1.Analysis for Result of the Treatment of Herniated Disc in the Lumber Spine: A report of 100 cases treated by operation

Result of operative treatment for 100 cases of herniated disc in the lumbar spine treated in the years 1959 through 1968 were reviewed and analyzed. 1. Of the 100 cases 86 were found in the age of 20 to 50 years and the frequency was higher in male than in female as 64 males and 36 females. 2. Sixty eight cases had predisposing trauma before they started to have one or more symptoms, and 32 cases had no history of trauma. 3. Fourty five cases had symptoms for more than one year and 31 cases had symptoms for less than six months before the operation. 4. Plain X-ray and myelogram were taken in all the cases. Amont them 72 cases showed narrowing of the intervertebral space in plain X-ray, while 91 cases showed indentation on the corresponding space by myelogram. 5. Archotomy alone was performed in six cases and total removal of degenerated disc material was performed in additon to archotomy in 94 cases. 6. Seventy five cases had hospitalization within two weeks, while 24 cases had three to four weeks hospitalization after operation. 7 Observation time after Operation was Within Six months in 85 cases and seven months to two years in 14 cases, and one had more than two years observation. 8. Result of the treatment was assessed according to Murphey s scheme, and 53 cases had excellent, 19 cases had good, 14 cases had fair and 14 cases had poor results.

3.Two Cases of Epithelioid Sarcoma with Immunohistochemical Study.

Two typical cases of epithelioid sarcoma were examined by immunohistochemical stain using antibodies to epithelial membrane antigen, carcinoembryonic antigen, vimentin and cytokeratin. Both cases showed positive reactivity for the four kinds of antibodies. These results point to the fact that epithelioid sarcoma simultaneously expresses epithelial markers and characteristic mesenchymal phenotypes. Epithelioid sarcoma appears to be a tumor derived from a multipotential mesenchymal cell with multidirectional differentiation.

6.A case of pigmented spindle cell nevus.

We report a case of pigmented spindle cell nevus occurred in a 4-year-old girl. The lesion was well-demarcated, 2.5 x 3mm sized black macule on the left thenar area. The histopathologic findings of excisional biopsy specimen revealed the prociferation of uniform spindle-shaped pigmented melanocytes at the dermoepidermal junction and sharply definded lateral margins.

8.A Clinical Analysis of Fatigue Fractures among the Navy Recruits: Report of 18 Cases

Among two groups of navy recruits undergoing training, one consisting of 500 enlisted men and another 100 newly enrolled midshipmen, 18 reported to Chinhae Naval Hospital with fatigue fractures during the period from February though September 1969. Following observations were made on these cases: 1. The incidence of fatigue facture was 1.4 percent among the enlisted men and 11 per cent among the midshipmen. 2. There were 6 cases of complete fraeture and 2 cases of incomplete fracture. In the remaining 10 cases, only periosteal reaction was evdent roentgenographically with no visible fracture line initially: fracture lines, however, became apparent on subsequent examinations in all the cases. 3. The fractures involved the femur in 4 cases and the tibia in 14 cases, a ratio of 1:3. 5 in latter s favor. 4. The site of predirection in the femur was the distal one third of the shaft, occurring in 3 of the cases. In the tibia the junction of the upper and middle thirds was most frequently affected, occurring in 10 of the cases. 5. All the cases in our series healed without any complications.

9.A Benign Chondroblastoma in the Greater Trochanter of Femur: A Case Report

Benign chondroblastoma of bone is a relatively rare primary bone tumor which develops in the 2nd to 3rd decade involving the epiphysis of the long bone. The over-all reported cases of the world have been lesser than 400 after determining the entity as benign chondroblastoma in 1942 by Jaffe and Lichtenstein. A case of the benign chondroblastoma with involvement of the greater trochanter of the femur of a 19 years old boy is presented. The conclusive diagaosis was led by histological examination. Considerable new bone formation and bony spicules which might be derived from previous multiple puncture made us to confuse with osteogenic sarcoma. The specimen was composed of reddish brown hemorrhagic and fragile tissue with scattered either yellowish gritty calcified material or bluish translucent ones. The postoperative course has been smooth and there has been no evidence of recurrence for postoperative 15 months.

10.Triple Osteotomy of the Innominate Bone: Experlence with an adult Paralytie Hip

In the treatment of dislocations and subluxations of the hip in the older children, whether congenital or paralytic, orthopedic surgeons are blessed with a wide variety of commonly used surgical procedures; namely, Salters innominate osteotomy, acetabtloplasties, Pembertons pericapsular osteotomy, shelf operations, Chiaris pelvic displacement osteotomy. and Colonnas capsular arthroplasty. However, with increasing age and soft tissue contractures,these procedures become ineffective, leaving a wide range of age between older children and young adults subject to uncertain or unfavarable prognosis. Steels triple osteotomy (1973) is aimed at coverig this age group, when displacement or in nominate osteotomy is either technically infeasible or likely to fail. It consists of an open reduction with or without soft tissue release and skeletal traction, redirection of the acetabulum to cover the femoral head by osteotonmies of the pelvis that has lost its young cartilagenous resiliency, and preservation and physiological remodelling of the articular cartilage of the acetabulum. We performed Steels osteotomy on a 21-years-old female with a severe paralytic subluxation of the hip associated with pelvic obliquity and paralytized both lower extremities. One and a half year follow-up result was satisfactory with a stable and congruous joint despite paralysis and with the patient walking for the first time in her life.
