We studied the efficacy of laser suture lysis in the promotion of filtration and bleb formation during the early postoperative period after trabeculectomy. Laser suture lysis was performed on thirty-two eyes of thirty patients considered overtight scleral flap suture among patients who underwent trabeculectomy with mitomycin-C, using an argon laser and a Hoskins lens. Patients were entered into the study if, after ocular massage, the intraocular pressure(IOP) was greater than our target pressure(8-12mmHg) 48 Hours after surgery, deep quiet anterior chamber, weak or no bleb formation, no bleb leakage, and a patent internal ostium on gonioscopy. Intraocular pressure measured within 5 minutes of lysis and gentle ocular massage was employed if there was no sopontaneous bleb formation. A further suture was lysed if at this stage there was still no bleb associated with a high IOP. Suture lysis was successful in all cases in the early postoperativy period. The mean IOP before lysis was 22.1mmHg and after serial lysis 9.8mmHg. Complications were noted with laser suture lysis: shallowing of anterior chamber, ocular hypotony, sunconjunctival hemorrhage, hyphema and cataract. All resolved with appropriate management. Laser suture lysis is a safe, effective, and non-invasive method, It can avoid surgical manipulation and increase long term success rate of trabeculectomy.

Allogenic peripheral blood stem cell transplantation could be used instead of allogenic bone marrow in treatment of leukemia in children. This 10-year-old female patient with high-risk acute lymphoblastic leukemia received a myeloablative regimen followed by allogenic peripheral blood stem cell transplantation from an HI A-identical sibling donor. Neutrophil recovery to greater than 500/pL occurred at day 11 and platelets recovered to greater than 20,000/pL at day 13. Allogenic peripheral blood stem cell transplantation can be performed safely and may result in a rapid neutrophil and platelet engraftment, without any apparent increased risk of acute graft versus host disease.

Total or mostly total intestinal aganglionosis is rare and invariably fatal. All patients with aganglionosis involving more than half of the small bowel have died with persistent intestinal obstruction and secondary malnutrition and infection. Recently, we experienced two cases of total colon aganglionosis extending to the middle of the jejunum in triplet. We report the cases with brief review of literatures.

A case of Aicardi syndrome with cleft lip and palate was experienced at the Department of Pediatrics, College of Medicine, Hallym University, and the patient's infantile spasm was treated with ACTH. In previous studies, four cases of Aicardi syndrome accompanied by cleft lip and palate were reported. We present the fifth case of Aicardi syndrome with cleft lip and palate in the world. Thus, facial clefts may be found as an occasional manifestation of Aicardi syndrome.

The segmental spread of epidural analgesia was measured in seventeen surgical patients aged between 17 and 52 years, and in fourteen patients between 60 and 77 years. The upper level in the young was 6.29(+/-1.56) thoracic vertebra level, but in the elderly was 4 (+/-0.65_ thoracic vertebra level at 20 minutes after epidural injection of 1.5% lidocaine 20 ml. A given volume of solution spreads to 0.9, 1.07, 1.54, 2.29 segments greater upper extent at 5, 10, 15, 20 minutes after epidural injection and 0.57, 1.07 segments greater lower extent at 5, 10 minutes in the elder than young. So with increasing age relatively small amounts of solution are required, to produce the same extent of anesthesia in the young.

OBJECTIVE: To investigate the relations among pain, emotional factors and cognitive-behavioral factors in chronic musculoskeletal pain patients. METHOD: This study was designed as a questionnaire survey. 73 patients who visited our outpatient clinics due to chronic musculoskeletal pain were evaluated. They checked visual analogue scale (VAS), pain site and frequency. Beck depression inventory (BDI) and state-trait anxiety index (STAI) were checked for evaluation of emotional factors. Somatization scale of symptom checklist-revised (SCL-R- 90) and pain catastrophizing scale (PCS) were checked for cognitive-behavioral factors. Correlations among each variable were evaluated by statistical analysis. RESULTS: VAS was correlated with pain frequency (r=0.538, p<0.01), somatization and catastrophizing thought. Rumination thought was correlated with VAS (r=0.443, p<0.01), pain frequency (r=0.446, p<0.01) and static anxiety (r= 0.449, p<0.01). Magnification thought was correlated with depression (r=0.403, p<0.01) and static anxiety (r=0.447, p<0.01). Helpless thought was correlated with VAS (r= 0.442, p<0.01), pain frequency (r=0.429, p<0.01), depression (r=0.510, p<0.01), static anxiety (r=0.640, p<0.01) and somatization (r=0.504, p<0.01). Duration was not correlated with VAS, depression, anxiety, somatization and catastrophizing thought in chronic musculoskeletal pain patients (p>0.05). CONCLUSION: Cognitive-behavioral factors such as catastrophizing thought are strongly correlated with both pain aspects and emotional factors. Consideration of cognitive- behavioral factors as well as emotional factors may be important for management of chronic musculoskeletal pain.

Allodynia is pain following a non-noxious stimuli which does not provoke pain normally and develops after incomplete spinal cord injury more commonly in cervical rather than thoracic level, and central cord syndrome. This article presents an unusual patient who presented with the single symptom of an intense allodynia after cervical intervertebral disc herniation. This 36-year-old male patient developed acute lancinating and burning pain aggravated by skimming light touch on both thenar area. Cervical magnetic resonance imaging (MRI) revealed central disc herniation and spinal cord compression. The allodynia secondary to acute herniated cervical disk has been successfully disappeared through pharmacotherapy with pulsed-use of steroid, gabapentin and comprehensive rehabilitation.
