1.DNA Patterns by Flow Cytometric Analysis of Squamous Carcinoma Cells of the Cervix Uteri : Relations with Prognosis and Biochemical Characteristics.

This study was performed to find out the relationship between DNA pattern by flow cytometry and the prognosis of cervical squamous cell carcinoma, The subjects were 71 patients who were admitted to Catholic University Medical College Kangnam St. Marys Hospital from September 1983 to May 1989 under the diagnosis of cervical squamous cell carainoma. Samples were obtained from the subjects before chemotherapy or radiation therapy, and a through follow up stduy was carried out an the subjects. The authors obserued the relations between flow cytometric DNA pattern and clinical stage, age of patients, histopathological findings and tumor markers, with the following results. 1. The S-phase of the cell cycle increased significantly in Stages I, ll and III-IV compared to that of Stage O.However, there was no difference between Stages in G0/G1 phase and G2/M phase. 2. The incidence of aneuploid tumor increased significantly according to stage with the rates of increase reaching 22, 22%, 31.57%, 44.44% and 68.75% respectively in Stages 0, I, II, and Ill-lV. 3. The incidence of aneuploid tumor increased significantly depending on the age of patients with the rates standing at 21.42%, 31.58%, 40.90% and 68.75% respectively in the groups of below 40, 41-50, 51-60, and 61 years and over. 4. As for the relation between histopathological findings and the incidenee of aneuploid tumor, no difference was found according to the degree of differentiation, but there were significant differences between the keratinization group(23.07%) and the non keratinization group(51.11%).

2.Eyelid Reconstruction by the Semicircular Flap.

Upper and lower eyelid defects of 40-60% can be reconstructed by the Tenzel semicircular flap technique. Lower eyelid defects of 75-80% can be successfully corrected if the lower eyelid retractors and inferior orbital septum are severed from their attachment. This technique provides several distinct advantages. It is useful for reconstruction of both the upper and lower eyelids. It can be accomplished without borrowing tissues from other sites. It provides a length about 1.5 times greater than would be available along the straight line diameter. The indications for the use of this procedure included basal cell carcinoma(4 cases), compound nevus(1 case), post-traumatic eyelid deformity(2 cases). We obtained cosmetically good results in all cases without any senous postoperative complications.
3.The Kinematic Study for Stability of Contact Surface Area between Acetabular Cup and Femoral Prosthesis on the Flexion, Abduction, or External Rotation of the Hip Joint

The range of acetabular cup and femoral prosthesis according to the different angle of insertion has been reported by many investigators. But no reports are still avairable about the surface area of the exposed femoral head at the position causing unstability of the femoral head. So the authors had studied the 3 types of prosthesis of Charnley, Muller and Mckee-Farrar and their acetabular cup by inserting them at different angles into the 10 human cadavaric skeleton to obtain the most stable position of the prosthesis and to calculate the surface area of the femoral head at the stable position. The result obtained are as follows. 1. In. Flexion: Among 3 types of prosthesis, the maximum stability is 78% to the exposed area of the femoral head in Mckee-Farrar prosthesis with 10 of the femoral anteversion, 45 of acetabular inclination and 45 of acetabular anteversion. The minimum stability is 41% to the exposed area of the femoral head in Mckee-Farrar prothesis with femoral neutral version, 45 of acetabular inclination and 15 of acetabular retroversion. 2. In Abduction: Among 3 types of prosthesis, the maximum stability is 86.5% to the exposed area of the femoral head in Mckee-Farrar prosthesis with 10 of the femoral anteversion, 45 of acetabular inclination and 45 of acetabular anteversion. The minimum stability is 50% to the exposed area of the femoral head in Muller prosthesis with femoral neutral version, 45 of acetabular inclination and 30 of acetabular retroversion. 3. In External Rotation: Among 3 types of prosthesis, the maximum stability is 85. 8% to the exposed area of the femoral head in Mckee-Farrar prostbesis with femoral neutral version, 45 of acetabular inclination and 30 of acetabular retroversion. The minimum stability is 46% to the exposed area of the femoral head in Mckee-Farrar prosthesis with femoral neutral version, 30 of acetabular inclination and acetabular neutral version. 4. There was no relationship between the degree of range of the femoral prosthesis & acetabular cup and the amount of the surface area of the exposed femoral head when dislocated.

4.The diagnostic value of barium enema in acute appendicitis

Acute appendicitis is the most common acute surgical condition of the abdomen. when the clinical presentationis atypical, barium enema has proven to be safe and useful in confirming the diagnosis and reducing the negativesurgical exploration. However, the performance of barium enema in acute appendicitis has known contraindicationprimarily because of fear of leakage by perforation of the inflammed appendix. This study using barium enema as adiagnostic aid in acute appendicitis with atypical clinical presentation was performed to further support thepreviously noted efficacy and safety of this procedure. The results were as followings: 1. In case of acuteappendicitis with atypical clinical presentation, the use of barium enema as a diagnostic aid increased theaccuracy of diagnosis and decreased the negative surgical exploration. In women between 11 to 50 years old age,especially, it played important role differentiating appendicitis from nonsurgical acute abdomen. 2. The resultsof the study were 92.31% in sensitivity, 7.69% in false positive, 6.9% in false negative, and 10.26% in negativeappendectomy. 4. A simple partial or nonfilling of appendix without other associated possitive finding could notexclude appendicitis, therefore, close clinial observation was necessary. 5. The positive findings of barium enemaand their sensitivity were as followings: 1. Nonfilling of appendix: 9% 2. Partial filling of appendix: 91.7% 3.Displacement or a local impression on temrinal ileum: 100% *, In all cases, combined with a local impression oncecum and/or irritability of cecum or terminal ileum.

5.A Case of Carcinoma Erysipelatoides Originating in an Internal Organ.

We present a case of carcinoma erysipelatoides which or iginated from an internal organ. The patient was a 50 year-old female who had peritoneal carcinomatosis, She presented an erythematous lesion resembling erysipelas on her right breast. Histopathologic findings were consistent with metastatic adenocarcinoma but clinically it was not associated with carcinoma of the breast.

6.Factor VIII Related Antigen in Granuloma Pyogenicum.

Using Avidin-Biotin immunoperaxidase method, we investigate the distribution patterns of factor VIII related antigen in various areas of skin lesions of 14 granuloma pyogenicum. and in the skin lesions of 5 capillary hemangioma and 5 normal skin. The results are summarized as follows: 1. Factor VIII related antigen was detected in 9 of 14 cases of granuloma pyogenicum, in 4 of 5 cases of capillary hemangioma, and in 5 of 5 cases of normal skin. 2. In the lesions of granuloma pyogenicum the areas with prominent capillary endothelial cell proliferation had less positivity than the vascular areas. And the areas with relatively large vesels in the lesions of granuloma pyogenicum had similar positivity as capillary hemangioma. 3. The lesions of granuloma pyogenicum and capillary hemangioma had less positivity than the vessels of normal derrnis.

7.A Case of Angiommunoblastic Lymphadenopathy.

Herein we report a 51-year-old male with angioimmunoblastic lymphadenopathy who had developed rnaculopapular eruptions after the administration of crystalline penicillin, acetylsalicylic acid and sulpyrine. Skin biopsy showed mild lyrnphohistiocytic infiltration and extravasated RBCs around the blood vessels in the upper dermis. He had run a progressively downhill course in spite of vigorous chemotherapy and ultimately died.

8.Statistical Analysis of Serologic Test for Syphilis in Normal Population (1981 ~ 1984).

Serologic tests for syphilis including VDRL and TPHA tests were carried out in 5,413 VISA applicants for emigration, from January, 1981 to December, l984 The results are summarized as follows: 1. In 5,413 VISA applicants, the reactive rate of VDRL test was 2,0% totally. The reactive rate of VDRL test of male was 2. 6%, and that of female was 1. 6% 2. The reactive rate of 1983 was 1.4% and that was the lowest in annual incidence, but there was no statistically significant differences in comparing the annual incidences. 3. The reactive rate increased with age, and males above the 5 th decade revealed an especially high reactive rate. 4. The biologic false positive rate of VDRL teet was 16. 5%, in gl VDRL reactive persons, using TPHA as standard. 5. The quantitative test of VDRL showed low titer below 1: 4 in 88. 4%, and high titer above 1: 8 in 11. 6% of the applicants.

9.Two Cases of Lichen Planus.

We have experienced 2 cases of lichen planus. Case 1 was a. 76-year-old male who has suffered from discrete, scaly, round to polygonal papules on the trunk and the e:xtremiities for 3 months. The VDRL test for syphilis was reactive at a titer of 1:2 and the TPHA test was positive. The skin lesions were responded to 3 weekly injections of benzathine penicillin 2.4 million units, Case 2 was a 17-year-old male who has suffered from discrete, erythematous to violaceous colored, flat papules on the right shin for several months. He has treated with oral medication of griseofulvin. After 2 weeks of the medication, the skin ]esions showed improvement with gradual reduction of pruritus and flattening of the papules. About one month of the medication, all skin lesions cliappeared with dark brownish hue and hypopigmentation. And recurreci lesions after stopping medication are disappeared after 2 months of retreatment with griseofulvin.

10.Arthroscopy in Traumatic Hemarthrosis of the Knee

We have investigated a series of 45 acute traumatic hemarthrosis of the knee between Jan. 1989 and Jan. 1993 at National Medical Center. All the patients who had normal findings on plain X-ray with hemarthrosis underwent arthroscopic examination under anesthesia. The arthroscopic results were as follows. 1. There were 14 cases(31%) of anterior cruciate ligament(ACL) tear, a case(2%) of avulsion fracture of tibial spine, 13 cases(29%) of lateral meniscus tear, 8 cases(18%) of medial meniscus tear, a case(2%) of osteochondral fracture of patella, and 8 cases(18%) showed no specific injury of internal structure. 2. The positive predictive value of Lachman and anterior drawer tests under anesthesia was 93% and 92%, respectively. 3. The positive predictive value of McMurray tests for medial and lateral menisci was 33% and 64%, respectively. 4. We concluded that a traumatic hemarthrosis indicated a significant knee injury and arthroscopy allowed a more accurate diagnosis of injury to joint structure.
