1.SCL-90 self-rating scale analysis on 278 cataract patients in pre- and post- operative periods

AIM: To know the psychological conditions of various cataract patients in pre- and post-operative periods.METHODS: From June 2008 to June 2009, 278 cataract patients had been asked to complete a questionnaire anonymously of their self-evaluation of symptoms scale (SCL) before and after operation. In their pre-operative and post-operative surveys, the results were compared with the norms respectively. Patients were divided into two groups. Group A were age-related cataract patients; while Group B were cataract patients with glaucoma, trauma or metabolic disease.RESULTS: When cataract patients' masculine scores of somatization, depression, anxiety and fear factors before and after the operation rank higher than the norms, the differences had statistical significance(P<0.05), especially the somatization factor(P<0.01).While cataract patients with diseases such as glaucoma, trauma or diabetes got higher marks than age-related cataract patients in the aspects of anxiety, somatization, depression (P<0.05), and fear factors, the differences were of statistical significance(P<0.01).CONCLUSION: Cataract patients both in pre- and post- and operative periods have somatization behaviors and emotions of depression, anxiety and fear. Cataract patients with glaucoma, trauma or diabetes especially anxiety have more obvious symptoms than age-related cataract patients.
2.Prevention of Peritoneal Adhesion Formation with Biomaterial-Degradable Polycaprolactone Film

Objective To investigate the effects of high molecular bio-degradable film on preventing peritoneal adhesion.Methods One hundred and twenty rats were divided into control group,non-bio-degradable film group and polycaprolactone(PCL) group.Animal models of peritoneal adhesion made by operations on rats serve as control group.In the study,non-bio-degradable film and bio-degradable,namely PCL films sensitive to pseudomonas enzyme,were put into the opening of peritoneum.The effect of the films on peritoneal adhesion was examined on 1,3,7 and 30 d after operation.Results Peritoneal adhesion was found in all the rats of control group and very low in PCL group and non-bio-degradable film group.Conclusion PCL film can effectively prevent the formation of postoperative peritoneal adhesion.
3.Analysis of the Clinical Effect of Dexamethasone Combined with Urokinase on the Tuberculous Pleurisy

Objective:To investigate the clinical effect of dexamethasone combined with urokinase on the tuberculous pleurisy.Methods:From August 2013 to May 2016,190 cases of tuberculous pleurisy patients in our hospital were selected.All the patients were randomly divided into the observation group and control group with of 95 patients in each group,both groups were treated with anti tuberculosis treatment,the control group was given urokinase treatment,the observation group was given dexamethasone combined with urokinase treatment,both groups were treated for 1 month.After treatment,the total effective rate,incidence of adverse reactions,total drainage time of pleural effusion,total amount of pleural effusion,thrombin time and prothrombin time of two groups were compared.Results:All patients were well tolerated with injection during the treatment and there was no severe complication after treatment;the total effective rates in the observation group and the control group were 88.4% and 72.6%,which was significantly higher in the observation group than that of the control group (P<0.05).The total drainage time and total amount of pleural effusion in pleural effusion in the observation group were 7.56± 2.44 d and 2867.33± 456.10 mL,the control group were 9.44± 2.89 d and 1989.92± 444.20 mL,the total drainage time in the observation group was significantly shorter than that of the control group,and the total amount of pleural effusion was significantly higher than that of the control group(P<0.05).After treatment,the pleural effusion thrombin time and prothrombin time in both groups were significantly higher than those before treatment (P<0.05),which were higher in the observation group than those of the control group (P<0.05).Conclusion:Dexamethasone combined with urokinase could prolong the thrombin time and prothrombin time,shorten the time of drainage of pleural effusion,increase the pleural effusion amount,with good safety and clinical effect in the treatment of tuberculous pleurisy.
4.Detecting the methylation of p16INK4A in primary hepatocellular carcinoma using a nested bisulfite sequencing-methylation specific polymerase chain reaction

Objective To detection the methylation of p16INK4A in primary hepatocellular carcinoma, a nested bisulfite sequencing and methylation-specific polymerase chain reaction (BS-MSP) protocol was designed and used.Methods Bisulfite-modified DNA were amplified to evaluate the quality of templates with a pair of bisulfite sequencing primers in the first round of PCR, then subjected to methylation assay with corresponding methylation or unmethylation specific PCR primers.Representative PCR products were sequenced to confirm its correctness.Results 3 of 40 cases (7.5%) were failed to assay due to poor quality of templates, and 29 of 37 cases (78%) were detected p16INK4A methylation.Sequencing results confirmed that templates were correctly amplified.Conclusion BS-MSP technique might be valuable for methylation study on carcinogenesis and clinical assay.
5.Design of portable integrated remote medical examination chest

Objective To design a portable integrated remote medical examination chest for telemedicine of the servicemen in remote areas.Methods The chest involved in a portable multi-parameter vital signs monitor,a 12-lead network ECG machine,an optical endoscope,an electrostethophone and etc,and applied medical information technology,biosensor technology,IOT,cloud technology and etc.The terminals for diagnosis and treatment,doctor as well as data uploading were developed,and the network storage,control and interaction of multi-type data were realized for the terminals.Results The chest implemented inter-connection and inter-communication between the elementary medical facilities and rear hospitals,and thus facilitated the telemedicine service for the troops in remote areas.Conclusion The chest executes integrated medical service of the central hospital and troops in remote areas,and thus is worthy promoting in the telemedicine system of the PLA.

7.Effect of hydrogen sulfide on rat cardiac function in vitro

AIM: To observe the effect of hydrogen sulfide (H_2S) on the rat cardiac function in vitro, to explorer the physiological regulation of endogenous H_2S on myocardial action. METHODS: H_2S concentration and production in the rat myocardial tissues were detected. The expression of CSE (a kind of key enzyme of endogenous H_2S production) mRNA in myocardial tissues was screened by RT-PCR. Langendorff apparatus was used to perfuse the rat heart in vitro. After 20 minutes of stabilization, NaHS (10~-6 -10~-3 mol/L) were added cumulatively in order to the perfusive fluid, and another group applied physiological concentration NaHS (4?10~-4 mol/L), continuously perfusion for 20 min, heart rate (HR), difference of left ventricular pressure (△LVP), left ventricular peak rate of contraction (+LV dp/dt_~max ), peak rate of relaxation (-LV dp/dt_~max ) and coronary perfusive flow (CPF) were measured at the times. 8.Effective arterial elastance in evaluating the fluid challenge in septic shock patients

Objective:To explore the validity of the effective arterial elastance (Ea) before and after fluid challenge in evaluating the fluid challenge in septic shock patients.Methods:A retrospective study was conducted in the medical intensive care unit (MICU) of Peking Union Medical College Hospital from October 2016 to October 2020. 116 septic shock patients were enrolled. All patients received fluid challenge by 500 mL Gelatin or normal saline under invasive hemodynamic monitoring. Heart rate (HR), mean arterial pressure (MAP), cardiac output (CO) and other hemodynamic variables were collected at 10 minutes before and immediately after fluid challenge. An increase in CO greater than 10% after fluid challenge was defined as the positive preload responsiveness, as well as the definition of positive pressure responsiveness was an increase in MAP greater than 10%. Receiver operating characteristic curves (ROC curves) were established to evaluate the predictive abilities of baseline Ea and other arterial load indices in detecting the preload responders and pressure responders. The correlation of the baseline Ea with CO changes after fluid challenge as well as MAP changes were tested by Pearson correlation analysis. Patients with positive preload responsiveness were divided into two groups according to the pressure responsiveness. The changes in Ea and other arterial load indices were analyzed.Results:A total of 116 patients were finally analyzed. Sixty-three patients were preload responders and 53 patients were preload non-responders. There was no significant difference in demographics and baseline physical variables between the two groups. Ea in preload responders was higher than that in preload non-responders (mmHg/mL: 2.51±1.08 vs. 1.87±0.68, P < 0.01). ROC curve analysis showed that the baseline Ea could predict the preload responsiveness at an area under ROC curve (AUC) = 0.71 [95% confidence interval (95% CI) was 0.62-0.81, P < 0.001]. The cut-off value was 1.97 mmHg/mL with a sensitivity of 71.4% and a specificity of 60.4%. The baseline Ea did not present the predictive ability to detect the pressure responders and pressure non-responders (AUC = 0.52, 95% CI was 0.41-0.63, P = 0.73). Pearson correlation analysis showed that the changes in CO after fluid challenge was moderately correlated to the baseline Ea ( r = 0.47, P < 0.001), meanwhile a weak positive correlation between the changes in MAP and baseline Ea was found ( r = 0.20, P = 0.03). In preload responders, 27 (42.9%) of 63 patients were pressure responders and 36 (57.1%) patients were pressure non-responders. No statistical difference was found in the baseline Ea or other arterial load indices between the two groups. Fluid challenge decreased Ea both in pressure non-responders and pressure responders (mmHg/mL: 2.13±0.94 vs. 2.51±1.08, P < 0.01; 2.47±1.18 vs. 2.69±1.30, P < 0.05). Moreover, the changes in CO and changes in MAP were strongly correlated with the changes in Ea ( r values were -0.50 and 0.58, respectively, both P < 0.001). Conclusions:The Ea > 1.97 mmHg/mL before fluid challenge could predict fluid responsiveness in septic shock patients. The baseline Ea was not able to predict the subsequent changes in arterial pressure through fluid challenge. A significant decrease in Ea inducing by fluid administration explained why patients increased their CO without improving blood pressure.
9.Preparation, Pharmacokinetics, and Tissue Distribution Properties of Icariin-Loaded Stealth Solid Lipid Nanoparticles in Mice

ObjectiveTo evaluate the difference of the pharmacokinetic (PK) and tissue distribution properties in mice administrated with lyophilized icariin stealth solid lipid nanoparticles (Ica-SSLN) modified by polyethylene glycol and icariin control solution (Ica-Sol).Meanwhile,to establish a sensitive,specific,and stable HPLC method for the determination of Ica in mice plasma and various tissues.MethodsIca-SSLN was prepared by high temperature melt-cool solidification method.Particle size and Zeta potentials were measured by a ZetaPlus.After iv administration of Ica-SSLN and Ica-Sol at a single dose of 7.46 mg/kg,the blood and tissues including brain,liver,spleen,lung,heart,and kidney were collected at different time points.The obtained concentration from HPLC analysis was statistically treated to determine the PK model and the relevant PK parameters using DAS1.0.Tissue distribution studies of Ica-SSLN were carried out in Kunming mice after iv administration and compared to Ica-Sol.Results The characteristic data showed that the mean particle size of Ica-SSLN was (50.03 ± 0.90) nm,entrapment efficiency was (71.67 + 1.09)%,and the particles carried negative charge,Zeta potential was (-22.77 ± 1.89) mV.The concentration-time profiles of Ica in mice after iv administrated with Ica-SSLN and Ica-Sol were shown to fit a two-compartment open model.Compared with Ica-Sol,the t1/2β of Ica-SSLN was prolonged by seven times and the AUC was increased by four times.The levels of Ica concentration in the kidney tissues were significantly increased.In addition,compared with Ica-Sol,the relative target efficiency to kidney tissue was 79% and the relative tissue exposure was 16.95.ConclusionIt demonstrates that Ica-SSLN has selective targeting to kidney tissue and the kidney targeted Ica-SSLN seems to have significant advantages and good development value.
10.Comparison of the effect between octreotide combined with omeprazole and omeprazole alone in the treament of upper gastrointestinal bleeding

Objective To compare the effect of octreotide combined with omeprazole and omeprazole alone in the treament of upper gastrointestinal bleeding.Methods Clinical data of 101 patients with gastrointesinal bleeding were retrospectively analyzed.According to the treatment,the patients were divided into the observation group(56 cases) and control group(45 cases).The patients in observation group were given octreotide and omeprazole,and patients in control group were given omeprazol alone.Then the results of the two groups were analyzed.Results The total effective rate of observation group was 91.07%,and was higher than that of the control group(73.33%) (x2 =5.61,P <0.05) ;The average hemostasis time of observation group was (15.9 ±3.9)h,and was shorter than that of the control group [(23.5 ± 4.7) h] (t =1.937,P < 0.05) ; The hemoglobin level of the observation group was (116.7 ± 12.4) g/h after 72 hours treatment,and was higher than that of the control group [(91.5 ± 17.3) g/h] (t =2.013,P < 0.05) ;The pH of observation group was higher than that of control group after 12h,24h,48h and 72h treatment(all P < 0.05).Conclusion Octreotide combined with omeprazole is effective for upper gastrointesinal bleeding,and it can shorten the bleeding time and improve the cure rate.
