1.A Case of Primary Ovarian Pregnancy.

Although tubal pregnancy is increasing, primary ovarian ectopic pregnancy has remained a rare event. However, recent reports suggest an increasing incidence to both tubal and term pregnancies. Ovarian pregnancy occurs within the ovary and on the corpus luteum. Earlier diagnosis is now possible, owing to the availability of highly specific radioimmunoassay for human chorionic gonadotrophin and the development of transvaginal ultrasonography. Clinical and even intraoperative diagnosis is difficult and confirmation may be made only by microscopic examination of the tissue specimen. Current understanding of the etiological factors, classification, possible pathogenesis, clinical presentation, diagnostic steps, reevaluation of diagnostic criteria, preferred management and future fertility are detailed. The therapy is surgical and currently more conservative than in the past, because of improvement in operative laparoscopy. We report a case of primary ovarian pregnancy treated conservatively under laparoscopic surgery with a brief review of literature.

2.Polycythemia as a Complication of Long-acting Injectable Testosterone Undecanoate.

Polycythemia is a condition in which the red blood cell count is increased due to an inherited or acquired mutation, a physiologic response to hypoxia, autonomous erythropoietin production, or deliberate erythropoietin administration. Higher testosterone levels appear to act as a stimulus for erythropoiesis and testosterone replacement therapies have rarely been reported as causes of polycythemia. We report here a case of a 51-year-old man with polycythemia that was caused by long-acting testosterone undecanoate (Nebido(R)).

3.Biochemical Analysis of Inter-Connection between Types of Nuclear Opacity, Oxidative Stress and Glycation in Cataractous Lens.

PURPOSE: To elucidate the inter-connection between lens nuclear opacity and its proposed two main factors, which are oxidative stress and glycation reaction initiated with sugars and proteins. METHODS: We have collected clinical samples after cataract surgery from 57 patients during 3 month period. We also classified these samples into three groups as initial, intermediate and advanced stages, and performed the biochemical assays for measurements of glutathione(GSH) and advanced glycation end products(AGEs). RESULTS: It showed that the concentrations of GSH in lens soluble proteins were 0.022+/-0.019, 0.017+/-0.011, and 0.011+/-0.010 nmoles/mg lens protein in initial stage, intermediate stage and advanced stage, respectively. Fluorophore formation(Ex370/Em440), which has been reported to represent general AGEs formation, was positively correlated with nuclear opacity showing that 108.8+/-48.3, 174.7+/-116.9, and 188.2+/-130.6 Fluorescence Unit(F.U.) in initial stage, intermediate stage and advanced stage, respectively. Likewise, a marker for AGEs, pentosidine-related fluorophore(Ex335/Em385) was increased with nuclear opacity(67.0+/-30.8, 75.5+/-36.4, and 80.4+/-41.7 F.U. in initial stage, intermediate stage and advanced stage, respectively), although significant differences existed between groups. CONCLUSION: Our results showed that there are clear inter-connection between lens nuclear opacity, oxidative stress and the formation of general AGEs compared with pentosidine, and suggested that therapy using antioxidants, chelating agents, or glycation inhibitors could be beneficial in delaying nuclear cataractogenesis.

4.ACase of the Patau Syndrome Diagnosed in Second Trimester.

Patau syndrome is trisomy 13, one of the most common autosomal aberration associated with multiple congenital anomalies. Because trisomy 13 is generally associated with severe congenital anomalies and postpartum poor prognosis, antenatal diagnosis through antenatal ultra-sonogram and triple screening marker is very important. We present one case of trisomy 13 with abnormal ultrasound finding, holoprosencephaly, microcephaly, cleft lip and palate. And confirmed chromosomally with pregnancy termination.

5.Antitumor Effect of in Situ Cryoablation with Systemic Immunotherapy on Murine Renal Cell Tumor.

PURPOSE: To investigate synergistic effect of local cryoablation with systemic immunotherapy, the tumor control ability and immunologic responses of combining these two modalities was compared with that of cryoablation, surgical excision, and immunotherapy only group in a tumor re-challenge model. MATERIALS AND METHODS: Preliminary experiments were performed in two stages. The first stage consisted of 36 Balb/c mice with Renca bearing tumors imbedded in the right thigh, and was treated with interleukin-2 (IL-2) and interferon-alpha(IFN-alpha) to evaluate the efficacy of immunotherapy and to determine the adequate dosage. The second stage was performed on 10 mice, to evaluate histological changes and efficacy after cryoablation. The main experiment was performed on 48 mice, divided into 6 groups of control with tumor implantation, excision of tumor, excision combined with immunotherapy, cryoablation of tumor, cryoablation with immunotherapy and control without tumor. After treatment, tumor re-challenge was performed with Renca cell, then the growth pattern was evaluated with physical measurements, and immune response was investigated with fluorescent activated cell sorter and cytotoxicity assay. RESULTS: Preliminary studies on immunologic efficacy revealed that IL-2 and IFN-alpha have a dose dependent inhibition of tumor growth. The main experiment evaluating the efficacy of combination treatment revealed that cryoablation with immunotherapy proved to be most effective in terms of tumor recurrence and tumor growth inhibition, yet the difference was not statistically significant from monotherapy with cryoablation. However, cytotoxicity was significantly increased cryoablation with immunotherapy compared with other groups. CONCLUSIONS: Cryoablation on tumor re-challenge mice model showed advantages with immunotherapy most prominently in cytotoxicity.

6.Behavioral and Emotional Problems in Children with Nocturnal Enuresis: Results of a Multicenter Study.

OBJECTIVES: The aim of this study was to examine the behavioral and emotional problems associated with nocturnal enuresis in Korean children. METHODS: Three hundred eighteen children with nocturnal enuresis, together with their parents, completed the Child Behavior Checklist (CBCL), Disruptive Behavior Disorder Scale according to DSM-IV (DBDS), Children's Depression Inventory (CDI), State-Trait Anxiety Inventory for Children (STAIC), and Piers-Harris Children's Self-Concept Scale (PHCSC). Ninety-three normal students were selected as the control group. RESULTS: Compared to the normal control group, the mean scores with regard to the withdrawn, social problems, attention problems, delinquent behavior, aggressive behavior, externalizing problems and total problems profiles were significantly higher in the nocturnal enuresis group according to the CBCL results. The nocturnal enuresis group also scored significantly higher in the ADHD and ODD profiles of the DBDS. The nocturnal enuresis group was more depressed and anxious than the control group according to the results of the CDI and STAI. The mean score of the PHCSC was significantly lower in the nocturnal enuresis group when compared to the normal control group. CONCLUSION: The results of this study suggest that children with nocturnal enuresis in Korea have clinically relevant behavioral and emotional problems. The findings support the link between nocturnal enuresis and psychopathology in Korean children.
