2.Factors affecting on the compliance of health screening program: A study for the insured in a district medical insurance association.

BACKGROUND: Recently preventive clinical care has been proved important to the health. A number of recommendations or programs for preventive health services have been developed. The government made laws for health promotion of the people and for medical check-up for the insured in the district medical insurance association, who had been alienated from reimbursement for periodic health examination. As a result, above 95% of Korean people could took the periodic health check-up by this legislation. According to the national data, however, about 20% of the insured participated in the health screening program which was carried for the first time by the district medical insurance association in 1995. The reason that the compliance of health screening program was much lower than expected has not been clear yet. The purpose of this study is to define the facors affecting on the compliance of the health screening program. METHODS: Two hundreds of compliers and as many non-compliers of the health screening program was selected by the stratified sampling in one district medical insurance association which consisted of one town and eight township within the National Capital region. The telephone survey was performed from June 1 to 15 in 1996. The questionnaire was framed by health belief model with other demographic data. The data was analyzed by chisquare test, wilcoxon rank sums test, and logistic regression for the comparison of the characteristics between compliers and non-compliers. RESULTS: There was no difference of demographic data and medical history of chronic disease between compliers and non-compliers. 73.3% of the subjects knew they had been the expectant examinee of the health examination, and among them, 68% took the health examination. However, 53% of non-compliers did not know that they had been the expectant examinee. The most common reason that non-compliers didnt took the health examination was they didnt know(43.5%). Among 73 families of compliers which had more than 2 expectant examinees of the health screening program, 44 families(60.3%) had more complier other than study subjects. In contrast, among 74 families of non-compliers, only 7 famiies(9.5%) had more complier other than study subjects. Compliers showed lower reliability of the health screening program than non-compliers. Non-compliers had more perception about the severity of the target disease for the examination and had more confidence about self health status than compliers. There was no difference of the answer about the benefit of the health examination, perceived susceptibility about target disease, cue to action, and perceived barriers. The odds ratio of compliance of other family member was 21.2, perceived severity, 0.70, and cue to action, 1.36. CONCLUSIONS: The main reason of low compliance of health screening program for the insured in the district mecical insurance association was lack of public information. By the fact that non-compliers had more perception about the severity of the target disease and there was no difference of perceived benefit of the health examination between two groups, public information should be focused on the benefit and efficacy of periodic health check-up than severity and susceptibility of target disease. Because the health screening program on study subjects was carried for the firt time, only compliers had experience of the examination. The fact that compliers relied on the examination less than noncompliers meant that the examinees was satisfied much less than expected. Therefore, further study of the expectation of the examinees about the health examination is expected.

3.A cases of sarcoidosis with annular cutaneous lesions.

Cutaneous manifestations of sarcoidosis are highly variable and can be devided into the specific lesions which reveal noncaseating granuloma and the nonspecific lesio is which are referred to as cutaneous changes. Among speciific lesions, annular lesions may be foimied from coalescing papular lesions that may clear centrally and elevate peripherally. We report a case of sarcoidosis with multiple annular lesions on the face in a 33-year-old female who had systemic involvements in the intrathoracic organs and eyes. The main histopathologir. findings were well-demarcated islands of epitheloid cells with a few giant cells in the dermis. In addition, a slight admixfure of lymphoid cells was present at the margins of the epitheloid cell granulomas. Complete regression of t!he skin lesions was obtained with systemic cortiocosteroid therapy and no recurrence has been found for 2 1/2years.

5.A case of Kaposi's sarcoma Treated with radiotherapy.

Kaposi's sarcoma is a neoplasm of multifocal origin which manifest primarily as vascular nodules in the skin and other organs. We report a case of Kaposis sarcoma of classic type in a 17-year-old boy, who had a 4 x 5cm sized, dusky erythematous plaque with multiple spots of acupuncture on the left foot for 1 year. Histopathologic examination showed dilated, bizarre-shaped vessels, ndothelial cell proliferation, and spindle cell formation with vascular slits throughout the dermis. He was treated with 3000 rads of radiation(300 rad per day for l0 successive days). Six months after radiotherapy, gross findings were much improved and histopathologic findings showed only fibrosismc hemosiderin deposits in the dermis.

6.Expression of Exon 5 - Deleted Estrogen Receptor mRNA in Human Uterine Leiomyoma and Myometrium.

OBJECTIVE: There is a large and increasing body of experimental and clinical data supporting the existence of variable estrogen receptors in both normal and neoplastic estrogen target tissue. Recently, exon 5 deleted estrogen receptor(D5-ER) has been identified in many estrogen dependent tumors. The purpose of this study is to assess the relationship between uterine myoma and D5-ER. METHODS: The relative amount of wild type estrogen receptor mRNA and D5-ER mRNA were examined in human myometrium and myoma tissue obtained from 12 myoma patients and 12 control subjects by RT-PCR. The correlation between WT/D5-ER ratio and age was also evaluated. RESULTS: There was no difference in expression of D5-ER mRNA not only between myoma and myometrim of the myoma patients, but also between myometrium of myoma patients and normal subjects. No significant correlation was found between D5-ER expression and age of the subjects in myometrial tissue. However, we could find negative correlation between WT/D5-ER ratio and age in myoma tissue. CONCLUSION: Upon these data, we conclude that D5-ER is a common variant mutation of estrogen receptor, which is not related with development of myoma. However, in myoma tissue, there is possibility that D5-ER may act some role in the growth of tumor.

7.A Case Report of Intussusception of the Vermiform Appendix with Adenocarcinoma.

The authors present a case of intussusception of the vermiform appendix with ade nocarcinoma. A 35-year-old male with lower abdominal pain and anemia was found to have an intussusception of the appendix associated with an adenocarcino-ma. The preoperative diagnosis was cecal cancer but we detected the iutussuscept-ion of the appendix with an adenocarcinoma during operation. Patient was managed with right hemicolectomy and has been followed up.

8.Atrophoderma of pasini and pierini.

We report a case of atrophoderma of Pasini and Pierini in which the characteristic soft atrophic patch with cliffdrop border developed on the neck and two well-demarcated indurated atrophic patches were on both shoulders. We took biopsies from centers and margins of each lesion. Histipathologic findings from atrophic center showed marked dermal atrophy comparing to that from margin of the lesion on the neck, but the atrophic center and margin of the lesion on the shoulder showed morphea-like features without, distinct differences between two.

10.Two Cases of Extramammar Paget's Disease.

We have experienced two cases of extramammary Paget's disease. The first case was a 68-year-old male who showed erythematous, crusted, oozing, eczematoid patches on the scroturn, penile root and pubic area, and verrucous plaque on the left pubic area of 5 years' duration. The second case was a 60-year-old male who showed erythematous, oozing, hyperkeratotic plaque and nodular mass on the scrotum of 4 years' duration. In electron microscopic findings of the first case, the Paget's cells was suggested to be formed by pleuripotential germinative cells; well developed microvilli, numerous electron lucent secretory granules, a few electron dense granules, apocrine type secretion and well developed desmosome.
