1.Deepen Clinical Teaching Reformation Under the Employment Oriented Guidance

College students' employment viewpoint has transferred ultimately from traditional"government-planning assignment" to modern job-seeking mechanism of "two way choice and independent option".Recently,medical college has reformed clinical teaching in 5-years program.But some problems still exist in clinical teaching reflected from the employment situation of medical graduates and the evaluation of recruiting employers.Some work can be done to solve problems existing in medical students' employment.It is an important issue in adapting social requirement through reforming the model of clinical teaching and arranging the period of clinical training reasonably.

6.Relationship between Air Pollution and Acute Respiratory Symptoms in Children in Taiyuan

Objective To investigate the relationship between air pollution and the acute respiratory symptoms in children in Taiyuan,China.Methods A case-crossover design was used to study the association between air pollution and the acute respiratory symptoms of 254 children aged 7-11 in a school in 2005.Results The acute respiratory symptoms(cough,nose running,stuffy nose and angina)increased as the air concentration of PM10 increased.Conclusion There seems to be a positive association between air pollution of PM10 and the acute respiratory symptoms in children in Taiyuan.
7.Development of laparoscopic hernia repair and prevention and treatment of complications

Ventral hernia is a very common surgical problem with its incidence gradually increasing due to aging population,and it has significantly threat to quality of life.Laparoscopic hernia repair is a kind of minimally invasive surgery based on tension-free hernia repair,which has become an effective way for treatment of ventral hernia after decades of development.Besides,the classic surgical approaches for adult inguinal hernia include intraperitoneal onlay mesh,transabdominal peritoneal and totally extraperitoneal repairs.However,some related complications including vascular injury and bleeding,postoperative pain,nerve damage,seroma,intestinal obstruction,infection and relapse have been increasingly reported with the extensive applications of laparoscopic ventral hernia repair.Surgeons still need to pay more attentions and take appropriate measures to prevent such events.Complications could be reduced by strictly following the indications of different laparoscopic hernia repair surgeries with a careful consideration of the advantages and disadvantages,understanding their pathogenesis,improving the laparoscopic operation techniques,getting familiar with the local anatomical structures and standardizing the surgical procedures.Establishing a correct and standard surgical training system will further shorten the learning curve of surgeons to promote the progression and development of hernia surgery in China.
8.Risk Factors of Nosocomial Infections Caused by Vancomycin-resistant Enterococcus

OBJECTIVE To investigate the risk factors for vancomycin-resistant Enterococcus(VRE) infections and their effective isolation measures.METHODS The data of 21 cases of VRE nosocomial infections were analyzed from Jul 2003 to Dec 2005 in Beijing Chaoyang Hospital;28 cases of antibiotic-sensitive Enterococcus infection were randomized as control.T test,chi-square test and Logistic regression analysis were used for statistics.Strict measures were taken to all of the VRE infected patients.RESULTS According to univariate analysis,the factors associated with the development of VRE nosocomial infection were age,in ICU,accepted invasive operation,Acute Physiology and Chronic Health Evaluation Ⅱ(APACHEⅡ),live in hospital more than 30 days,co-infection with other pathogens,and fluoroquinolone and vancomycin/norvancomycin use 15 days before isolation of VRE.Multivariate Logistic regression analysis identified two independent factors: accepted invasive operation and previous vancomycin/norvancomycin use.Spreading of VRE had not occurred.CONCLUSIONS Accepted invasive operation and previous vancomycin/norvancomycin use are independent risk factors for VRE infection.Effective measures can prevent the spread of VRE.
9.The value of emergency sonography in the diagnosis of patients with abdominal trauma

Objective To assess the value of emergency ultrasound in the diagnosis of patients with abdominal trauma. Methods Retrospective study was performed on 245 patients with abdominal trauma. All patients were examined with emergency ultrasound, and their sonographic findings were compared with their CT and operative findings. Results 1) Abnormal signs suggestive of injury were detected in 346 organs among 245 patients, and correct diagnosis was made in 299 cases, while misdiagnosis occurred in 47. The overall accuracy of sonographic diagnosis was 86.4%. 2) The accuracy rate of sonographic diagnosis of various organs: for splecn, liver, kidney, mesentery, bladder, retroperitoneal injuries were 93.4%, 91.9%, 80.9% and 51.3% respectively. 3) The accuracy of sonography on the free fluid and retroperitoneal fluid were 100% (213/213) and 89.5% (51/57) respectively. Conclusion Emergency sonography to evaluate patients for injury caused by abdominal trauma is highly accurate.
10.Expression of CD80?CD86 and CTLA-4 in Peripheral Blood Mononuclear Cells of the Patients with Systemic Lupus Erythematosus

Objective To investigate the role of CD80/CD86 and CTLA-4 in the pathogenesis of systemic lupus erythematosus(SLE) and its clinical significance. Methods Applying RT-PCR technique to semiquantitatively analyze CD80/CD86 and CTLA-4 mRNA expression on peripheral blood mononuclear cells (PBMC) from 32 active SLE patients. Results Compared with normal controls, the percentage of positive CD86 expression in active SLE, accounted for 90.63% (29/32), was significantly increased (P 0.05, both). Conclusion The abnormal expression of CD86 and CTLA-4 might play an important role in the pathogenesis of SLE.
