Objective To investigate the aggressive signs of benign spinal lesions appearing on medical imaging and their impact on diagnosis.Methods 139 cases of benign spinal lesions with aggressive signs confirmed by pathology of needle aspiration or surgery were reviewed,including 18 cases of osteoblastoma(OB),12 cases of aneurysmal bone cyst(ABC),14 cases of osteoenchondroma (OC),19 cases of Langerhans cell histiocytosis (LCH),15 cases of hemangioma (HA),34 cases of tuberculous spondylitis (TS),and 27 cases of pyogenic spondylitis (PS).All patients underwent radiography,119 cases CT plain scan,75 cases MRI scan,and 57 cases performed all the three imaging modalities.The aggressive signs,including bulging of posterior margin of the vertebral body,pathological compression fractures,ill-defined boundary,abnormal soft tissue mass,bone marrow and soft tissue edema were showed.The benign and malignant misdiagnosis rate,the consistent rate of diagnosis with pathology were statistically analysed.Results Bulging of posterior margin of the vertebral body were found in 2 cases of OB,1 case ABC,3 cases LCH,1 case OC,6 cases HA,6 cases TS,2 cases PS.Pathological compression fracture were found in 6 cases of OB,10 cases ABC,16 cases LCH,4 cases HA,21 cases TS,16 cases PS.Ill defined boundary were found in 3 cases of OB,8 cases HA,34 cases TS,27 cases PS.The abnormal soft tissue around spine were found in 6 cases of OB,2 cases ABC,15 cases LCH,10 cases TS,15 cases PS.Bone marrow and soft tissue edema were found in 5 cases of OB,4 cases ABC,10 cases LCH,4 cases HA,30 cases TS,27 cases PS.For benign and malignant misdiagnosis rate,MRI was better than CT(P＜ 0.05).For accuracy of the consistent rate with pathology,CT was better than MRI(P＜0.05).The integrated application of the three imaging methods could significantly improve diagnostic accuracy (P＜0.05).Conclusion The imaging features benign spinal lesions are various,which may be associated with aggressive signs.A comprehensive method combined with three kinds of imaging methods,is a simple and feasible way to avoid the misdiagnosis.

Objective A comparability analysis was performed between the various parameters of the self-developed dry-type blood cell analyzer with the reference instrument (Sysmex XT-1800i).Methods In line with the evaluation program of blood cells issued by ICSH,the detection results,six parameters as HCT,Hb,GRAN (granulocytes),LM (lymphatic and monocytes),WBC and PLT,were compared between the two instruments.Some indexes of the subject instrument such as precision,comparability,outlier points,bias,etc.were analyzed and compared so as to calculate the equation of linear regression and the expected bias intervals.Results The intra and inter CV of the subject instrument about six parameters were respectively less than 5％ and 6％,so it showed that the instrument had good precision.Correlation analysis showed that the classification results of HCT,Hb,GRAN and WBC had a good correlation (r =0.991,0.972,0.986,0.975,P ＜0.01),while that of LM and PLT were slightly lower (r =0.952,0.942,P ＜0.01) ; As to the medical decision level,HCT values were 14％ and 70％,WBC 0.5 × 109/L and 3 ×109/L,and PLT 10 × 109/L,50 × 109/L and 100 × 109/L,and the acceptable bias of these values all fell in the confidence intervals; the values of the rest items were slightly higher than the high value of the confidence interval of expected bias.Conclusions The subject instrument produces the same results in the test of its parameters as the reference instrument Sysmex XT-1800i and therefore the two instruments are comparable.The subject instrument is particularly suitable for field rapid detection.

Objective:The present study aimed to conduct a clinical analysis of nosocomial infections in hospitalized cancer pa-tients and propose effective precautionary measures. Methods:We retrospectively analyzed 56,430 cases of discharges from the Cancer Institute and Hospital of Tianjin Medical University between January and December 2012. Results:Among 825 cases, the rate of noso-comial infections was 1.46%;46.42%of the patients aged>60 years. The highest infection rate was observed in the Medical Depart-ment of Stomach Cancer. The highest composing rate was recorded in the Department of Hepatobiliary Cancer. The major pathogenic bacteria were Gram-negative bacilli;fungal infections were relatively rare. The most vulnerable body parts were the abdomen and diges-tive tract, which were mainly infected by pathogenic Escherichia coli. Conclusion:Healthcare workers should consider the importance of influencing factors and take precautionary measures to reduce the rate of nosocomial infections in cancer patients.

Objective To observe contents and effect of total flavonoids of acanthopanax senticosus extract on erythrocyte membrane fluidity.Methods The contents of total flavonoids of acanthopanax sentieosus were deter- mined by spectrophotometry,and erythrocyte membrane fluidity and microviscosity were measured by fluorescence polarization technique.Results The total flavonoids contents of acanthopanax senticosus extract and injection were alike.Both of them increased erythrocyte membrane fluidity and lowered microviscosity,and the effect of acan- thopanax senticosus injection was more significant.Conclusion Total flavonoids of acanthopanax senticosus can in- crease erythrocyte membrane fluidity,so that blood viscosity is lowered and hemorheology factors are improved.

Purpose To investigate the diameter change of dorsal root ganglion (DRG) in lumbar disc herniation using three-dimensional MR neurography. Materials and Methods Sixty-ifve patients with lumbar disc herniation and 30 healthy volunteers were selected. Bilateral DRG diameter was measured using MR three-dimensional constructive interference steady state (3D-CISS) sequence at the level of L3-S1 in the control group and at the level of herniation disc in patient group including central and lateral subgroups. The relationship between the sagittal index and DRG diameter at the level of herniation disc was analyzed. Results In the control group, the DRG diameters increased from the level of L3 to S1. The DRG diameters of the central subgroup were bigger than those of the control group (t=-2.485--2.253, P<0.05). The DRG diameters of the lateral subgroup were bigger on the diseased side than the contralateral side (t=1.956-2.432, P<0.05). The DRG diameters of the contralateral side in lateral subgroup were slightly bigger than those of control group without statistical signiifcance (t=-1.248--0.981, P>0.05). The sagittal index was not correlated with DRG diameter. Conclusion 3D-CISS sequence clearly demonstrates morphological changes of lumbosacral nerve root and measures its diameter.

Objective To develop a method for determination of sirolimus in human whole blood by HPLC -MS/MS.Methods The whole blood samples using ascomycins as internal standard were treated with zinc sul-fate and acetonitrile.After precipitation, the supernatant 5μL was injec-ted for analysis.The used column was Extend -C18 column (2.1 mm × 50 mm, 3.5 μm), mobile phase consisted of 10 mmol· L-1 ammonium acetate solution ( A) and methanol ( B) with a gradient elution.The flow rate was 0.4 mL· min -1 , the analyte and internal standard were detec-ted by multiple reaction monitoring scanning in positive ion mode using triple quadrupole mass spectrometer.The specificity , standard curve and lower limitation of quantitation , precision and recovery rate and stability as well as the matrix effect were investigated.Results The linear range of sirolimus was 0.5-100.0 ng · mL-1 ( r=0.998 3 ).The intra-day and inter-day RSD were both less than 10%.The extraction recoveries were 89.7%-91.6%.Conclusion The method is simple, rapid, sen-sitive and accurate for monitoring of whole blood concentration of siroli-mus and the pharmacokinetic study in human body.

OBJECTIVETo investigate the myocardial damage and changes of myocardial mitochondrial Mn-superoxide dismutase (Mn-SOD) activity in craniocerebral injured rats and the effect of Ginkgo biloba extract (GBE) on them.

METHODSCraniocerebral injured rats model was established by fluid-percussion and treated with GBE. The dynamical changes of electrocardiograph (ECG) in 24 h were monitored, the serum level of MB isoenzyme of creatine kinase (CK-MB) and the change of myocardial mitochondrial Mn-SOD activity as well as the pathologic changes of myocardium (HE staining) were observed.

RESULTSThe occurrence of ECG abnormality obviously increased in the injured rats, accompanied with increased serum CK-MB (P<0.05) and decreased myocardial Mn-SOD levels (P<0.05), and the Mn-SOD activity was negatively correlated with the level of CK-MB (r=-0.997, P<0.05). Pretreatment of GBE resulted in the decrease of ECG abnormality occurrence (P<0.01), serum CK-MB level (P<0.05), and degree of myocardial damage, as well as the increase of Mn-SOD activity in post-craniocerebral injured rats.

CONCLUSIONSCraniocerebral injury can result in distinct myocardial damage, which is possibly correlated with the lowering of anti-oxidation stress level of myocardial cellular mitochondria. GBE possesses the protective effect on myocardial damage after craniocerebral injury.

Objective To explore MSCT optimal examination time window for patients with occult rib fracture, and provide objective evidence for forensic injury. Methods Totally 105 cases with chest trauma were retrospectively analysed. They were examined in the first week after trauma and re-examined in different time windows by MSCT. The quantities of occult rib fractures in the first examination were compared to those in re-examinations. Results The quantities of occult rib fractures at different inspection time windows were mostly different. There was no statistically significant between the quantities of rib fractures in the first week and the second week, the first week and the sixth week later, the fourth week and the fifth to sixth weeks (P>0.05). There was statistically significant between the quantities of rib fractures in the first week and the thrid to sixth weeks, the second week and the third to sixth weeks, the third week and the fourth to sixth weeks (P<0.05). Conclusion The result of the study imply that the fourth to sixth week is the optimal time window of MSCT examination in the occult rib fracture.

Objective To study the relationship between multidrug re-sistance gene(MDR1)and cytochrome P450 2C19(CYP2C19)gene poly-morphisms and the effect of clopidogrel in patients with acute coronary syndrome ( ACS ) after undergoing percutaneous coronary intervention ( PCI ) .Methods The patients with ACS who were undergoing PCI were divided into three groups according to the result of MA ADP which tests by thromboelastogram ( TEG).Group A consists of patients with MA ADP >47, group B consists of patients with 31≤MAADP≤47 and group C con-sists of patients with MA ADP <31, respectively.The genotype of CYP2C19 is decided by gene chips hybridization and the genotype of MDR1 is decided by gene sequencing.Then the distribution of gene poly-morphism between were compared with different groups.Results There were no significantly differences among three groups in different genotype of MDR1( P>0.05 ).The proportion of patients with poor metabolism of CYP2C19 in group A was higher than the other two groups significantly ( P<0.05).Conclusion CYP2C19 gene polymorphisms affect the an-ticoagulant effect of clopidogrel.
