic dermatitis the IgA was significantly increased to compare with control group(p<0.01) 3.In urticaria the IgG was increased (p<0.05) 4.The IgM has no differences between allergic dermatoses and control group.

Fracture of the coracoid process are rare and those associating with a complete acromioclavlcular separation is even more rare. The mechanism of injury may be attributed to either direct or indirect trauma. Most coracoid fractures are minimally displaced because It is by maintained by the coracoclavicular ligament and coracoacromial ligaments. Usually good result is obtained In the coracoid fracture by non-operative treatment. On the other hand open reduction Is rarely Indicated. The case we are reporting was treated by open reduction and Internal fixation of the acromloclavicular joint using two Kirschner wires, and screw fixation was done for the fracture of the base of the coracoid process. Review of the literature ls also done.

The present study is an observation of 678 cases of children's fractures; which was treated in the Dept. of Orthopedic Surgery, inchon Christian Hospital, for the past ten years, from 1968 to 1978. The peak age was in from 5 years to 6 years. The sex ratio was 2.9 male to 1 female. The ratio between the right and left extremity was nearly equal. The frequency of fracture of the upper extermity was silghtly dominant then lower extremity. The cause of fracture was fall down Injury in nealy 70% of all cases. The epiphyseal Injury was noted as 16.8% of total orthapedic cases. The most common type of Salter & Harris Classiflcation was type II or 56.1%. Mostly all of children's fractures were treated by closed method, and others were treated by open reduction. Complication Included 29 cases of changed carring angle 18 cases of traumatic arthritis and 8 cases of malunlted supracondylar fracture and 3 cases of postoperative Infections and 2 cases of radial nerve palsy and 3 cases of delayed ulnar nerve palsy of the elbow. In fractures of the femur, 3 cases of the peroneal nerve palsy and 2 cases of leg longth discrepancy was noted. Brief discussion and review of Iiterature is presented.

STUDY DESIGN: A case report is presented of a symptomatic sacral perineural cyst with lumbar disc herniation that has reported a few case in the literature. We report the case of symptomatic perineural cyst with lumbar disc herniation. SUMMARY OF LITERATURE REVIEW: Symptomatic sacral perineural cyst is rare and some cases of sacral perineural cyst associated with disc herniation were reported. But it is not well known that the correlation between clinical symptoms caused by mass itself and concomittant lumbar disc herniation. METHODS: A 34-year-old male presented with low back pain, both leg dysesthesia and bilateral radiating pain. Clincal sign and symptom, physical examination, diagnostic imaging studies including MRI, myelography and pathologic findings of surgical specimens were evaluated. RESULTS: After the decompressive partial laminectomy and discectomy, the patient who had diapnosed with herniated nucleus pulposus of L4-5 and L5-S1 complained residual both leg paesthesia and hypoesthesia. Patient's clinical symptom was correlated with sacral perineural cyst. So we removed the cyst. Postoperatively neurologic symptoms and signs were completely relieved. Pathologically, Sacral perineural cyst composed of meningoepithelial cell and neural tissue within cystic wall were confirmed. CONCLUSION: We could find that sacral perineural cyst sufficiently evoked a clinical symptom. So, careful observation was needed in that case of sacral perineural cyst associated with disc herniation.

PURPOSE: To compare the results of a total knee arthroplasty (TKA) between the degenerative osteoarthritic patients and the posttraumatic arthritic patients. MATERIALS AND METHODS: 38 cases with posttraumatic arthritis (group A) and 40 cases with degnerative osteoarthritis (group B) had a total knee arthroplasty between April 1995 and May 2002. The knee score, functional score, activity level and radiologic analyses were performed. RESULTS: There were no significant difference in the knee score, functional score, activity level and radiological findings between the two groups. The range of motion arcs were reduced in the posttraumatic arthritic patients. There is a higher complication rate in the post-traumatic arthritic patients (36%) when compared with the degenerative osteoarthritic patients (15%) after TKA . CONCLUSION: In group A showed a higher complication rate and reduced range of motion arcs. However, in terms of our results, we considered that a total knee arthroplasty is an useful treatment for the posttraumatic arthritis in spite of the technical difficulties.

PURPOSE: We evaluated the polyethylene wear rate between patients from farming and non-farming communities to determine whether active life styles, knee scores, and occupations could affect outcomes. MATERIALS AND METHODS: 201 knee replacements were performed in 105 patients who were an average of 68.7 years old. 30 non-farming patients(59 cases) and 75 farming patients(142 cases) who had operation between 1989 and 1996 were reviewed retrospectively. Mean follow-up period was 12.1 years(range 10~16 years). The difference of clinical, radiographic, and functional results of total knee replacement were assessed by Knee Society scores and activity levels. RESULTS: The non-farming group had lesser activity levels(non-farming group 82.5, farming group 89.7), higher knee scores(non-farming group 88.4, farming group 81.1) and functional scores(non-farming group 87.9, farming group 80.4) than the farming group(p<0.05). Farming patients showed 33 cases(23%) of polyethylene wear and non-farming patients showed 7 cases(12%) which is statistically significant(p<0.001). CONCLUSION: Primary osteoarthritic patients from farming communities had higher activity levels and polyethylene wear rate with lower Knee Society scores which were expected to participate in more activities involving impact on knee joint. We assume that the increase rate of complication is due to the overuse of the prosthesis. Therefore, working condition and life style modification will be needed.

Angioleiomyoma is an uncommon benign tumor composed of smooth muscle cells and vascular endotheliums. It is common in the uterus and the gastrointestinal tract but rare in the head and neck. This neoplasm is extremely rare in the nasal cavity, accounting for less than 1% of all vascular leiomyomas. The tumor has a benign nature and the recurrence of tumor is rare and complete removal is the treatment of choice. We present a case of angioleiomyoma which involves the nasal vestibule and was excised by a sublabial approach.

PURPOSE: To compare the ratio of renal cortical thickness to renal parenchymal thickness in early diabetic nephropathy and in normal control group. MATERIALS AND METHODS: We performed spiral CT in 17 patients suffering from diabetic nephropathy without renal failure or renal atrophy. The normal control group consisted of 19 persons who were normal on renal function test and did not show any abnormality of the kidney. Renal cortical and parenchymal thicknesses were measured at renal hilum level perpendicular to the renal surface by electronic caliper on contrast-enhanced transverse scan demonstrating the cortical nephrogram phase. Using student's test, the difference in renal parenchymal and cortical thickness between the two groups was tested for statistical significance. RESULTS: There was no significant difference in renal parenchymal thickness between the two groups(P>0.05) ; the patient group had a thicker renal cortex than the normal control group however (P<0.05). The ratio of renal cortical thickness to parenchymal thickness in early diabetic nephropathy patients(Rt.:0.041+/-0.051, Lt.:0.382+/-0.053) was significantly higher than in the normal control group (Rt.:0.331+/-0.067, Lt.:0.323+/-0.064). CONCLUSION: The kidney of early diabetic nephropathy patients had a thicker renal cortex than normal kidney.
