1.Change of estrogen receptor in hippocampus, hypothalamus and pituitary of female rats exposed to psychological stress

AIM: To investigate the change of estrogen receptor (ER) in hippocampus, hypothalamus and pituitary of female rats exposed to psychological stress, and to illuminate the mechanism of dysfunction on ovarian reproductive endocrine function. METHODS: Sound, light and electricity were combined into a psychological stressful stimulus to induce female rat dysfunction on ovarian reproductive endocrine function. Immunohistochemical technique and image analysis were used to assess the expression levels of ER in hippocampus, hypothalamus and pituitary. RESULTS: When exposed to compound stressful stimulus of sound, light and electricity for 20 days in female rats, the expression levels of ER in hippocampus, hypothalamus and pituitary dropped. CONCLUSION: The decrease in estrogen receptor expression in hippocampus, hypothalamus and pituitary of female rats exposed to psychological stress may be one of the mechanisms of ovarian reproductive endocrine dysfunction.
2.Lichtenstein tension-free hernioplasty repair in adult patients with inguinal incarcerated hernia

Objective To investigate the preoperative management and the clinical effeciency of Lichtenstein tension-free hernioplasty in adult patients with inguinal incarcerated hernia.Methods Clinical data of 86 patients with inguinal incarcerated hernia were analyzed retrospectively.Hernia was repaired with Lichtenstein tension free after reposition.Results There were 59 male patients and 27 female patients with median age of (63 ± 18) years.There were 8 patients with liver cirrhosis.The operation was performed successfully in all patients.Segmental bowel resection with end-to-end anastomosis was performed in 38 emergency cases.Operative time was 20-120 min,with an average time of 54 min.The postoperative hospitalization was 5-17 d,with an average of 8 d.There were 7 cases of skin ecchymosis at the scrotum,there were no intestinal perforation,hepatic encephalopathy and upper gastrointestinal hemorrhage after operation.In early series of 24 cases without drainage tube left in place,there were 10 cases of fat liquefaction,10 cases of hydrops of hernial sac,6 cases of seroma and 3 cases of wound infection after operation.After 12 to 48 months of follow-up,there was no mortality after 2 years,no hernia recurrence.Conclusions Tension free repair in the treatment of incarcerated inguinal hernia is safe and feasible.
3.Analysis of brain MRI and clinical features of Japanese encephalitis in children

Objective The purpose of this study was to investigate the clinical features and the magnetic resonance imaging of brain of children with Japanese encephalitis (JE) and whether this relationship can be used in early diagnosis.MethodsMRI was performed in 145 children with JE.T1-weighted,T2-weighted and FLAIR of brain were obtained in 98 patients with 0.35 T MRI system and conventional MRI and DWI were performed in 47 patients with 1.5 T MRI system.The position,range and MRI features were analyzed.ResultsMR imaging studies of 106 patients showed hyperintensity in the thalamic with 87 patients involved the caudex cerebri,as seen on T2-weighted images.The lesions were also seen in basal ganglia (32 cases),cerebral cortex (43 cases),cerebellum (2 cases),and white matter (15 cases).Lesions showed high intensity signal on T2WI and T2-FLAIR,and iso- or low intensity on T1WI.The number of the lesions in FLAIR were more than T2WI.The positive rate on 1.5 T MRI (87.2%) was higher than that on 0.35 T MRI(66.3%,χ2=7.06,P<0.01).ConclusionsMultiple brain tissues can be involved and the thalamic and the caudex cerebri involvement are the conspicuous characteristics in children with JE.T2-FLAIR and DWI are helpful in the early diagnosis of JE.
4.Difficulties and countermeasures of construction of the professional degree clinical master's stan-dardized and regulated training system

In the process of construction of the professional degree clinical master's standardized and regulated training system, training institutions have encountered some disparities and problems such as lack of correct recognition, no strict restrictions on enrollment, curriculum system construction not meeting the requirements of medical education, lack of perfect clinical evaluation system, inadequate clinical re-search and the role of elimination mechanism being ignored. To resolve these problems, feasible suggestions are raised: strengthening the cognition of training model reform, strictly checking on candidates' qualifica-tions to ensure the quality of students, refining courses and reforming teaching method, completing examin-ing system and enhance the management of practical training, focusing on the feature of clinical scientific research, emphasizing the construction of mechanism of elimination and diversion to improve the quality of training.
5.Analyzing the correlation between coronary artery stenosis and left ventricular function and myocardial ischemia using dual-source computed tomography.

OBJECTIVETo study coronary artery stenosis, myocardial ischemia, and left ventricular dysfunction in dual source computed tomography (DSCT) coronary artery angiography.
METHODSTotally 32 patients underwent both DSCT and X-ray coronary angiography within one week to detect coronary artery stenosis separately. Meanwhile, the values of left ventricular myocardial mass (LVMM) , left ventricular ejection fraction (LVEF) , and left ventricular stroke volume (LVSV) were calculated using cardiac function software in DSCT. Electrocardiography was carried out to diagnose myocardial ischemia. The coronary artery stenosis, values of LVMM, LVEF, and LVSV, and myocardial ischemia were compared.
RESULTSThe results of DSCT and X-ray coronary angiography were not significantly different. LVMM, LVEF, LVSV, and myocardial ischemia were significantly different between two- or three-branch groups or between middle or severe groups (both P<0.05) . However, no such significant difference was observed between single and two branch groups and between mild and middle groups. There were no statistically different findings for LVEF and LVSV, but there was statistical deference between LVMM and myocardial ischemia (P<0.05) . For single branch and middle to severe stenosis in left anterior descending (LAD) coronary artery, right coronary artery, left circumflex coronary artery, only the values of LVMM,LVEF,and LVSV in LAD group showed significant difference (P<0.05) .
CONCLUSIONSMore stenotic branches and severer stenosis in coronary artery often are associated with higher incidence of myocardial ischemia and severer left ventricular dysfunction. The stenosis of LAD coronary artery has especially severe impact on cardiac functions. LVMM is a sensitive indicator for myocardial ischemia in coronary artery stenosis.

7.Effects of Ca2+ and SA on physiological and photosynthesis of Platycodon grandiflorum under high temperature stress.

In order to reveal feasibility of different concentrations of Ca2+ and SA on Platycodon grandiflorum under high temperature stress, the effects of Ca2+ on physiological index and related photosynthetic parameters were studied. Pot cultured P. grandiflorum leaves under the same outdoor conditions were sprayed with CaCl2 and SA separately, and then placed in the high incubator [35 degrees C/25 degrees C (day/night), light intensity 3 600 lx], and sprayed with distilled water at 25 degrees C and under high temperature stress were set as the control. The related photosynthesis, relative conductivity, contents of proline, malondialdehyde, soluble protein, activities of SOD and CAT, ASA and GSH content were measured. The results show that the 6 mmol x L(-1) CaCl2 and 1.5 mmol x L(-1) SA enhanced the activities of SOD and CAT, the contents of proline and soluble protein, and effectively reduced the damage of heat stress on cell membrance. At the same time, the exogenous Ca2+ and SA increased the contents of chlorophyll and carotenoid, the efficiency of leaf photosynthesis and ASA and GSH content, and thus effectively resisted the oxidative stress caused by high temperatures, but with the increasing concentration of spraying, P. grandiflorum decreased the ability to resist high temperature stress. In conclusion, the foliage spraying CaCl2 and SA could reduce the damage of high temperature stress on P. grandiflorum leaves.

8.Early warning value of laboratory indexes for death risk in children with critical hand foot and mouth disease

Objective To quantitatively analyze the early warning value of laboratory indexes for death risk in children with criti ‐cal hand foot and mouth disease (HFMD) .Methods The univariate and multivariate Logistic regression analysis were conducted to explore the independent risk factors of death in critical HFMD children .Then the area under receiver operating characteristic curve (AUC) was applied to give the comprehensive assessment of the test model ,as well as the early warning capacity and the optimal cut‐off level of laboratory indexes in critical HFMD children .Results The AUC of the Logistic regression model (Y ) established based on white blood cell ,neutrophil ,myoglobin ,creatinine for early predicting the death risk in critical HFMD children patients was 0 .847 (95% CI :0 .783 - 0 .911) ,which indicating that its diagnostic value was superior to single index .Conclusion The diag‐nostic value of the Y model established based on four indexes of white blood cell count ,neutrophile granulocytes count ,myohemo‐globin and creatinine is superior to any single index ,which has the better early warning value for the death risk in children with crit‐ical HFMD .
9.Effect of different nitrogen forms and ratio on growth and active ingredient content of Platycodon grandiflorum.

To providing evidence about nitrogen adequate application of Platycodon grandiflorum, the pot culture experiment was conducted to study the effect of nitrogen on the growth, physiological metabolism and the quality of P. grandiflorum. The activity of NR, GS and SOD, POD and CAT were determined. And the nitrate and ammonium nitrogen content, photosynthetic characteristics, active components of P. grandiflorum were determined. The results showed that the nitrate nitrogen content and P. biomass reached its maximum value, when NH4(+)-N/NO3(-) -N was 0: 100, the activity of NR. The activity of GS was the highest at the NH4(+) -N/NO3(-) -N ratio of 25:75 and ammonium nitrogen content was the highest at 75:25. The activity of SOD decreased and then increased with the increasing of NO3(-) -N. At the NH4(+) -N/NO3(-) -N ratio of 25: 75, the activity of CAT had its maximum value and the content of MDA had the minimum value. At the same time, the content of platycodon D was the highest at this treatment. The studies had shown that different nitrogen forms and ratio had a significant effect on the characteristics of photosynthetic physiology, nitrogen metabolism and resistance adjustment, growth and the quality of P. grandiflorum. The NH4(+) -N/NO3(-) -N ratio of 25: 75 was a suitable ratio of nitrogen forms for the growth of P. Grandiflorum and accumulating the content of platycodon D.

10.Molecular epidemiology and antimicrobial susceptibility of Salmonella isolates recovered from children with diarrhea

Objective To investigate molecular epidemiology and antimicrobial susceptibility of Salmonella spp. isolates recovered from the stool samples of children with diarrhea. Methods Seventy-two isolates of Salmonella spp. were collected from children with diarrhea. The serum type of Salmonella spp.was determined by serology agglutinating method. Antimicrobial susceptibility was determined by K-B disk diffusion method and MICs of cefotaxime and ceftazidime were measured by agar dilution method for Salmonella spp. isolates. PCR and DNA sequencing were used for detecting ESBL, ISEcpl and AmpC genes; The transfer of cefotaxime resistance was determined by conjugation experiments. PFGE was performed for determining the homogeneity of the S. typhimurium isolates. Results A total of 72 isolates of Salmonella spp. were collected, among which S. typhimurium accounted for 86 % (62/72) and was the main serum type. S. typhimurium isolates and S. thompson isolates were often resistant to most of clinically used antimicrobial agents. Resistance of S. thompson isolates to ampicillin was the highest (90%, 56/62),followed by tetracycline (81%, 50/62), trimethoprim/sulfamethoxazole (74%, 46/62) and chloramphenicol (66%, 41/62). Seventeen S. typhimurium isolates (27%, 17/62) and two S. thompson isolates were resistant to cefotaxime. Forty-nine S. typhimurium isolates and two S. thompson isolates were positive for blaTEB-1b and resistant to ampicillin. Thirteen ESBL-producing S. typhimurium isolates (21%, 13/62) were positive for blaCTX-M (eight for blaCTX-M-14, three for blaCTX-M-15, one for blaCTX-M-55, one for both blaCTX-M-14 and blaCTX-M-55). All isolates harboring blaCTX-M genes were positive for upstream insert sequence ISEcpl. blaDHA-1was detected in a cefoxitin-resistant S. thompson isolate. Two main clones (PFGE type A and D) accounting for 19% (12/62) and 50% (31/62) respectively were found among 62 S. typhimurium isolates. Seven CTXM-producing isolates belonged to PFGE type D. Conclusions The multi-resistance to antimicrobial agents and high prevalence of blaCTX-M genes are found among S. typhimurium and S. thompson clinical isolates. blaCTX-M-55 is first found in S. typhimurium isolates and blaDHA-1 is found in S. thompson isolates. Clonal spread is responsible for the dissemination of S. typhimurium isolates.
