OBJECTIVETo observe the effects of Qingyi II Recipe (QR) on the intestinal barrier function (IBF) of severe acute pancreatitis (SAP) rats.
METHODSForty-eight SD rats were selected to prepare SAP model. After modeling, they were randomly divided into the model group and the QR treatment group, 24 in each group. Another 8 rats were selected as the sham-operation group. The intervention was started just when they came to resuscitation from anesthesia. Rats in the QR treatment group was administered with QR (1 mL/100 g) by gastrogavage, while an equal volume of normal saline was given to rats in the sham-operation group and the model group by gastrogavage. The gastrogavage was performed once every 6 h. Eight rats were selected from each group 6, 12, and 24 h after intervention. The ascites amount was measured after opening abdomens. The concentrations of serum diamine oxidase (DAO) and D-lactate were measured. Pathological changes of pancreas and ileum were observed. The pathological scoring of the pancreas was performed. The ileum were scanned by electron microscope.
RESULTSThere was no ascites in the sham-operation group. There was no obviously pathological abnormality in the pancreas or ileum. Compared with the sham-operation group, the ascites amount, serum DAO, D-lactate, and pathological scoring all increased at 6 h in the model group, showing statistical difference (P<0.05). In the model group, the serum DAO, D-lactate, and pathological scoring increased gradually at 6 -24 h with statistical difference (P<0.05). Results under microscope showed disarranged structure, interstitial edema, hemorrhage, a large amount of neutrophil infiltration, local foci or lamellar intestinal necrosis. The 24-h ascites amount increased more at 24 h than at 12 h in the model group with statistical difference (P<0.05). The serum D-lactate and patho- logical scoring increased gradually at 6 -24 h in the QR treatment group with statistical difference (P<0.05). The 24-h ascites amount and DAO increased more at 24 h than at 12 h in the model group with statistical difference (P<0.05). Compared with the model group at the same time point, each index decreased at 6 -24 h in the QR treatment group with statistical difference (P<0.05). Results under microscope showed edema and congestion of the intestinal mucosa tissue, little amount of neutrophil infiltration. But the degrees were milder when compared with those of the model group.
CONCLUSIONQR protected IBF of SAP rats.

The expression of the vgb gene in vivo could improve the fermentation density and then contribute the extracellular secretion of the product of bxn gene. Constructed the recombination plasmid pPIC9K-vgbbxn and transformed into Pichia pastoris GS115. The results of PCR and SDS-PAGE indicate that the vgb gene and bxn gene had integrated into the genome of Pichia pastoris GS115 and expressed in efficient level. Also, the protein activity of their products had been verified respectively. Shake flask fermentation experiments showed that the presence of VHb in yeast Pichia pastoris efficiently enhanced cell growth and secretive expression of bxn gene under hypoxic habitats.

Objective To observe the value of ultrasound-guided intra-articular injection of hypertonic glucose(HG)for treating knee osteoarthritis in plateau area. Methods Ninety-one patients with knee osteoarthritis in plateau area(124 affected knee joints)were enrolled. The knee joints were divided into 20%HG group(n=67),25%HG group(n=42)or sodium hyaluronate group(n=15)based on the medication.Clinical and ultrasound scores were compared before and after treatment, and the efficacy of injection of HG was evaluated. Results At the 12th and 48th weeks after treatment, visual analog scale(VAS)scores decreased in all 3 groups(all P<0.05),and Lysholm scores of 20%HG group and 25%HG group increased compared to those before treatment(all P<0.05).The difference of Lysholm score before and in the 12th week after treatment, and of VAS score before and in the 48th week after treatment of 25%HG group were higher than those of 20%HG group(both P<0.05).The joint exudation score of 20%HG group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial blood flow score decreased in the 12th and 48th weeks after treatment compared to those before treatment(both P<0.05).The joint exudation score of 25%HG group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial hyperplasia and synovial blood flow score decreased in the 12th and 48th weeks after treatment compared to before treatment(all P<0.05).The joint exudation score of sodium hyaluronate group decreased in the 48th week after treatment compared to that before treatment(P<0.05),and the synovial hyperplasia score decreased in the 12th and 48th weeks after treatment compared to that before treatment(both P<0.05).Conclusion Ultrasound-guided intra-articular injection of HG,esp.25%HG,had certain value for treating knee osteoarthritis in plateau area.

Objective: To investigate the safety, feasibility and short-term efficacy of total laparoscopic loop ileostomy reversal in patients after resection of rectal cancer. Methods: The clinical data of 20 patients who underwent total laparoscopic loop ileoscopic loop ileostomy after radical resection of rectal cancer at Cancer Hospital, Chinese Academy of Medical Sciences and Peking Union Medical College, or Beijing Chaoyang District Sanhuan Cancer Hospital from October 2019 to June 2020 were collected and retrospectively analyzed. Results: All patients had successfully underwent total laparoscopic ileostomy reversal without conversion to open surgery or discontinued operation. No perioperative related death cases were found. In the whole group, the median operation time was 97 (60-145) minutes and the median intraoperative blood loss was 20 (10-100) milliliters. The median Visual Analogue Scale (VAS) score was 1.9 (1-5) one day after the operation. Nobody needed to use additional analgesic drugs. The median time to grand activities was 25 (16-42) hours, the median time to flatus was 44 (19-51) hours, and the median hospitalization after operation was 6.9 (5-9) days. No patients underwent operation related complications such as operative incision infection, abdominal and pelvic infection, intestinal obstruction, anastomotic leakage, bleeding and so on. Conclusions: Total laparoscopic loop ileostomy reversal appears to be safe, feasible and with promising efficacy for selected patients.
